Gila River Indian Community District 2 Multi-Purpose Building Project #04-05-12 RFP FOR CONSTRUCTION MANAGEMENT SERVICES A. Purpose: SECTION I. ADMINISTRATIVE INFORMATION This RFP is meant to provide prospective firms with sufficient information to enable them to prepare and submit proposals for consideration by the Gila River Indian Community (GRIC) for the District 2 Multi-Purpose Building Project. B. Introduction: The Gila River Indian Community is soliciting qualified Construction Management Firms to provide construction management services for the District 2 Multi-Purpose Building Project. C. Location: The Gila River Indian Community is located in Central Arizona in Maricopa and Pinal Counties. The reservation is south of the Phoenix metropolitan area divided by Interstate 10. The reservation contains approximately 372,000 acres (581 square miles) of which about one third lies in Maricopa County and remainder in Pinal County (Tribally-owned land account for about 26 percent). A majority of the land is held by the Federal Government in a Trust status. The Tribal headquarters is located in Sacaton, Arizona, which is about 30 miles southeast of Phoenix. The Tribal government is made up of elected officials who consist of a Governor, Lieutenant Governor and seventeen members of the Community Council. For governmental purposes, the Reservation is organized into seven districts. D. Project Team: The Owner's Team will consist of 2-5 permanent members and the Tribal Projects Development division. Other members may be added, as needed, to provide input in the Project. The selected firm will report to the Tribal Projects Development Project Manager in charge. 2

Mandatory Pre-Proposal Conference. Site Visit (optional): Friday, April27, 2012 3. Deadline for questions: Monday, April30, 2012 at 4:00p.m. MST 4. Deadline for answers to questions: Wednesday, May 2, 2012 5. Proposal submission deadline: Wednesday, May 9, 2012 at 2:00p.m. MST 6. Proposals reviewed by selection committee: Thursday, May 10, 2012 7. Approximate selection date: TBD G Request for Proposals: The Gila River Indian Community will be contacting prospective firms who have demonstrated previous interest in providing the services requested. All interested firms or individuals who were contacted are invited to submit a proposal in accordance with the specifications, requirements and dates set forth herein. All firms must attend the Mandatory Pre-Proposal meeting in order to submit a proposal. H. Inquires: Prospective firms may make written or verbal inquires concerning this RFP to obtain clarification of requirements. No inquires will be accepted after April 30, 2012. Addendum or Supplements to Request for Proposal: In the event it becomes necessary to revise any part of this RFP, an addendum will be provided and posted to the website at www.gilariver.org under the tab "About Tribes" then "Bids/Request for Proposals." Add addendums will be posted after the deadline for questions May 2, 2012. It is the responsibility of each firm to download and print the addendum(s) who received the original RFP. J. Proposal Submission: One (1) original and five (5) copies of the proposal must be received on or before Wednesday, May 9, 2012 at 2:00pm. Firms mailing their proposals must allow sufficient mail delivery time to ensure receipt of the proposal by the time specified. Propriety Information: Any restrictions on the use of the data contained within a proposal must be clearly stated in the proposal itself. L. Response Materials Ownership: All materials submitted regarding this RFP becomes the property of the Gila River Indian Community and will only be returned to the vendor at the Community's option. M. Incurring Costs: The Gila River Indian Community is not liable for any cost incurred by firms prior to issuance of a fully executed contract. N. Acceptance of Proposal Content: The contents of the proposal of the successful firm may become all or part of the Scope of Work and as such contractual obligations. Failure of the successful firm to accept these obligations in a contract may result in cancellation of the award. 0. Selection/Acceptance Time: A. District 2 Multi-Purpose Building Project Selection Committee representing the Gila River Indian Community will judge the merits of proposals received in accordance with the general criteria described herein. B. The objective of the District 2 Multi-Purpose Building Project Selection Committee will be to recommend firm's whose proposals are most responsive and responsible to the Gila River Indian Community's needs while staying within the stated criteria. C. The three (3) top ranked firms may be called for interview to further evaluate the qualifications and capability of the Respondent(s). District 2 Multi-Purpose Building Project Selection Committee in conjunction with Tribal Projects Development will conduct the interviews. P. Award of Bid/Right to Reject: A contract will be awarded to the successful firm whose proposal conforms to the RFP, and is most advantageous to the Gila River Indian Community. The Community reserves the right to reject any and all proposals and to waive informalities and irregularities in proposals received and to accept any portion of any proposal or all items proposed if deemed in the best interest of the Community. 4

District 2 Multi-Purpose Building Project Selection Committee will recommend the entity, which has submitted a proposal that is most responsive and responsible and qualified to the Gila River Indian Community's needs for award of the contract. Proposal Evaluation & Submittal Criteria Each proposal shall include the following information. Points will be awarded in accordance with the evaluation criteria described herein. Descriptions of the rating factors and the points associated with each are outlined below. A maximum award of 100 points is possible. To expedite the Proposal evaluation process, and ensure that the Proposal is provided a thorough and complete review, please organize your response to the rating factors in the same order that they are described below. Only include documentation that will clearly and concisely support your response to the rating criteria. 5

Qualifications A general statement of the proposer's qualifications, services offered, years in business, representative projects, number of employees, number of registered professionals, etc. B. Management Team A list of each of the individual firms or professionals that would be involved in the project, the role they are scheduled to play, their professional qualifications, experience, project experience, professional affiliations or designations, and any other information pertaining to their ability to perform their duties on this project. C. Firm's Experience with Similar Projects A list of at least three (3) representative projects that are similar in scope and nature to this project. For each project specify the services contracted for, the start and completion dates, and the name, address, and telephone number of the contact person. Provide information on the name, type, size, cost, and overall schedule of the project as well as the Owner and Architect and Contractor of record for each project for the Owner and Architect. D. Indian Preference Provision Preference will be given to Community Owned Economic Enterprises, Community member contractors and Qualified Indian Construction Contractor Proposers. Indian Preference Requirements Persons or businesses seeking qualification under the Community/Indian Preference shall submit sufficient evidence to establish their qualification as a Community/Indian-owned enterprise or organization by submitting the form "Indian Enterprise Qualification Statement" included with this document. E. Project Approach Describe your approach to assisting with the construction management of Residential Youth Homes Facilities and how you will work with the Owner's Team and the design and construction professionals in achieving the goals as stated in this RFP, with respect to, but not limited to, program, budget and schedule. Give examples of this approach has led to successful completion of similar projects. 6

Fee RFP FOR CONSTRUCTION MANAGEMENT SERVICES Each proposal shall include a fixed fee to provide all services required by this RFP and the project schedule developed above. The fee will be paid in monthly installments through the duration of the project. See attached Fee Proposal form. G. Organization Provide an organizational chart and narrative description of how your firm will provide the services. Clearly indicate the role of each individual, their location, and the percentage of their time spent on this project (employees). If not provided elsewhere in the proposal, provide a background history/resume of all personnel assigned to the project. Provide a chart of the staffing levels through the various stages of the project (if applicable). H. Reimbursable Expenses Reimbursable expenses should be presented as a not-to exceed figure. I. Additional Services The proposer may highlight and explain any additional services that the firm would provide if requested, that are included in the fixed price proposal. 7
