New Information posted to your portal Your name HMC Patient Portal Homepage If you are a Proxy, click to view their Portal Click to log out of the Portal. Your upcoming appointments at the hospital Last three HMC hospital visits After logging into the HMC Patient Portal with your Logon ID and Password, the first screen you will see is the Homepage. The information displaying in the HMC Patient Portal is based on the data that is included in your personal medical record here at Holyoke Medical Center. Your name will display at the top of the page. All of the items on the page are clickable allowing you to view the detail. New Information displaying in your Portal within the last seven days will be listed at the top of the page under the What s New header. NOTE: When new information is posted to your Portal, you will be sent an email message to notify you of the new information: Click on this link to log into your portal from the email 3
Proxy Patient Access If you have received authorization in person from the Holyoke Medical Center Medical Records Department to be Proxy to another person s portal, you will see a button to Change Person in the upper right corner of the HMC Patient Portal Homepage. The portal for the patient that you are proxy to will then display: Click on that person s name, and then you are launched into their Portal homepage: Simply click on the Change Person button once more to return to your own Patient Portal. ************************************************************* 4
Contact Us Please note that there is a Contact us link at the bottom of each page of the HMC Patient Portal: When you click on the link, it opens a text box for you to type a question about your Patient Portal. Please include your patient name in your message: Click on the send button, and please allow 3 business days for a response. What s New When new data has been posted to your HMC Patient Portal, you will be sent an email to notify you. New information within the last 7 days will display on your Portal Homepage under the What s New header: In this case, there is new Result activity on the portal. Simply click on this text to view the new results in your portal. 5
Here are the latest results in the Results section of the portal: Once you view the new information, the What s New heading will no longer display on the Portal Homepage. 4 Main Buttons of Information on the Patient Portal I. The Profile page allows you to view your demographic information including your address, phone number, birth date, primary care physician, next of kin, etc. 6
The off-white boxes at the top of the Portal pages have instructions to assist you in navigating through the screens: The Update Profile button allows you to request an update of certain fields in your profile. After you update your profile information on the screen, click on the Submit button at the bottom of page 2. Please note: You will not see the changes display immediately and we ask that you allow 3 business days for your information to be updated. ************************************************************* 7
II. The Appointments button allows you to review your upcoming appointments at the hospital and request an appointment. The appointments listed are booked by the hospital already, since they have a date and time associated. Click on the appointment to view the details such as the location and the phone number: To request an appointment, click on the button and fill out the questions on the 3 screens: 8
Instructions to assist you with this screen You can request an appointment from specific hospital departments, suggesting days of the week that work for your schedule. Once your appointment request has been submitted, you will be contacted by that department to book the appointment in the system and verify your information: 9
Your request will display on the Portal as pending until it is booked by the department. You can click on it to view the details: This screen may include instructions on what to do before coming in for your appointment: The HMC Patient Portal also allows you to cancel or request a new date and time for your appointment. 10
III. The Medications button contains a list of your medications documented by your Inpatient physician or your ED physician during your hospital visits at Holyoke Medical Center. There is always an option to print the information as well. Click on the medication to view the detail including the medication monograph. To navigate back to the previous screen, there is a button on most of the Portal pages to go Back on the right toolbar. 11
IV. The Health Record button contains several different buttons of information within, including Health Summary, Allergies, Results, Medications, Reports, and Visit History. A. Allergies& Conditions: Your allergies and conditions that have been recorded during your hospital visit will display here: 12
Clicking on the information will reveal more detail, such as the reaction and severity of your specific allergy: B. Results: Your Laboratory and Microbiology Outpatient results from November 2011 to the present are listed within this button. The results are available in the Portal 36 hrs after your tests have been completed. The tests are sorted by the most recent result that has been done at the top of the screen. Click on any of the column headers to sort your results High (H) and Low (L) flags are listed here 13
To view your past results for a particular test, simply click on that test name to view the history. For example, below the Sodium test was clicked on to reveal the history of all the sodium results for this patient at HMC: C. Medications: Your same Medications are also listed within the Health Summary section of the Portal, as you already viewed under the Medications button from the Portal Homepage. 14
D. Reports: Provider Reports are listed within this button, including your inpatient visit discharge summaries, Cardiology Reports and Radiology Reports. These reports will be available in the Portal 36 hrs after being signed by the physician. **Please note that Radiology Images are not available to be viewed on the Patient Portal. ** Click on the report to view the detail: Click on the scrollbar to view the entire report. 15
E. Visit History: Each of your visits to Holyoke Medical Center will be listed within the Visit History button. The type of visit and the reason for the visit are listed on this screen. Clicking on each visit will display the details including the list of discharge medications, forms, and patient instructions. 16
Each of these discharge documents can be clicked on to view the information. This is one of the Patient Instructions that was given to you as the inpatient upon discharge, which you can also print from the Patient Portal. F. Health Summary: The Health Summary button allows you to view or print your Continuity of Care Document (CCD). This document contains portions of your HMC Medical Record including Allergies, Medications, Procedures, Family History, Problems, Vital Signs, and Immunizations. 17
The Continuity of Care Document also includes your Inpatient LAB results as well as important information such as your Advanced Directive, Next of Kin, and Insurance information. You also have the option to Print the CCD via the Print button on the right side of the screen. Future Enhancements The HMC Patient Portal will be enhanced with more functionality in the future as it becomes available, in an effort to improve your on-line portal experience. For example, there will be an option for viewing and paying your hospital charges on-line via your HMC Patient Portal. Also coming in the future on the HMC Patient Portal will be the ability to view, download or email your Continuity of Care document from the Health Summary button. This will provide you the ability to email your hospital information to your primary care physician. We will be posting any news regarding the Holyoke Medical Center Patient Portal on our hospital website, www.holyokehealth.com. Please be sure to visit the website regularly for information. 18
