2007-2008 PERFORMING ARTS FUND SAMPLE FINAL REPORT To complete your Final Report, prepare each of the items listed below and submit as instructed. Your Final Report is not complete until we receive all of the components below. In many cases, we require both electronic and hard copies. If you have any questions, you may contact either Christy Dickinson or Katherine Haugen. egrant FINAL REPORT (online) Log into your egrant final report using your user name and password from the application process. Print and submit one hard copy of the following: SCHEDULE OF EVENTS (Upload complete Excel spreadsheet) Report on all of your activities, including the final attendance figures, using the provided Excel spreadsheet. Upload to egrant and mail a hard copy to Arts Midwest. FINANCIAL SUMMARY (Upload complete Excel spreadsheet) Report on all of the expenses and revenues pertaining to your Performing Arts Fund supported engagement. using provided Excel spreadsheet. Upload to egrant and mail a hard copy to Arts Midwest. EXAMPLES OF CREDITING & PRESS Send samples of your organization s crediting of the Performing Arts Fund. Include print and digital communication showing crediting; also include reviews and other press pertaining to your engagement. FEEDBACK AND IMAGES We would like to receive any additional feedback you may have received from your community member, teachers, and/or students. Please also submit images (if available) of the engagement, including performance(s) and educational activity(ies), on CD-ROM (preferably.jpg files) and include credit language. CERTIFICATION PAGE Include the Certification Page (downloadable in the egrant) as the cover sheet to your Final Report package that will be sent to Arts Midwest. Your Final Report is not considered complete until we receive your hard copy. You must submit a complete Final Report no later than 30 days following the conclusion of your engagement. We suggest referring back to your response from your application. 3. Describe a highlight of the performance(s) and/or educational activity(ies) and tell us why it made such an impact. 4. Describe, in detail, how your engagement connected to the members of your community. 5. Please offer suggestions on how Arts Midwest may improve our service to you through the Performing Arts Fund, and/or share any comments on your organization s need for this type of granting program.
