INTRODUCTION ABOUT UNITED WAY OF METROPOLITAN DALLAS Mission: Vision: To improve lives in our communities. To be the trusted leader in mobilizing resources for transformational community impact in Education, Income, and Health. Since 1924, United Way of Metropolitan Dallas (UWMD) has been committed to fostering a strong and thriving community. We have learned that the best way to make the biggest possible impact is to first focus on the areas that change people s lives forever: preparing kids to graduate and succeed, helping families leave poverty permanently and enabling people to live healthy, responsible lives. PURPOSE OF THIS REQUEST FOR PROPOSALS The purpose of this Request for Proposals (RFP) is to clearly communicate each aspect of the Basic Needs Grants process and, ultimately, to encourage organizations doing outstanding work in the Basic Needs Focus areas of Disaster Response, Emergency Assistance, Food, Housing/Shelter and Emergency Information & Referral to apply for funding. BASIC NEEDS The Basic Needs Area is not part of the Community Impact Grants process. Organizations interested in applying for funds in the Basic Needs Area must use this Basic Needs Request for Proposals. Programs applying for funding in the Basic Needs Area must offer services that align with at least one or more of the following Basic Needs Focus Areas: 1. Disaster Response: Programs that help individuals prepare for, protect against, respond to, and recover from natural disasters such as hurricanes, earthquakes, tornados, floods, fires, hazardous spills, acts of nature and acts of terrorism (http://www.fema.gov/about). 2. Emergency Assistance: Programs that help individuals manage a crisis situation by providing emergency materials or financial assistance. 3. Food: Programs that help individuals access nutritious food and avoid hunger. 4. Housing/Shelter: Programs that provide safe shelter and/or housing opportunities for individuals who would otherwise be homeless. 5. Emergency Information & Referral: Programs that offer referral services on a large scale that help connect individuals with social service providers during times of crisis. The Basic Needs Area is separate from UWMD s core Impact Areas of Education, Income, and Health. As a result, the strategies and metrics associated with the Impact Areas of Education, Income, and Health will not be associated with programs submitted in the Basic Needs Area. Successful programs in the Basic Needs Area will: Demonstrate a clear connection to at least one of the five Basic Needs Focus Areas. Offer services that do not fit within the core Impact Areas of Education, Income and Health. Clearly track client progress and services. Collaborate with other programs and service providers to help clients become self-sufficient. 4 of 37 UWMD BASIC NEEDS RFP 2015-2016

Serve the entire UWMD service area (Dallas, Collin, Rockwall, Southern Denton counties), or a portion of this area. ELIGIBILITY The following requirements must be met in order for a program to receive funding through the Basic Needs Grants Process: The proposed program must concentrate its efforts and services within the United Way of Metropolitan Dallas service area (Dallas, Collin, Southern Denton 1 and Rockwall counties). The proposed program must align with one or more of the Basic Needs Focus Areas (page 4). The applying organization must pass the Stewardship process (To see a complete list of stewardship documents see page 35 Stewardship Documentation Checklist) BASIC NEEDS GRANTS STAFF CONTACT The Vice President, Community Impact is responsible for implementing the Basic Needs Grants process, and is therefore an important resource for organizations interested in applying. PERCENTAGE OF FUNDING AVAILABLE FOR BASIC NEEDS The UWMD Board of Directors sets the percentage of the Community Fund for Basic Needs. Note: While these percentages are set, the dollar amounts will fluctuate depending on the size of the Community Fund. BASIC NEEDS GRANTS PROCESS OVERVIEW OF PROCESS There are four steps in the Basic Needs Grants process: 1. Stewardship Review 2. Proposal Compliance and Proposal Review 3. Program Site Visits 4. Funding Approval The process begins with the Stewardship Review to determine whether or not an organization is eligible to apply for funding. After Stewardship is completed and proposals are submitted for review, each proposal is evaluated by UWMD staff. Programs that pass the initial reviews are awarded program site visits and are thereby eligible to receive a funding recommendation. Funding recommendations must go through a volunteer approval process before being finalized. All programs are evaluated using standardized tools that measure the relative strength of the proposal. Successful programs will demonstrate a connection between program goals and Basic Needs Focus Area(s), effective programming and evaluation efforts, and access to the resources necessary to achieve high impact. Final funding decisions cannot be appealed. Each of the above steps are described below. 1. Stewardship Review The Stewardship Review will occur before the grant application is due. This is to ensure that all Stewardship decisions will be finalized and communicated before the grant application deadline. A review of the Stewardship documents will be completed by UWMD staff in order to ensure the organization s financial health. Please see the Stewardship Staff Review Form (pages 33 and 34) for a complete list of items to be reviewed during this process. Please note that if any of the nonshaded areas of the review form are marked no, your organization may be removed from the application process and sent notification in January with an explanation of the decision. If one of the shaded areas of the review form is marked no, your Stewardship documents will be moved to a volunteer panel of Certified Public Accountants (CPAs) for further review. The volunteer CPAs will then review your entire Stewardship Packet and decide if your organization passes the Stewardship process. If your organization does not pass the Stewardship Review, you will be notified of this decision via email correspondence in January 2015 and your organization will not be eligible to submit grant applications during the 2015-2016 Basic Needs Grants funding cycle. Please note that the Stewardship packet has a separate due date from the grant application. Please see the Stewardship Checklist (page 35), for a complete list of required documents. 