Welcome Welcome to MyHealth (formerly known as Strensall Medical Practice) where we aim to provide a comprehensive and high standard of healthcare. We have four health care centres serving approximately 18,000 patients across an area of over 200 square miles to the North East of York. MyHealth is committed to working in partnership with you to help you maintain your health and wellbeing; keeping you as fit and as healthy as you possibly can be. We offer a wide range of services including access to improved information, alongside a dedicated healthcare team. So we firmly believe that by working in partnership, "together, you're healthier". Using Services Registration To be treated by the Practice you must be a registered NHS patient. We only accept patients who live in our Practice Area. Visitors to the area are accepted as 'temporary residents' for a limited period. How to register: Please visit your local MyHealth Care Centre where you will be provided with a 'new patient registration' form. Alternatively, you can download the form from our website: MyHealthgroup.co.uk /documents Appointments All surgeries are by appointment only. To make an appointment, contact the surgery that you normally attend. Let the receptionist know if you need to be seen on the same day so that she can arrange this with an available doctor. If you wish to see a particular doctor your preference will be respected but you may need to wait to see a specific doctor. You can also book your doctors appointments 'online' through our website. Contact your local surgery to pick up a registration form to take advantage of this service. Our appointment hours are listed on the inside cover of this brochure. Please contact your local MyHealth Care Centre as soon as possible, if you need to cancel your appointment. It is estimated that the cost to the NHS, in missed appointments, is 700 million each year. By calling ahead and cancelling your appointment when you need to, will help to reduce this. Any action we can take as individuals helps to provide a stronger service for us all, when we need it. Telephone Consultations We offer telephone consultation appointments with Doctors and Practice Nurses. Telephone appointments can be made by contacting the surgery you normally attend or online through our website: MyHealthgroup.co.uk Home Visits If you feel too unwell to come to surgery and think that a doctor may need to visit you at home, please contact Strensall surgery before 10.00am if possible. The doctor will decide if a visit is appropriate. Access for Disabled Patients and Visitors All sites have provision for disabled parking, access ramps /street level access, accessible toilets and wheelchairs for use on the premises. Dispensing If you live more than one mile from the Practice and the nearest chemist, we can dispense your medicines without the need to take a prescription to a chemist. Medicines can be collected from the surgery you normally attend or, by arrangement, from the 'call houses' in Flaxton (Elm Tree Antiques) and Sheriff Hutton (Post Office) during their opening hours. Repeat Prescriptions To request a repeat prescription, please send your request (preferably using the printed sheet attached to the right-hand side of your old prescription) by post, fax, online or personal delivery to the surgery. For safety reasons we do not take prescription requests by telephone. Please allow 2 working days for checking and preparation of prescriptions. To send online requests for prescriptions, you should collect a registration form at reception. At intervals your medication will need to be reviewed and you may be asked to make an appointment to see the doctor to discuss this. Medicines can only be repeated if this has been agreed by your doctor or nurse. Test Results To get your test results contact the surgery one week after your test. For reasons of confidentiality you must telephone yourself as we will not give information about you to anyone else.
Getting Information Private Medical Examinations & Reports Normally reports, not requiring medical examination, are processed within ten working days. A charge will be made for these, payable on collection, and a full list of fees is available at the reception desk. Violence and Abuse: Zero Tolerance Although this is very rare, the Practice will request the removal of any patient from our list who is aggressive or abusive towards a Doctor, Nurse, member of staff, any other patient or who damages property. Education at GP Registrars Fully qualified doctors, gaining General Practice experience under supervision, may be based at any of our surgeries. Medical Students Six of the Partners train students at the Practice. The Medical Students are from the Hull & York Medical School. We always ask the permission of patients, for students to be present, before a consultation. We are always seeking volunteer patients to assist with student education. Research MyHealth takes part in high-quality NHS clinical research to help improve the wellbeing of the nation. Speak to your GP for more information on how to volunteer. Urgent Problems Practice If you wish to be seen in surgery urgently, please make this clear to the receptionist. They will take your details and arrange for you to see an available doctor. Patient Records All patient information is stored confidentially and securely. Records are not disclosed to non-medical personnel without written permission. Patients have a right to see their own medical records under the Data Protection Act 1998. To request access to your records please book a telephone appointment with your usual doctor. If you require a copy of your records there will be an administration charge for this, payable in advance. NHS Practice Publication Scheme You can access this public information document through our website or by contacting the Practice Manager. Suggestions and Concerns We are always looking for ways to improve our service. Please contact the Practice Manager with suggestions or concerns. Suggestions can also be submitted using our waiting room suggestion boxes. A copy of the 'complaints procedure' is available on request. If we are unable to resolve difficulties to your satisfaction, the following organisations also offer help: Patient Advice and Liaison Service (PALS): This North Yorkshire NHS customer care service can be contacted on 0800 068 8000. Independent Complaints Advocacy Service: This service, available from the Citizens Advice Bureau, can be contacted on 0845 120 3734. The Practice Nurse Manager, is Liz Stenton. Phlebotomy For routine blood taking, phlebotomists hold clinics at all four sites. Practice Manager Joanne Rowe oversees the running of the entire Practice. She can be contacted at Strensall. Joanne works with a team of departmental managers and support staff to provide a quality service. Locum Doctors Doctors are occasionally employed on a temporary (locum) basis to help cover absent doctors. Community Nurses perform a wide range of nursing duties for housebound patients. Telephone 01904 724 390 for Strensall and Huntington patients or 01904 551 760 for Stamford Bridge and Dunnington patients. Counsellor appointments can be arranged at Medical Centres. Referral is by GP. Physiotherapists have treatment sessions at Strensall surgery and York Hospital. NHS Podiatrists treat foot problems at our Strensall and Huntington surgeries, Whitecross Court and Pocklington Health Centre. Speech Therapists can be seen at our Huntington surgery.
