C O F F E E & G E L A T O A LETTER FROM THE PRESIDENT & CEO Welcome. At some point in our lives, we will all be touched by life limiting illness, death or grief. Matthews House Hospice is creating a Centre of Excellence to help you, your family or your friends through those difficult times. For the past 18 years, Matthews House has fulfilled its mission as a community organization dedicated to helping its clients live life fully every day. We offer volunteer based services in home, in the residential hospice and at the original community hospice. We have welcomed clients into our various day programs, visiting hospice programs and the residence with 24 hour nursing and medical services. Our high standard of excellence requires ongoing investment to provide meaningful programs and services by qualified passionate staff; and recruitment and training of dedicated volunteers to support the programs. Our community demographic is changing as more and more retiring urban dwellers are finding our community attractive. These baby boomers are aging, and the need for a hub to house the interprofessional team who provide outstanding chronic disease management, community palliative care and end of life care is growing. Our government also recognizes these changes and is committed to providing financial support for the quality of community and residential hospices. Your interest, support and donations for a new facility for Matthews House will feature the centre of excellence for hospice palliative education and care planning in South Simcoe; and a hub including meditational gardens and accessible gathering spaces for families, groups and the work of the hospice palliative care service team to support clients, residents and families. The success of this new integrated facility will depend on engaging our entire community in this special project by everyone Walking the Journey Together. 3 THE SPENCE FAMILY S STORY WALKING THE JOURNEY WITH US Adam Spence (17) says his mom, Debbie, was the best person in the world. She was everything to him. When a rare form of cancer led her to end of life care, Debbie wanted to find an option for her last moments that was comfortable for her husband and her teenage children. She felt hospital wasn t an option for her, and neither was her home but Matthews House gave her a sense of family and home that she felt comfortable with. "We could come here whenever we wanted; the front door was always open to us, the ladies were so nice. It was like living in my second home., Adam shares. Debbie s best friend, Tammy, describes the staff and volunteers at Matthews House as very, very special people who care not only for the patient, but for the whole family. It just made me happier to come here, Adam says. Matthews House is grateful to be part of the Spence Family s story. 4

FUNDING Matthews House has a strong history of responsible and prudent fiscal management. Copies of our audited statements are available on our website at www.matthewshousehospice.ca. Our Annual Operating Budget for 2016 17 is $1,100,000. To maintain our high level of programming and quality of services, Matthews House relies on our generous community supporters like you, to balance the budget and cover the $878, 400 (66%) of annual costs not provided by our current health care system. Community donations are vital for our organization to ensure Matthews House continues to operate. 5

Matthews House provides a home like, comforting environment with around the clock nursing and medical support by our inter professional health care team. The focus of health care is shifting away from the hospital and toward the community. Matthews House continues to walk the journey with the Brown family, through counselling, support and mentorship with Howard s surviving twin brother. 7 THE BROWN FAMILY S STORY Howard Brown, a 66 year old identical twin with special needs, was under the primary care of his brother Charlie when he was diagnosed with cancer. Charlie describes Howard as a funny guy who loved old movies; he was special, he shares. To bring his brother home for end of life was not an option for Charlie, and he needed a place to turn that would understand the unique needs of his brother Howard. We came here, and it made us feel like home. It didn t feel like a hospital it felt like a better option., Charlie shares. Matthews House feels like home, he asserts. He describes the staff as accomodating, he says they make you feel special, and you know that your loved ones are well cared for when you re not there. As a resident, my brother was allowed to have dignity. My other brother was made to feel welcome here the staff were just amazing. I would have to say they were angel like. They gave us a place to breathe."

THE CRAIB FAMILY S STORY It is a growing reality that numerous young families are caring for aging parents. As with many of us in this sandwich generation, my father in law, a city boy his entire life, moved to our country home with my husband, myself and our two young boys. As years progressed, Dad suffered with a chronic lung disease that destroyed his lung capacity. Dad arrived at Matthews House Residential Hospice on December 23, 2014 and it felt like Christmas morning. The home was filled with scents of turkey and Christmas pudding and we were all welcomed with loving and open arms. A sudden burst of tears filled my heart with so much peace and comfort. Ludi is a very special dedicated professional that cared for Dad for his two weeks at Matthews House. Our entire family was able to be by his side while he received the round the clock care he needed. Matthews House and my family walked the journey together. We were not alone and I am forever grateful. 9

