1. Transfer to Leaving Care Team Guidance To be completed by Children s Social Care Social Worker Transfer to the LCT document on ICS to be completed as soon as the eligible child is 15 years of age. This will send a trigger to LCT admin for notification. (This document will help inform allocation to LCT SW) Invites to the next LAC Review will be sent to Leaving Care Team. Updated Core Assessment will be completed and presented at the LAC Review. Court Report to be completed for discharge of Care Order if part of the Child Plan. CSC SW to arrange a Transfer Meeting to be held 1 month before the eligible child reaches 16 years of age in discussion with their Team Manager. Invites to the Transfer Meeting will be sent to all relevant people, including LCT SW, Senior Practitioner, LCT and Team Manager from LAC SWS. Introductions of the young person to LCT worker will be arranged and agreed with the young person. 2

Transfer Meeting to be chaired by Team Manager from the referring team. All relevant information about the young person shared. Outstanding tasks to be allocated to a named worker. SW to complete 1 st part of Pathway Plan and Transfer Record on ICS. Date case and files to be transferred agreed. Record of discussion taken and distributed by CSC Team Manager. File Audit by Children s Social Care Team Manager. Case Transferred at agreed date. 3

2. Leaving Care Team Guidance To be completed by the Leaving Care Team Worker Transfer Document on ICS accepted from Children s Social Care (CSC) Social Worker (Manager s/senior Practitioner s) (Name of allocated SW agreed) to be available on ICS. LCT worker/ Manager will attend the LAC Review of the YP held when the YP reaches 15 years of age and provide information about the service and what the YP can expect to be provided with. (LCT SW/Manager will gather information to inform allocation of case to LCT SW). (The allocated SW from the LCT will be introduced to the young person around the age of 15.5yrs to smooth the transition. The LCT SW will offer 3-4 appointments with the young person prior to transfer at 16). (The allocated SW from the LCT will attend the pre-16 LAC review if available). A Transfer Meeting date will be arranged by the referring SW and will be held one month before the eligible child reaches 16 years old. 4

Manager of the LCT/Senior Practitioner attends the Transfer Meeting Allocation to named worker confirmed. Date of Introductions agreed. Allocation of tasks to include discharge of Care Order and timescales agreed. (CSC SW will complete the Court Report should this be the LCT agreed worker Plan). will formulate a Pathway Plan based on an assessment of the individual needs of the YP. Referral to relevant/other agencies will be considered. Links will be made to all relevant agencies. Transfer date will be agreed. LCT worker will arrange visits to alternative resources and accompany the YP, if appropriate. LCT worker will carry out ongoing risk assessment/management to establish changes in circumstances and support needed should the YP return to live with family. LCT to hold a Care Team Meeting two weeks prior to LAC/Pathway Plan Review. LCT worker will complete LAC Documents and present to the IRO 7 days prior to the LAC/Pathway Plan Review. LCT worker will present an up to date and completed Pathway Plan to the IRO 7 days prior to the LAC/Pathway Plan Review LCT worker will send invites to relevant participants to LAC/Pathway Plan Review LCT to attend the LAC/Pathway Plan Review at least 6 monthly 5

Children s Social Care Social Worker to complete Court Report action timescale? 8. Complex needs of young person 9. Outstanding tasks completed by named worker timescale agreed. 10. Date of transfer agreed following completion of Transfer Record and all relevant documents on ICS and file audit by CSC Team Manager. 11. Referral to other service/s agreed named worker to carry out task and agreed timescale. 12. Financial support offered to young person by LCT. 6

Has the young person been placed with family and have relevant assessments been undertaken re; Kinship /Placement with Parents Regulations? Is there scope for reunification planning? When?
