Position Description: Nurse Practitioner POSITION TITLE REPORTS TO STAFF RESPONSIBILITY Nurse Practitioner Nurse Manager/General Practitioner Peer support to members of the team WORKING RELATIONSHIPS Patients Practice Team Practice management Practice development staff at Green Cross Health PHO staff Other relevant organisations PURPOSE Key tasks and behaviours To work within the practice team as an autonomous practitioner to provide high quality care using advanced knowledge and skills. To promote health, prevent disease and to diagnose, assess and manage people s health needs. To implement and evaluate treatment plans both independently and in collaboration with other health care professionals. To ensure a high quality of service is provided to internal and external customers of the centre ensuring centre s success as a leading primary health provider. 1. Practice Standards a. To provide expert clinical practice Undertaking assessments consider differential diagnoses, ordering, conducting and interpreting diagnostic and laboratory tests, and administering therapies for the management of potential or actual health needs Running cost-effective clinics covering the clinical areas within the scope of practice (long term conditions/primary health care lifespan) Reviewing patient medications and eliciting patient health concerns Appropriately request, monitor and interpret diagnostic tests and results When appropriate, running an acute care clinic with GP support and discussion, and billing appropriately Ensuring that you correctly utilise available funding streams and that your services are appropriately charged out, to achieve specific key performance indicators (KPIs) b. Ensure care is provided to patients in accordance with the appropriate Actively working in a way that will enhance and increase the Centre s reputation and profile in the community Ensuring that the appropriate health assessments are conducted in accordance with the professional and company standards Page 1 of 5

Ensuring that all patient contacts are documented, and treatment plans implemented and evaluated in a timely manner consistent with evidence-based practice Ensuring safe prescribing of medicines within the specific area of practice (must have a collegial relationship with a registered prescriber in an appropriate scope of practice) Reviewing health assessment procedures and ongoing care documentation and evaluation activities to ensure consistency with practice manuals Liaising with general practitioners, pharmacy, ambulance, laboratory and other health professionals to ensure that patients health needs are met Assisting patients and their families to make informed decisions Liaising with patient groups and encouraging an active involvement in the provision of care and associated activities of the company Acting as an advocate for patients Understanding, promoting and meeting the cultural, spiritual and health needs of patients Ensuring care and services are provided in accordance with the Code of Health and Disability Services Consumers Rights 1996 Having a working understanding of: The Treaty of Waitangi; The Privacy Code 1994; Accident Rehabilitation and Compensation Insurance Act 1992; Occupational Health and Safety Act 1992; The Contraception, Sterilisation and Abortion Act 1997; The Health Practitioners Competence Assurance Act 2003; The NCNZ Code of Conduct; The NZNO Standards of Practice for Practice Nurses; The NZ Health Strategy; The NZ Primary Health Care Strategy; He Korowai Oranga Following Green Cross Health s clinical guidelines for practice as set out in www.inner-circle.net.nz and elsewhere Page 2 of 5

c. To provide a safe and therapeutic environment for patients, visitors and other staff. Understanding and promoting emergency procedures such as fire response and evacuation in accordance with statutory requirements Taking responsibility for your own personal health and safety and ensuring that your work practices do not provide a hazard to others Understanding and promoting all procedures and guidelines to ensure safety of medication/vaccine storage and administration Observing and promoting all professional guidelines for practice regarding infection control Working in partnership with individuals, families, whanau and communities across a range of settings 2. Professional development and education a. Maximise personal development and support the development of colleagues. Completing all orientation and induction requirements Taking up opportunities to improve your skills and knowledge through Continuing Nursing Education activities Maintaining a professional portfolio which demonstrates competency to practice Maintaining a current Annual Practising Certificate and personal professional indemnity insurance Maintaining a current (validated within the last 12 months) CPR certificate Participating in a personal performance appraisal process annually, and identifying personal goals and strategies for achievement with your line manager and senior nurse against the NCNZ competencies for Nurse Practitioner http://www.nursingcouncil.org.nz/nurses/scopes-ofpractice/nurse-practitioner 3. Quality improvement a. To utilise treatment and care related resources appropriately. Being fiscally aware of consumables and equipment; reporting needs for stock repurchase, providing suggestions in regard to effective products and involvement in ordering and checking as appropriate Identifying and reporting if any care related equipment is not maintained in a serviceable and cost efficient manner and assist in planning for replacement as needed b. To participate in quality improvement activities. Attending nurse team and practice team meetings Leading and supporting problem solving into quality issues Participating in formal systems for quality assurance, such as audit activities Identifying care or clinical practices or processes that require review and liaise with the Nurse Team Leader and practice Page 3 of 5

manager to discuss and review these Actively participate in professional activities, and in local and national policy development c. To support and collaborate with peers and other providers within the wider health care team Providing feedback to colleagues about their performance so to enhance patient care Demonstrating leadership as consultant, educator, manager and researcher as appropriate Discussing patient care issues with other colleagues involved as needed Reporting any concerns to the Nurse Manager/Line Manager in regard to competency issues Working constructively within the wider multi-disciplinary team within the centre Being an honest, open communicator who demonstrates respect for themselves, and all persons 4. Utilise treatment and care resources appropriately Adheres to practice guidelines and procedures and assists with maintaining these Revenue: ensures correct invoicing and receipting for all patients as applicable Being responsible in the use of clinical supplies b. Accreditation and compliance Demonstrate understanding and commitment to Cornerstone and other accreditation processes (eg. ACC requirements). c. And any other relevant tasks which the Employer may require form time to time QUALIFICATIONS/EXPERIENCE: A New Zealand registered Nurse Practitioner A minimum of five years primary care experience Current smear taking validation Current immunisation validation ESSENTIAL SKILLS Ability to meet goals and achieve deadlines Works as a member of the team Page 4 of 5