Our large multi purpose programming rooms will be easily transformed into various programming rooms to suit the needs of our community. The view of the new facility features a peaceful, private garden with an accessible outdoor patio with a door wide enough for a bed to fit through. We will recreate and restore that same feeling in our new building. In addition, it will provide us with ample space in which our programs and services can continue to grow. JUDY S STORY AS A CAREGIVER WALKING THE JOURNEY WITH US My husband had Parkinson s disease, dementia and limited mobility, and I was his primary caregiver. I hardly slept; I was tired and worn out... and that s when Matthews House came to help me. The love and support of someone who understood what a caregiver was experiencing was amazing. It was a sounding board: I could cry my eyes out, I could gain more knowledge about how to help and care for my husband. It was the beginning of a long journey. I have heard so many people say about how beautiful an experience it is to be in hospice; so much love, compassion, even joy. Following the death of my husband, the people at Matthews House surrounded me with love and support, bereavement counselling, and a bereavement support group, which really helped me in my grief journey which was long and hard. Now, I give back to Matthews House by offering my services as a volunteer. I am now a cofacilitator of a caregiving support group. 14

We need the new Matthews House Centre of Excellence to meet our demand and significant increased client interactions and referrals. And with increased awareness from our Walking the Journey Together Campaign, we anticipate these numbers will grow. COLLABORATION IS THE HALLMARK OF OUR PHILOSOPHY OF CARE We know that in order to provide excellent hospice care we must work as part of an interprofessional team. We have always worked closely with our community health partners to ensure that we do not duplicate services and that we deliver programs that meet our community s ongoing needs. KATE MOOIJ S STORY Our experience at Mathews House Hospice brought comfort, resolution and even joy to our family. From the time our Bunty was admitted to the hospice, our large family were embraced by staff and made to feel at home. The ability to be ever present with our beloved mother, in beautiful surroundings, with fully attentive and caring staff, was an incredible gift. Staff also recognized our pain and grieving, and they treated us marvellously. When the day came for us to finally say goodbye to our beloved Bunty, the staff invited us to take part in a tradition offered each time someone dies at the Alliston residence. For the ceremony, the person is draped in a memorial quilt and family members place special stones on the patchwork with a message they feel represents their departed loved one. We thought it was meaningful and willingly participated as we said goodbye to our mother, grandmother and great grandmother. Then Ludi asked what we would do in our culture, and being Jamaican, I said we would sing and dance. Staff and volunteers lined up to pay their respects as we sang and danced my mother out to the hearse. That experience touched our family indelibly and we will always be grateful for Matthews House. They really did provide my mother with a comfortable end to her life and that's what Mum deserved a dignified, wonderful, family filled, culture, and faith infused departure. We thank the staff, volunteers and donors for making this wonderful facility available in our community. 17

T I M E L I N E OUR JOURNEY TOGETHER ENDURES Sally Taylor, a retired RN, worked with Hospice Simcoe in Barrie during the early founding years, coordinating volunteers and facilitating training programs and offering a Circle of Friends (a support group for people with life threatening illnesses) to Alliston residents in her own living room. 2003 2004/2005 2007 2010 2012 Founders Sally and Frank Taylor's dream of establishing hospice services in Alliston becomes a reality. Matthews House facility on Highway 89 is purchased (through a contribution from Sally Taylor and help from the Matthews family). Its first staff member is hired. Matthews House becomes an independent hospice and registered charity. Mary Chantler encourages board to begin thinking about a residential hospice and the property at 131 Wellington Street East is donated to Matthews House. The Board of Directors, under the leadership of Marv Chantler, opens its first two residential beds. 2015 2016 2016/2017 2018 Matthews House develops its new strategic direction and unveils its plan to build the new Centre of Excellence, endorsed by community partners and stakeholders. The Ministry of Health and Long Term Care announces investment of $1.05 million annually for a new palliative care hub featuring 10 bed residential care and a centre of excellence for education, a volunteer visiting program and outreach. The new Matthews House Hospice capital campaign goes public to build a 10 bed residential hospice. Opening of the new Matthews House Hospice a hub for hospice palliative care services and Centre of Excellence. 19

F R E Q U E N T L Y A S K E D Q U E S T I O N S ABOUT THE CAMPAIGN Q: WHAT IS THE WALKING THE JOURNEY TOGETHER CAMPAIGN? A: Walking the Journey Together Campaign will raise $6,000,000 for the new Matthews House Hospice - Centre of Excellence and build a 10 bed Residential and Community Care facility to meet the current and future needs of our community. The Campaign will result in monies being available each year to fund building development and spaces for program expansion. Q: HOW MUCH WILL IT COST TO OPERATE MATTHEWS HOUSE? A: Matthews House plans to open 6 beds immediately providing a value to the community of $2,400,000 for an investment of $804,000 annually. The remaining 4 beds will be offered to the community, Examples include: short term pain and symptom management services and respite for families and caregivers of persons with both life threatening illness and debilitating chronic diseases. Q: WHY IS NOW THE TIME FOR THE CAMPAIGN? A: Matthews House Residence provides 4 beds (unfunded) of only 7 residential hospice beds available within the Central LHIN and Matthews House is the only hospice participating in the regional hospice bed registry for the Central LHIN. In 2015, the annual operational funding for funded hospices in Ontario was $90,000 per bed to cover the costs of Nursing and PSW only. Q: HOW WILL THE NEW CENTRE OF EXCELLENCE AFFECT THE SURROUNDING COMMUNITY? A: The new Matthews House Centre of Excellence will architecturally blend into the surrounding community. The driveway, parking area and gardens have been carefully designed in the tradition of Matthews House to allow privacy, practicality and to add beauty to the community. 20

