Tracking Quality Measure Progress Quality Measures Arrive! Beginning Tuesday July 18 th it becomes simpler for Physicians and Nurses to track Quality Measure Progress. If a patient qualifies for a Quality Measure, the measure will appear as a widget on the Physician Workflow Mpage. This allows physicians and nurses alike to easily see what specific QM tasks are still outstanding and allows for quick ordering and documentation by clicking on the link to Order, Administer, or Document. Quality Measures: Stroke, VTE, Immunizations, Elective Deliveries <39 weeks, Cesarean Sections, Ante Natal Steroids between 24 34 weeks, Hospital Associated Bloodstream Infections in Newborns, Exclusive Breastfeeding, Newborn Hearing Screening. EXAMPLE: VTE Prophylaxis 1. Incomplete No VTE documentation 2. Incomplete VTE documentation in process. 3. Complete All needed VTE documentation completed. 2

Very Important Clinical Update. SODIUM BICARBONATE INJECTION SHORTAGE Situation: TMMC is currently experiencing supply interruptions of Sodium Bicarbonate injection Background: This shortage is related to manufacturing issues and a recent manufacturer recall which affects all strengths, formulations and sizes. This national shortage is expected to take several months to resolve, and based on current usage patterns the hospital will run completely out of sodium bicarbonate in 1-2 weeks without extreme conservation efforts. Recommendation: TMMC Pharmacy will reserve sodium bicarbonate in all formulations for critically ill patients. Floor stock and non-emergent orders will not be filled. Conserve sodium bicarbonate for critical uses. Avoid using product for non-essential purposes such as buffering lidocaine during this shortage. The Society of Critical Care (SCCM) has recommendations for conserving sodium bicarbonate. Conserve supplies such as drawing up doses from vials rather than wasting unused product. Review storage locations such as crash carts and reduce inventory where possible. Please note the extended dating that has been approved by the FDA. Sodium bicarbonate can be substituted with sodium acetate at a 1:1 ratio. Sodium acetate cannot be given as an IV push, but rather should be infused over 20 minutes for rapid infusion. Urine alkalization can be achieved using oral sodium bicarbonate or IV acetazolamide as an alternative in patients receiving high-dose methotrexate.