2. Proposal Compliance and Proposal Review The yes/no questions listed below will be assessed by UWMD staff in order to ensure proposal compliance. If a proposal receives a no to any of these questions, it will be removed from the process and the applying organization will be notified via e-mail in January 2015. There is no appeals process. 1. Was the organization application packet submitted on time? 2. Did the organization submit their annual UWMD report on time (if previously funded, must have been submitted via e-cimpact by July 25 th, 2014. Contact Gene Putnam if you have questions)? 3. Was the program proposal complete (including all required sections and attachments)? 4. Did the organization pass the Stewardship Review? Once passing the compliance/review, all proposals will be reviewed and scored by UWMD Staff using the Proposal Evaluation Tool (page 36). The proposal is divided into three major scored sections which are listed below in Figure 1. The proposal will receive a score for each proposal section. Throughout the review process, proposals are evaluated using the Best Investment criteria to make their decisions. When comparing programs in Basic Needs, Best Investment refers to the programs that most clearly demonstrate: ALIGNMENT: Connection between program goals and UWMD Basic Needs Focus Area(s) IMPACT: Effectiveness of programming as reflected by meeting community need(s), and demonstrating internal and/or external linkages to programs that promote self-sufficiency. RESOURCES: Access to the resources (staff, experience, financial, etc.) to successfully implement the program UWMD Staff and Community Impact Committee Leadership will determine which programs will receive Program Site Visits and which programs will be removed from the process. If a proposal is removed at this step, it will be removed from the process and the applying organization will receive feedback explaining the decision via e-cimpact in early March 2015. There is no appeals process. 3. Program Site Visits a. Scheduling Those programs awarded a site visit will be contacted by UWMD staff. UWMD staff will then coordinate with organization staff to make arrangements for a site visit. Site Visits will be conducted by UWMD staff only. There will be no volunteers involved at this point. b. Content The programs that receive a site visit are encouraged to include the following items during their visit with staff: Review of key program activities and staff Review of key outputs and evidence of impact Content that helps UWMD Staff experience or understand the program The total site visit is not to exceed 1 hour. There is no formal agenda for the site visit. The site visit is an opportunity for UWMD Staff to gain a deeper understanding of the proposed program. It is also the only opportunity available for the organization to share new information about the proposed program and provide answers to pertinent questions posed by UWMD staff. It is recommended and highly encouraged that the following organization staff be in attendance to help present material and answer questions that UWMD staff may have: President, Program staff, Grant Writer and Finance staff. 9 of 37 UWMD BASIC NEEDS RFP 2015-2016

4. Funding Recommendation and Approval UWMD staff will present an initial set of funding recommendations that are based on application score and site visits to the Community Impact Committee Leadership. The final funding recommendations will be reviewed, possibly modified, and then ratified by the UWMD Community Impact Committee, and the UWMD Board of Directors. The decision of the Board is final. There is no appeals process. The UWMD Board will approve final funding amounts in May 2015. Funding decisions will be communicated to each organization via e-mail. Feedback explaining the funding decision will be communicated via e-cimpact. Changes in Reported Information If reported information changes during the Basic Needs Grants Process (after submission of the application, but before funding has been awarded), the organization must submit written (preferably via email) notification to UWMD immediately after the organization becomes aware of the change that explains: what the change has been, why the change was made, and what the effect of the change is on the program/proposal. Please direct such communication to the Vice President, Community Impact, Ashley Brundage, abrundage@unitedwaydallas.org. If reported information changes after the proposal has been funded (after the grant has been awarded and organization has started to receive funding), the organization must submit notification to UWMD within 30 days after the organization becomes aware of the change that explains: what the change has been, why the change was made, and what the effect of the change is on the program. Unrestricted Funding UWMD allows up to 10% of the total program award amount to be used as unrestricted funding throughout the organization. For instance, if an organization was awarded $100,000, then up to $10,000 can be used for unrestricted funding and $90,000 must be used for program costs as detailed in the proposal budget. e-cimpact ONLINE APPLICATION SYSTEM Logging Into e-cimpact Organizations that are already United Way Service Providers can login to their existing e-cimpact webpage. Service Providers new to the United Way system may create an e-cimpact account from the login screen by selecting Create a New e-cimpact Account (see illustration below). Login to your e-cimpact account (using your username and password). 