Contributions may take one or more of the following forms: a pledge over a period of time (1-3 years) a single donation securities and personal property (stocks, bonds, life insurance policy) Q: WHY ARE PEOPLE BEING ASKED TO CONSIDER MULTI-YEAR PLEDGES? A: Matthews House will accept one-time donations in the form of cash, cheque or securities. While tax incentives make donating securities an attractive option for many donors it is not an option for everyone. Spreading a commitment to Matthews House over several years may suit your tax and financial planning. Q: HOW WILL GIFTS BE RECOGNIZED? A: All gifts to Walking the Journey Together Campaign are greatly appreciated and will be recognized on a donor wall in our facility. Larger gifts may also have specific naming opportunities for consideration based on availability, and we will work with you to personalize and tailor recognition to suit your needs. F R E Q U E N T L Y A S K E D Q U E S T I O N S ABOUT HOSPICE CARE Q: WHAT IS THE DIFFERENCE BETWEEN A HOSPITAL PALLIATIVE CARE UNIT AND MATTHEWS HOUSE? A: Hospitals continually strive to make their palliative care units or suites as home-like as possible within their large acute care setting. A residential hospice specializes in end-of-life care. There is peace within the ambience of a home. Matthews House staff and volunteers will serve 6 clients and their families. Clients can eat when, where and what they want. Kids are comfortable to visit and can play with the toys. Families can wander in the garden or read in the library. Pets can visit or stay. Q: ARE CHILDREN ADMITTED TO MATTHEWS HOUSE AS A CLIENT OR RESIDENT? A: Matthews House currently provides day program services for kids with grief and bereavement needs and their parents. Residential pediatric hospice care can be very specialized with needs that require specific equipment and supplies, for the children to be cared for safely and optimally. Matthews House serves each family on a case by case basis in direct consultation with the child s community service team and the family s hospital. Adolescents who meet other admission criteria may best be served at Matthews House with the support of local pediatric palliative expertise. All of these applications are assessed on an individual basis. Q: HOW DOES A CLIENT GAIN ACCESS TO MATTHEWS HOUSE? A: Referrals are welcomed from families, nurses, physicians, the CCAC (Community Care Access Centre), hospitals, long term care homes, community hospice organizations, or from any other health care professional. Family members can call and initiate a referral and Matthews House staff will assist to incorporate an appropriate health professional in the referral process. 22

F R E Q U E N T L Y A S K E D Q U E S T I O N S ABOUT MATTHEWS HOUSE Q: HOW MANY PEOPLE DOES MATTHEWS HOUSE ASSIST IN A YEAR? A: Since inception in 2013, our 4 bed residential hospice has served 211 clients in the residence. These clients have been surrounded by friends, family, staff and volunteers as residents have taken their end-of-life journey. In 2015/2016, Matthews House supported 99 clients in our residence, and 1664 individuals through a variety of community outreach programs. Q: HOW MUCH DOES IT CURRENTLY COST TO OPERATE MATTHEWS HOUSE? A: $1, 100, 000 annually. Matthews House revenues grew 23% (29% in 2015) to $1,021,016. Our fundraising events play a critical role at 33% of total revenues, however are reduced from last year s $401,175. This year s residential costs were $456,000, approximately $118,000 more than prior year, reflecting additional staffing costs in addition to operational responsibility for PSW functions. Thanks to the generosity of our donors and supporters, Matthews House generated a surplus of $44,871 ($93,370-2015). Increased reserves by $61,000 allows Matthews House to increase its service to the community. Q: IS HOSPICE CARE COST-EFFECTIVE? A: Yes. In fact, the Auditor General of Ontario did a study in 2010 to determine how much the government would save using residential hospices. She estimated the cost of an acute care bed in a hospital to be $1100 daily. In a residential hospice, the cost fell to just $460 per day! Q: DIDN'T I RECENTLY SEE AN ANNOUNCEMENT FOR MATTHEWS HOUSE FUNDING? A: Yes! Hospice Care in Ontario is a community driven initiative, supported by local donors and volunteers, as well as Government. Matthews House is proud to have received annual sustainable funding which was announced September 2016. Government support is crucial to help meet the need and demand for care. This is a commitment for permanent, ongoing, operating funding through our Provincial Government. This money is operational funding for Residence Care, provided at a per-bed amount of $105,000 annually. These dollars are for residential service and residential service support activities only such as nursing care and personal support work. Matthews House relies on donations from individuals, businesses and organizations and the proceeds of our fundraising efforts to support our visiting hospice, outreach, day hospice programs and of course to meet our capital investment needs. 23
