Tuberculosis A positive result for either a skin test or titer requires a chest x-ray. A completed Tuberculosis Symptom Questionnaire is also required annually. 11/21/2017 19
Miscellaneous Information Student Physical Examination use the program s form. A complete exam is required initially at the program s start, unless there s a significant change in your health. The physical can be completed at one of the Occupational Health Clinics sites or by your Primary Care Provider. Call and schedule an appointment. Miscellaneous Information continued Drug Test Results 10 panel or greater. Must be done at one of the designated Occupational Health Clinics. Final Report is sent to designated staff at Moraine Park. The results will be e-mailed to you via your Moraine Park student e-mail. Upload the test result, enter the date it was done & submit documentation. Professional Certifications CPR Must Be AHA BLS Healthcare Provider Sign your card! Record date of class and date it will expire Submit documentation CPR is valid for 2 years. You need to remain certified until graduation. Take refresher course if taking before old CPR expires. If CPR expires must take full course. Ex. American Heart Association Ex.
