BACKGROUND Priscilla Chan and Mark Zuckerberg s unprecedented $75 million grant received by Foundation (2015) Convened Patient Care QI Governing Body and Working Group Developed project criteria and evaluation process Developed fund oversight and disbursement structure First Round projects approved Grant Agreement established Priscilla Chan and Mark Zuckerberg Patient Care Quality Improvement (ZPCQI) Fund ( Fund ) Membership included cross representation of donor, Foundation senior leadership, board and ZSFG/SFHN leadership Working group curated projects Governing body reviewed proposed projects Foundation approved Governing body recommendations Oversight and disbursement process for Fund would be similar to other restricted grants received by Foundation on behalf of ZSFG and Hearts Grants program Purpose of ZPCQI Fund was to help equip and optimize Building 25 and to support entire campus by providing staff with the resources to continually improve the quality of patient care. 2
FIRST ROUND PROJECTS First round of projects totaling approximately $9 million over next four years Category 1 Continued optimization of Building 25 for access, readiness and safety Category 2 Continued transformation of the patient, staff and visitor experience Category 3 Investing in our future by enabling leaders to transform patient care 3
CATEGORY 1: B25 OPTIMIZATION 1 Forensic Unit Readiness Project Safety and security improvements made to fortify the four beds in this area 2 Mezzanine Safety Enhancement Project Construction of a glass barrier set back from the edge of the mezzanine 3 Emergency Department Access and Flow Improvement Project Redesign the patient entrance space to improve access and flow in this critical area. 4
CATEGORY 2: TRANSFORMING PATIENT, STAFF AND VISITOR EXPERIENCE 4 5 6 7 Building 5 Lobby Remodel Project Campus-wide Wayfinding and Navigation Support Project Nutrition Service Transformation Project Expansion of Interpreter Services Redesign the Building 5 lobby, including the gift shop area. Optimize physical signage in Building 25 and around the campus Utilize technology to appropriately direct staff, patients and visitors Expand and redesign the café food stations within the cafeteria Redesign the stations to improve flow and develop a sustainable grab n go food dispensing machine. Optimize Building 25 patient rooms with affixed interpreter technology. 5
CATEGORY 3: ENABLING LEADERS TO TRANSFORM PATIENT CARE 8 Strategic and Leadership Coaching Project Support our leaders in achieving ZSFG True North strategic goals 9 Professional Development and Kaizen Promotion Office (KPO) Fellowship Program Create a new Kaizen Promotion Office Fellowship program Further professional development opportunities 10 Access & Flow Improvement Work Coaching Project External coaching on our improvement work to increase overall access and flow throughout ZSFG. 6
NEXT STEPS Develop financial processes and oversight in partnership with the Foundation to accept and expend the funds in each of the categories Collaborate with Foundation leadership to develop mechanisms to account for the funds Establish reporting process to monitor progress of projects and to steward donors. 7

REVIEW AND APPROVAL PROCESS Governing Body (GB) Determines Project Categories, Evaluation Criteria and Big Questions (on annual, semi-annual and/or rolling basis) Step 1: Working Group (WG) curates potential projects and identifies project leads. Step 2: WG receives initial presentation of projects and budgets from project leads, WG performs additional due diligence (as needed) and selects final projects to present to GB. Awarded projects submit reports of progress (@6 months) and final report of impact at conclusion Step 3: GB receives presentation of projects, budget and project summary sheets completed by project leads. Additional presentations from project leads may or may not be required. 1-2 months 6 months Step 4: GB votes and recommends final projects. Step 5: Recommended awards presented to SFGH Foundation Board at subsequent board meeting. 10
