Epsom and St Helier University Hospitals NHS Trust provides a range of medical services to approximately 420,000 people living across southwest London and northeast Surrey. In addition, it provides more specialist services, in particular renal and neonatal intensive care, to wider areas covering parts of Sussex and Hampshire. St Helier Hospital has six operating theatres in the main building plus two theatres in the separate Queen Mary s Hospital for Children. The Obstetric unit has two theatres and there is an additional theatre in X-ray. Epsom Hospital has seven theatres and a single theatre on labour ward. Surgical and anaesthetic specialties at Epsom & St Helier: Acute and chronic pain Colorectal surgery Day surgery Dental surgery Endoscopy Enhanced recovery ENT Eyes General surgery Gynaecology Major joint replacement (EOC) Orthopaedics and Trauma Paediatrics (Queen Mary s hospital) Paediatric MRI Priority surgery Renal access Simulation Spinal surgery Stabilising patients ready for the paediatric retrieval team Transferring critical ill patients cross-site and to specialist units Urology Trainees: Roughly 20 anaesthetic trainees from the St George s School rotate through the trust at any one time, predominantly at ACCS, core and ST5-7 levels. ACCS trainees with home specialties of EM and AM have a 6 month ICU module and a 6 month anaesthetic module with anaesthetic home specialty ACCS trainees completing 3 months ICU and 9 months anaesthetics. (All ACCS training is at the St Helier site). Core trainees spend 9 months at the St Helier site and 3 months at the Epsom site, with all aspects of basic level training covered. For specialist trainees at intermediate and higher levels, modules in intensive care, pain, paediatrics, management and the vast majority of general duties can be provided. Specialist trainees attached to the trust for one year will spend 6 months at the St Helier site and 6 months at the Epsom site. Advanced training in intensive care and pain is also provided within the trust. The trainee on-call rotas are currently all 1 in 8 (all EWTD compliant) and are paid at band 1A. There are on-calls rooms for all bleep holders at the St Helier site and an on-call room for the Obstetric bleep holder at the Epsom site. Core trainees do all their on-calls at the St Helier site, even when attached to the Epsom site for day time training. Exam preparation: Trainees are always allowed to take study leave for both private study and for courses, where possible. In addition to this, several members of the department are keen and experienced in providing viva practice for both the primary and final exams. OSCE practice is provided for the core trainees. Two members of the department are current/recent RCoA examiners. 3

Educational activities: Half-day, trustwide anaesthetic trainee teaching at the St Helier site (biweekly). Regional core trainee basic science teaching for pre-primary CTs (biweekly). ICU teaching for ICU module trainees (most weeks). Departmental meetings with journal clubs, M&M, consultant presentations (weekly) Trust Audit/Educational half-days (8 per year). Access to core, ST3/4 and ST5-7 Regional Teaching in addition to national and regional meetings etc. ALS, ATLS and APLS courses are frequently run by the trust.support Obstetrics: The St Helier obstetric unit has about 3300 deliveries per year. The unit has an attached midwife led birth centre and a recovery/hdu area on the delivery suite. The Caesarian section rate is 25% with separately staffed elective section lists taking place 3 times a week. The epidural rate is currently 32%. Alternative forms of analgesia for labour, such as Remifentanil PCA are available for patients. There is a consultant led anaesthetic antenatal assessment clinic, held weekly, where highrisk women are seen and delivery plans arranged. This unit is used for the core trainee s obstetric module. The Epsom obstetric unit has roughly 2000 deliveries per year. The C-section rate is 25%, with elective section lists taking place twice a week. The epidural rate is currently 36%. Intensive care: The unit at St Helier currently has 12 beds (six level 3 and six level 2) with around 350 admissions per annum. There are 6 consultant intensivists providing daily all-day cover with a dedicated ICU on-call rota. The unit offers invasive and non-invasive ventilation, cardiac output monitoring, haemofiltration and a tracheostomy service. It provides training for FY2 doctors and anaesthetic trainees at basic, intermediate, higher and advanced stages of training. Epsom has an 8 bedded level 2 unit with the capability to ventilate level 3 patients overnight if required. All other level 3 patients are transferred to the St Helier site. Paediatrics: Paediatric ENT and dental day surgery cases with some inpatient paediatric general and orthopaedic cases are anaesthetised in the stand alone Queen Mary s Hospital for Children. This has a dedicated children s theatre suite including two theatres with stage one and stage two recovery areas. This is staffed by paediatric nurses and a play specialist, providing an excellent child focused service. The Epsom site has weekly paediatric ENT and urology day case lists. Simulation: The St Helier patient simulator facility is used to enhance multi-professional training in skills and patient management. Dr Lim leads regular sessions for core and specialist trainees throughout the training year. The department has a blue phantom ultrasound trainer, an epidural back part-trainer and a part airway trainer, to provide additional simulation training to anaesthetic trainees. Pain Medicine: The trust s acute pain service comprises of two consultants, one associate specialist and three clinical nurse specialists. The Chronic pain service is primarily based at St Helier Hospital with a team of 4 consultant anaesthetists and one associate specialist. Pain modules are provided for intermediate, higher and advanced levels of anaesthetic training. 4

Accommodation: The hospital provides single accommodation and limited married quarters and family accommodation. Transport connections: The Trust has excellent rail, bus and road links. A shuttle bus service links the two main hospital sites. Miscellaneous: The St Helier anaesthetic department space comprises an anaesthetic office, communal coffee room, consultants offices, a seminar room, and trainees rest/on-call rooms with a kitchen. There is a small department library and a computer / study room. The hospital has a main library with 24-hour access and doctor s mess facilities. Similar facilities are found on the Epsom site. 5
