Physicians receive incentives for eprescribing for Medicare patients; penalties are imposed for noncompliance. 2012 1 % incentive, 1 % penalty 2013 0.5 % incentive, 1.5 % penalty Beyond no incentive, 2 % penalty
Any patient can be given an eprescription, unless the medication is a narcotic or other controlled substance. The incentives only count for Medicare patient encounters, i.e. office visit.
Medicare physicians and practitioners are eligible as long as the provider has prescribing authority. No need to sign up as soon as claims are reported with the correct G-code, the provider is ready to go.
To get incentives for 2012 (if it is the first year you are participating): Each provider must submit 10 valid Medicare eprescriptions by June 30, 2011. To get incentives for 2013: Each provider must submit 25 total valid Medicare eprescriptions by December 31, 2011.
All eprescriptions must be reported by the same method to be counted. 3 methods: 1) Claims based* *For 2012 incentives, all eprescriptions must be reported by claims. 2) EMR based must be certified by CMS. 3) Registry must be certified by CMS.
In order to avoid the 2012 payment adjustment, the 10 eprescriptions MUST be reported via claims. Claims must include the code G8553 to count towards eprescriptions.
Allscripts has a free web-based eprescription service. Allows for escripts to be sent electronically to pharmacies across the country. Pharmacies that have a fax machine can also receive eprescriptions. The service has a database of pharmacies that have been added (search by city!)

Choose one doctor who is available when you re signing up to answer identity confirmation questions. Allow one to two days for first confirmation. Add additional doctors AFTER confirmation.
During the registration process the registering provider will have to choose a login name and password. After registration has been confirmed, he/she will log in for the first time. Click on the Contact customer support link to get to a webpage where you can send an email to customer support. They will get back to you at the email address used when you registered for Allscripts.
An administrator must be designated (choose no more than 1 or 2 users) who can add new users and change user privileges. Staff members have no prescribing privileges, but can enter patient info. Each user has his/her own username and password to log into Allscripts.

Designation is used for Nurses or Physicians Assistants to enter prescriptions on behalf of the doctor. 3 Levels: 1) No review required 2) Some review required (we use this one) 3) All review required
DUR Drug Utilization Review DUR warnings come up when the system does a drug check and the provider has prescribed a drug for a longer period of time than is normal (including more refills than usual) or a greater dosage that is usually expected. The purpose is to keep track of and avoid inappropriate prescribing to patients. You can change the DUR settings for the practice in the settings tab.
PQRI and eprescribe incentives are two separate programs. Physicians participating in PQRI can also submit for eprescription incentives.
eprescribing with Allscripts takes some effort to set up, but will help you receive the incentives and avoid the penalties.
