However, we will always see urgent consultations the same day at the Moir Medical Centre. For a non-urgent appointment, it may be quicker to find a particular doctor at a surgery other than the one you normally use - shop around! (Please ask for your records to be transferred from wherever they are usually held to where you wish to be seen.) All surgeries are wheelchair accessible, including the toilets. TELEPHONE ACCESS To help reduce costs for our patients, we have introduced a direct dial phone number to our appointment desks at all three of our sites and these are now advertised within the practice. In working hours (8.00am to 6.30pm) a receptionist will take your call. If you need urgent medical attention when the surgery is closed you should dial 111. This will connect you to the NHS 111 service free of charge, which is provided by Derbyshire Health United. You will be assessed and directed to the most appropriate point of care for your particular needs. You can also dial 111 if you require health information. WE ARE A TEAM There are 18 receptionists and administrative staff, headed by our practice manager, Mr Steve McHale. There are also two advanced nurse practitioners, seven practice nurses, two healthcare assistants, midwives, community nursing staff, health visitors and six doctors. All of these people are in an on-going process of re-education and training and between us we offer a wide range of skills to help with your problems. Our receptionists work hard to act as a bridge between you and those providing health care to you and do so under the guidelines laid down by the doctors. Please stick to one doctor to work a problem through. Should that doctor not be available (because they are working at a branch surgery or on a study day etc) in an emergency another doctor will always be able to see you that day. Occasionally there may be visiting doctors or students (eg medical, pharmaceutical, health visitors, community nurses or midwifery) who would be grateful if they could join in your consultation. Of course, you are completely free to exclude anyone from the consultation and the receptionist will warn you of any possible visitor in order to give you this right. During the week all the members of the team can be found at the main surgery at some time or other and appointments can be made to see them. TO SEE A DOCTOR We use an advanced access appointment system which means patients should be able to access a GP within 48 hours and ideally on the day of their request. A few appointments for each doctor (six morning and afternoon) are available up to three weeks in advance; the remainder are booked from 8.00am the same day. A few appointments for each doctor are available to book online. To access this service you will need to register at the surgery for SystmOnline. There will be times though when specific GPs are not available within the 48 hours but at least two doctors are available each day in Long Eaton. There are short and long appointments available, but the system is always under review to match it to your needs. If you need a longer appointment (for example if you are going to tell us that you are pregnant) please let the receptionist know. Sometimes a patient does not actually need to see a doctor on a face-to-face basis and each doctor now has, built into their surgery, telephone appointments when they speak to the patient at an agreed time. In an EMERGENCY you will still be seen even if there are no appointments, but you may have to wait or go to the Moir Medical Centre. Please tell us if you are taking any medication, vitamins or health products. CAR PARKING The car park at the Moir Medical Centre is ONLY available to practice team members. There is of course a shoppers car park only 30 seconds away off Lawrence Street for your convenient usage.

Visits are allocated at the discretion of the on-call doctor for that day, and only carried out if your condition would deteriorate if you travelled to the surgery. You will need to call before 10.30am each morning. The on-call doctor will call you back to discuss your case with you. If you are medically fit to travel you will be asked to attend the surgery. You do not automatically qualify for a visit if you are an elderly person or young child. Patients who do not have their own cars should seek the help of family, friends and neighbours or use public transport. It is not appropriate to ask the doctor to visit you if you do not have enough money to use public transport. It is also inappropriate to ask for a visit because you would otherwise find it inconvenient to get to the surgery. ZERO TOLERANCE In keeping with the rest of the NHS, this practice operates a zero tolerance policy with respect to the protection of all its staff. This means that anyone who is violent or abusive in any way to any member of staff may be removed from the practice list with immediate effect and without a second chance. Extreme cases will result in being reported to the police. This applies to all patients and all members of staff, not just the GPs, and includes contact over the phone. SOME OF WHAT WE OFFER We will help, if we can, to solve your problems and this will not always need a prescription! Advanced Nurse Practitioners We have three experienced advanced nurse practitioners working at the practice; Rebecca Taylor, Kathy Joynes and Kelly Rushby. They can deal with many problems that you may have previously seen a doctor for. They can prescribe most types of medication. They cannot see children or women with gynaecological problems. If needed, they will liaise with a GP or other member of staff to provide you with the optimum treatment. Treatment Room And Chronic Disease Management Working with us at the Moir Medical Centre there are at present seven practice nurses: Sister Jo Ranson, Sister Shirley Simms, Sister Angela Watson, Sister Kelly Rushby, Staff Nurse Penny Jones, Staff Nurse Lisa Adams and Staff Nurse Alison Bayliss. They offer a variety of services including screening for and managing blood pressures, diabetes, asthma, coronary heart diseases and COPD.

