FTS Application Package Notes A complete FTS Transfer / Redesignation Package consists of two parts: 1. Submitted BUPERS On-Line (BOL) FTS Transfer / Redesignation Application form found at http://www.public.navy.mil/bupers- NPC/OFFICER/COMMUNITYMANAGERS/RESERVE/FTS/Pages/FTSRedesigna tionboard.aspx Under the BOL E-Application link 2. Supporting / Supplemental documentation Use the sample FTS application cover letter, which begins on the next page, as your template for assembling part 2. * All applicants are required to have a Command Endorsement. For SELRES currently on Active Duty orders (MOB, ADSW, extended ADT, etc), Command Endorsements may be submitted via either their Reserve Unit CO or from their current AC gaining command. * Letters of Recommendation (LOR) are not required, but are encouraged. No more than FOUR LORs will be accepted. * Enclosures 2-7 are only required if this information is not already in your Official Service Record. * Enclosure 9 (Copy of medical requirements) is required for all applicants, as follows: - 13X7 applicants need to submit upchit. - 1137 applicants need to submit current physical that must be stamped PQ for diving duty. - All other applicants need to submit a copy of their BOL IMR. This can be accomplished by the following (must complete this from a CAC card enabled computer): 1. Log into BOL via: https://www.bol.navy.mil/bam/ 2. Click on Individual Medical Readiness (IMR) Status 3. Open the Snagit application by clicking on the windows icon at the lower left corner of your computer screen, select All Programs, Startup, then Snagit 12, click on Send to Word in the Snagit control center, capture the entire IMR screen by left clicking on the upper right corner of the IMR status screen and drawing a box around it. When you let go of the left mouse button, what you drew your box around will open in Microsoft Word. Label it as enclosure (9), print it, and include it as part of your application. * HSC applicants must delineate and rank their choice for logistic support (HSC-3 SAU/SCORE) and/or Special Operations Forces (SOF) support (HSC-85/Tactical Support Unit) within their application.