2. Submit your 2014 Stewardship Packet (complete Stewardship forms, attach Stewardship documents, and submit the Stewardship form packet). 3. Complete and submit all Basic Needs Grant program proposal(s) (complete grant forms & attachments, and submit the application). More detailed e-cimpact instructions are provided in this RFP (page 28).We have also included a list of Frequently Asked Questions about e-cimpact on page 31. TIMELINE RFP RELEASE Tuesday, January 6, 2015 STEWARDSHIP DEADLINE The Stewardship packet for current UWMD Service Providers was due Wednesday, December 31, 2014 4:00 p.m. (Electronically via e-cimpact) The Stewardship packet for new applicants is due Tuesday, January 13, 2015 4:00 p.m. (Hard copy and electronically via e-cimpact only). New applicants should note that certain forms and information must be completed online via the e-cimpact system and then printed off to be included in the hardcopy Stewardship Packet. PROPOSAL SUBMISSION DEADLINES Complete application packets (online via e-cimpact) must be submitted to UWMD by 4:00 pm on Wednesday, January 28, 2015. Late submissions will not be accepted. Complete application packets (hard copies -1 per proposal) must be submitted to UWMD by 4:00 pm on Wednesday, January 28, 2015. Late submissions will not be accepted. FUNDING PERIOD Funding will begin on July 1, 2015 and extend through June 30, 2016. ALLOCATION ACCEPTANCE If the proposal is selected for United Way Allocations either partially or fully the Service Provider will receive the following documents: Allocation Letter via e-cimpact Proposal Feedback Form via e-cimpact Service Provider Contract via DocuSign By executing the Service Provider Contract, the Service Provider accepts all United Way allocations as awarded in the United Way Allocation Letter. In order for the Service Provider to receive the awarded funds, the Service Provider must agree to all United Way requirements for the accepted United Way funding, as outlined in the Service Provider Contract. The Contract must be electronically signed and dated via DocuSign by both the Service Provider's President/CEO and Board Chair. Assigning Programs to the Basic Needs Grant Process 1. To assign a program to the 2015-2016 Basic Needs Grant process, click on Assign Program to Investment Process. This will allow you to assign an existing program to the Grant Process, or to Add a New Program if you need to. (Previous Service Providers: please do not add a new program if the program is already in the system. We want to avoid creating duplicate programs.) 2. Click the checkbox next to the Grant Process. 3. Then click on Save and Assign Program to Investment Process. G. Filling out and Submitting Program Proposals via e-cimpact 1. Locate Basic Needs Grants in the left hand menu. 2. Locate Funding Year 2015 2016 Make sure you are in the correct funding year since you may have access to past funding years in your home page. 3. Select Basic Needs Grant Application 4. Select the form packet you want to work with and click edit 5. Select a form to work on and click edit NOTES: a. When entering responses, e-cimpact will show you if you are over the character and space limit for each response. You will not be able to save a Form until the over limit response has been corrected. b. e-cimpact will also let you know if you have input an unacceptable response (for example: inputting.57 when only whole numbers are possible). You will not be able to save a Form until the unacceptable response has been corrected. c. Always save your work before leaving/closing each Form. If you do not save before closing out each Form, you will lose the data you input. 6. Complete and Mark as Completed each required form for submission. The System will not allow an application to be submitted until each individual, required form has been Marked as Completed. 7. When all forms are Marked as Completed select Submit to transfer completed applications to UWMD. (You may have to click on Submit twice to verify your intent to submit.) Once submitted, you cannot modify the application or the attachments. The Status column will change from Completed to Submitted when this step is completed. Q: The name of my program has changed since last year. How can I change the name of my program in the online application? A. On the top left of your Agency Home Page, click on Programs. Find the program you need to update and then click on Edit to the right of that program name. Then type in the updated Program Name and click Save/Update. When you return to your Home Page, your Grant Application will be renamed. Q: I have a new program and need a blank grant application to fill out but I have no additional applications to use in e-cimpact. How can I obtain one? A: Select the link on the e-cimpact home page that says Assign Program to Investment Process. That link will allow you to add an existing program to the Basic Needs Grant process, or to add a new program. Q: The data I entered has changed since the last time I logged on. How did this happen? A: If more than one staff person at your Organization has access to the online application system, it s likely that another Organization staff person has modified the grant application. If you are working with another Organization staff to complete a specific application, please make each other aware when you have accessed an application and made revisions. Q: I m ready to submit my application, but I m not seeing the Submit Application option, but I ve completed all my forms. Why is the option not appearing? A: All required forms for an application must be Saved and Marked as Completed before the system will allow you to submit your application.