They carry out cervical smear screening and offer contraceptive services (following initial consultation with a GP), travel advice, immunisations, wound care and ear syringing. We offer an important influenza immunisation programme every autumn (from October/November) as well as pneumonia protection, which is available all year. They are also able to give advice on cystitis and thrush, amongst other complaints. Our nurses can offer you pre-conceptual advice including screening for Rubella. Health Visitors There is a team of health visitors working within the Long Eaton area and a further team working in Sawley. All families have a named health visitor, but may see any one of the team. What Is A Health Visitor? A health visitor is a qualified nurse who has completed additional training in order to use the title of health visitor. The health visitors work as part of the Primary Health Care Team to support families with children. Where To Find A Health Visitor The health visiting team are based at the Long Eaton Health Centre, Midland Street, Long Eaton and can be contacted by telephone on 0115 855 4091. Child Health All families with children under five years old have a named health visitor. Health visitors offer a wide variety of services, for example, home visiting, information on child immunisations, postnatal support, child health promotion, support groups and many others. For our patients who live in Sawley, the health visitors run a community clinic and play sessions from 10.00-11.30am each Friday at the Sawley Community Centre, Draycott Road, Sawley. Community District Nurses Our community nurse team is now based at Long Eaton Health Centre. Community nurses work seven days a week 8.30am - 6.00pm and can be contacted via the District Nurse Liaison Office on 01332 258200 who will then contact a district nurse on your behalf. The criteria for district nurse visits are similar to those of the doctors in that they aim to provide nursing care only to those patients who are genuinely unable to attend the surgery for their treatment. Please note that when a patient is fit to attend the surgery they will be asked to do so. The nurse will arrange initial prescription items but subsequent prescriptions will need to be obtained by the patient. Please be aware that district nurses are not permitted to carry prescription items ie medication/tablets. Antenatal Care Midwives run their own clinics at the Moir Medical Centre. Most women will receive the majority of their antenatal care from the midwife. If your midwife feels it is necessary for you to see a GP, you will need to make an appointment during normal surgery hours. Long Eaton Wednesday 9.00am - 4.30pm Amanda Newman Thursday 9.00am - 1.00pm Sawley Tuesday 9.00am - 4.00pm Relaxation and parentcraft classes are held at the Long Eaton Health Centre. Your midwife will let you know the time and date of these. Aquanatal sessions are held at West Park Leisure Centre on Fridays 3.00 4.00pm (times change during school holidays). All are welcome from 12 weeks of pregnancy. Contraception Between us we offer a full service including coils, emergency contraception, caps, pills etc. Immunisations Adult immunisations including holiday vaccinations are usually available through our practice nurses. Please note that for holiday vaccinations, we sometimes need to give the vaccination up to five weeks before travelling. For complicated itineraries/last minute requests, you may be asked to attend a non-nhs (fee paying) service. We also have a specialist clinic for routine childhood immunisations: Moir Medical Centre - Wednesdays between 1.50-4.40pm For further information regarding childhood immunisations please visit www.immunisation.nhs.uk Minor Operations We can help you with a wide range of procedures. These are carried out at our Sawley site and you should book your appointment with the receptionist; this will usually be of 15 minutes' duration or more.

Repeat Prescribing For a number of medical problems we feel it is not necessary to see you each time you need a fresh supply of medication. However, as a rule it would be unwise to continue to take medication for longer than one year without review by your doctor. You will appreciate that we require time to prepare your prescription (generally a 28-day supply will be given) and we need two working days' notice of your requirements. We provide a counterfoil to hand in with your request for a repeat and this makes it easier for us to produce the prescription quickly. The prescription can be picked up at reception or we can send it to you by post if you provide us with a stamped, addressed envelope. To avoid error and reduce telephone congestion, we do not accept requests for repeat prescriptions by telephone. Prescriptions can also be requested using SystmOnline by clicking the link on our practice website, you will need register for this service at reception. (If you regularly use more than two items per month you may find it cheaper to buy a pre-payment certificate.) SPECIFIC CLINICS Our practice nurses are pleased to be able to offer a wide range of clinic services to help you stay fit and well. These are always in evolution but currently are in the form of: asthma, diabetes, COPD, hypertension and other chronic disease management clinics. Our healthcare assistants are pleased to be able to offer a range of services in the form of blood taking, blood pressure checks, suture removal and ECGs If you are sent a reminder letter to attend surgery please have it to hand when booking as this will help to ensure you are booked into the correct clinic. NHS DIRECT NHS DIRECT 24-HOUR TELEPHONE NUMBER 0845 46 47 When you contact this number an advisor will answer your call and be able to help and advise you. PATIENT ADVICE AND LIAISON SERVICE The Patient Advice and Liaison Service (PALS) has been set up to focus on improving the services offered to NHS patients. The service aims to: provide information on NHS services listen to your concerns, suggestions or queries help sort out problems quickly on your behalf The practice has PALS link workers at each surgery or you may contact the PALS Information Officer direct on the freephone number 0800 783 7279. You can also obtain information via the website at www.derbyshirecountypct.nhs.uk ANY COMPLAINTS? Do let us know and we will try to sort them out. Please ask for a complaint form or discuss the problem with our practice manager or with the doctor you know best. CONFIDENTIALITY You can be sure that anything you discuss with any member of this practice - family doctor, nurse or receptionist, will stay confidential. Even if you are under 16 nothing will be said to anyone, including parents, other family members, care workers or tutors, without your permission. The only reason why we might have to consider passing on confidential information without your permission, would be to protect you or someone else from serious harm. We would always try to discuss this with you first. If you are being treated elsewhere, for example at a hospital or Brook Centre, it is best if you allow the doctor or nurse to inform the practice of any treatment you are receiving. If you have any worries about confidentiality, please feel free to ask a member of staff.
