The Objective The objective for Meaningful Use Stage 1 is a Menu Set Measure, which means that it is one of the optional objectives. In order to meet Meaningful Use Stage 1, an office must satisfy 5 of the 9 possible Menu Set Objectives. The measure states that an appropriate reminder must be provided to more than 20% of patients 65 years or older or 5 years old or younger per their specific preference for preventive/follow-up care. CMS does not specify a reminder and solely leaves this to the provider to determine what is most relevant based on their care efforts. How to Meet the Objective Normally this objective would be performed in the latter half of the Eligible Provider s reporting period. This way you will be sure to provide reminders to more than 20% of the patients that fall into the criteria. This is a great measure to use as a backup in case you are unable to achieve the 5 out of 9 Menu Set Objectives. 1. To meet this objective, you will first need to know where to setup the patient s reminder preference. This by DEFAULT will always be a LETTER unless a user modifies via the patient setup screen. a. Open the patient setup b. Navigate to the Demographics tab c. Find the option Reminder d. Select the Patient s Preference and click SAVE Page 1 of 6 March 2014

2. Next is to setup the correct criteria via the ireports. The basic criteria CMS requires is that for patients 65 years or older or 5 years and younger be provided with an appropriate preventive/follow-up care reminder. This means you can also narrow this criteria based on the Problem List, Medications, Allergies or etc., but must be more than 20% of the basic criteria. a. Navigate to the Reports Portal b. Click on ireports under the naviagation pane c. Next select the Black Plus sign to begin adding the criteria 1 Page 2 of 6 March 2014

d. Select the Patient Demographics/Patient Setup/Additional Information from the Section area e. Select Age-Years from the Field area f. Select Greater than or equal to from the Operator area g. Add 65 h. Click Apply (the criteria will be added to the grid at the top, repeat above steps for additional criteria). TIP: If there are multiple providers in the practice, you can narrow the search by adding in a rendering provider criteria via Claim. Page 3 of 6 March 2014

i. Now Criteria 2 needs be selected on the right which is creating the OR statement. j. Click the Black Plus sign to add the new row. k. Select the Patient Demographics/Patient Setup/Additional Information from the Section area l. Select Age-Years from the Field area m. Select Less than or equal to from the Operator area n. Add 5 o. Click Apply (the criteria will be added to the grid at the top, repeat above steps for additional criteria.) Page 4 of 6 March 2014

p. Now you are ready to run to click. This will show you how many patients match the criteria q. Lastly, select. This will pull in all of the patients and allow you to select a letter to be queued up for all the patients (this is the default). The notes section is required so be sure to enter Patient Reminders or etc. into the box. If the patient s preference was changed via the patient setup, you will notice the their name under Phone or MyMedical Locker instead. You would have to call and provide the reminder for those patients. Please contact support@isalushealthcare.com to create a letter if it does not already exist. The Provider Dashboard would now show the Objective is fulfilled. How the Objective is Measured The measure on the Provider Dashboard is calculated in this manner: a. Denominator: Number of unique patients 65 years old or older or 5 years older or younger seen in EP s reporting period. b. Numerator: Number of patients in the denominator who were sent the appropriate reminder. Exclusions An EP who has no patients 65 years old or older or 5 years old or younger with records maintained using certified EHR technology. Page 5 of 6 March 2014

CMS Guideline http://www.cms.gov/regulations-and- Guidance/Legislation/EHRIncentivePrograms/downloads/4_Patient_Reminders.pdf Setting up your system to run the : In order for OfficeEMR to allow you to run this report, a couple things must be setup. First, your user must have access to this screen to create the report. This is managed via Roles. Also, a letter needs created based on the type of reminder you will be mailing out to your clients. All of these items can be configured by the isalus Support Department (support@isalushealthcare.com). 1. Role: ireport - User role must have Write access to this screen. 2. Letters: Access to the preventive/follow-up care letter Page 6 of 6 March 2014
