North Cypress Medical Center Patient Portal is a secure, private web portal that allows you to access health information online. WHY USE THE PATIENT PORTAL? Manage and maintain your personal health information, including allergies, medical conditions, medications you take and your procedure history Review your visit history, insurance information, active home medications and more Review procedures performed during each visit Quick access to laboratory and diagnostic imaging reports Quick access to Problems and Conditions associated with each visit Keep track of your dependent s information Share information you choose securely with people you trust (ex.: your family or your provider) Print or save your information in a convenient PDF file Check your hospital appointments, get directions and contact information
HOME
On the Home page you can see a snapshot of the Patient Record: Personal Information, Visit History, Personal Health Information and Appointments Summary. You can go to each section for more detail either by clicking directly on the links under the My Visit History directly or by navigating through each tab on top of the page.
VISITS The list of visits is on the left side of the screen. You can select a visit to view detailed information on the right side of the screen. Click on the arrows in each section header to expand or collapse the specific data. The specific visit data has only been loaded beginning in Q3 of 2013. You can get the detail for the specific visit data by clicking directly on the links on the right side of the screen or by
navigating through the tabs on top of the page. Medications displayed on the Visits tab display medications administered during your each particular visit. Reports displayed on the Visits tab displays reports for studies performed during that visit. Similarly, Vitals displayed on the Visits tab display all Vitals recorded during that visit.
REPORTS Your list of reports is on the left side of the screen. Select a report to view details on the right side of the screen. At this time the following reports are available in the Patient Portal (not retroactive):
LAB: CBC and urinalysis CMP, Lytes, Creatin, BMP release date 3/14/14 Thyroid, Renal and Liver function tests release date 5/6/14 Coagulations, Whole blood and additional chemistry tests release date 6/5/14 Urine, special, and referenced lab chemistry tests and serology tests release date 10/16/14 Imaging: Mammography and X-Ray release date 4/28/14 CT, MRI and Ultrasound release date 6/10/14 Cardiology, Echo and EKG release date 7/3/14
VITALS The vitals displayed in the Patient Portal are those recorded at admission and throughout your stay. You can select a visit on the left side of the screen or you can click on the Vitals Summary for a complete summary of the vitals.
VITALS You can select different visit from the drop-down menu. Once you select a visit you need to click the Update button to refresh the information.
MEDICATIONS The Medications screen displays all Active Home Medications recorded upon your discharge from your last hospital stay or emergency Room visit.
APPOINTMENTS A list of all your past and upcoming appointments is on the left side of the screen. You have the ability to request a new hospital service appointment, to set up personal email or text reminders, as well as the ability to add appointments to your calendar. Hospital service appointments include, but are not limited to radiology, laboratory, physical therapy, and sports medicine. If you need to make or change an appointment with your physician office, you will need to contact them directly. Some procedures performed during your Inpatient Hospitalization visit may also be displayed on the Appointments page. Upcoming appointments displayed on this page will only include those that have been verified and confirmed by the hospital registration department. At this time you are only able to request scheduling Mammography appointments via the Patient Portal.
PROBLEMS The Problems tab lists all Problems and Conditions recorded by your Care Team during your hospital stay. There are three sections: Problems Associated with the Specific Visit, All Active Problems, and All Past Problems. You can use the Visits drop down menu to select and view Problems and Conditions associated with a different Visit.
SHARING OF THE INFORMATION You may share any of the information available on the Patient Portal with a family member, you may download or print the information, you can send it via secure email or you can send it to your provider.
SETTINGS From the Settings page you can manage your Patient Portal login and security information: change your email address associated with the portal, change your password, change your security questions and manage profiles. You can also change your image, request a change to your contact information and set up default appointment reminder(s).
SETTINGS You can setup to receive appointment reminders via email or text. You can set up as few or as many as three different reminders. The default reminders you set up here will apply to all of your upcoming appointments.
