Villa St. Gabriel Villa celebrated its 5 th year anniversary on March 30, 2016. In honour of this occasion, dignitaries, residents, family members, staff and volunteers attended a special liturgy and celebratory event held today. Monseigneur Jean-Paul Jolicoeur of the Diocese of Sault Ste. Marie provided a special liturgy in recognition of Villa St. Gabriel Villa s 5 th anniversary. A host of dignitaries and special guests including: Marc Serré, MP Nickel Belt, France Gélinas, MPP Nickel Belt, Evelyn Dutrisac, Councillor Ward 4 and Gerry Montpellier, Councillor, Ward 3, were in attendance. It has been an eventful year of constant change at VSGV. We focused on strengthening our services, improving results for our residents and positioning ourselves to tackle future challenges. During this past year, Villa St. Gabriel Villa made significant strides in meeting its first year objectives outlined in our 2015-2018 three year strategic plan. Operationally, Villa St. Gabriel Villa continued to perform well. We met our quality improvement objectives, as outlined in our annual Quality Improvement Plan. During the past year, our staff demonstrated the organization s continued commitment to public accountability, performance reporting and community engagement. Below are some the achievements and accomplishments in our five strategic areas; 1. Focus on Residents and Patients First 2. Lead in Quality and Safety 3. Inspire and Engage Our People 4. Optimize Resources 5. Engage Community Focus on Residents and Patients First VSGV focused its efforts on residents first and delivering the right care at the right time. In doing so, below are highlights of how we focused our efforts this past year. A new Program Coordinator position commenced August 10 th, 2015 to coordinate Life Enrichment Programs at VSGV. The Make a Wish program granted a wish to a VSGV resident. His wish was to meet the Sudbury Wolves hockey team and to attend a game. His wish was granted when on September 11 th, members of the Sudbury Wolves attended VSGV to meet with the recipient and provided him with free tickets to a game, an autographed jersey and hockey stick, as well as a game of hockey in the neighbourhood dining room. The Life Enrichment department at VSGV increased its exercise programs during the afternoon time slot from 3 times per week to 5 times per week in order to keep the residents active and engaged more often throughout the week. A one-year secondment for a Nursing Restorative Lead was initiated in June 2015. This position was filled with an RPN from SJCCC. This individual focused on implementing a nursing restorative program commencing with VSGV and eventually implementing over all our sites over the next year. Through this initiative this program increased the functional ability of selected residents, and in turn increased the CMI leading hopefully to increased funding.

VSGV acquired a CAD pain pump to administer continuous flow of medication to residents in their end stages of life. Each neighbourhood Resident Activity room at VSGV now has a cabinet dedicated to Montessori program tools and BSO programs. This cabinet is open to all staff and will not be locked in order to provide easy access to the tools which will assist staff in helping residents who are exhibiting behaviours. Lead in Quality and Safety The quality and safety experiences of our residents and staff were enhanced last year through the following initiatives: VSGV was added to the City of Greater Sudbury notification system for community emergencies. This was as a result of the Vale Hazardous Material release in August of 2015. Our VSGV Site Administrator met with the City s representatives for Emergency Management Services to ensure inclusion on this notification system. VSGV implemented the use of wireless tablets for RNs to use throughout their day. The tablets allow RNs to access information at the bedside while doing rounds with the Physician, access information for families, and retrieving data when required, in a more efficient manner. VSGV implemented infrared motion alarms at the entrance to each of the 32 resident rooms in the Vermillion neighbourhood. These alarms are used to alert staff of a resident who may be wandering out of their rooms, who may be at risk for behaviours, and will also assist staff to be aware of any wandering residents with behaviours who may enter another resident s room. The alarms can be activated and deactivated as required based on the residents needs. Inspire and Engage Our People Recognizing the intrinsic worth of all of those that we serve and encouraging active participation in education opportunities were met this year through: A new Program Coordinator position commenced August 10 th, 2015 to coordinate Life Enrichment Programs at VSGV. VSGV Acting DOC completed the Non Acute Infection Control course through Public Health Ontario which enables her to act as a resource for the organization in terms of infection prevention control and best practices. VSGV signed an ONA agreement that took it to March 31, 2016. This brought us in line with the ONA Central Agreement timelines. Celebrated our Annual Sisters of St. Joseph of Sault Ste. Marie Awards for Excellence at each of our four facilities. All events were well attended and the recipients were truly humbled by the awards. The recipients at VSGV were Ruth Ladouceur, RN, Angie Brunet, Receptionist, and Sheree Landry, Environmental Services Assistant.

Optimize Resources This year VSGV was able to balance its budget successfully and is working towards maximizing efficiencies through the following opportunities: A contract was signed with Novo Peak Health for the provision of Physiotherapy services to VSGV. We signed our L-SAA for VSGV, this is a three year agreements effective from April 1, 2016 to March 31, 2019. VSGV Life Enrichment Department received a grant to hire a full time student for the summer. VSGV will have a student for 6 full weeks, from July 11 th to August 19 th allowing the department to enhance resident activities during the summer months. The MOHLTC has announced that VSGV a not for profit home will no longer be required to pay property taxes. This was confirmed by MPAC. This change is effective January 1 st, 2016. Med e-care consulting was contracted by VSGV to audit the MDS/RUGs from 2015/2016 Q1 to determine whether there were coding errors that, if rectified, would result in the CMI increasing. The audit of all 128 residents was completed in August, and resulted in the Q1 CMI increasing from 0.97 to 1.01. This contract will be extended for a full year to audit coding and provide education to our staff. VSGV embarked on a LEAN project regarding incontinent products. The incontinent system was looked at from all aspects including inventory and ordering as well as distribution and correct product usage. Savings have been achieved in a short period of time. The VSGV Café closed effective April 1. The café had been open since the opening of VSGV however it is no longer viable to continue to operate given the resources and efforts required to operate the café. An additional coffee and vending machine with more food options have since been installed in the facility in order to provide staff, visitors, and residents with food and beverage options. Engage Community Ongoing collaboration continued with our community and systems partners this year with the following key highlights: Meetings continued with our community partners to develop strategies to assist in resolving the PSW shortage that is currently in our community. An electrical safety course through the Electrical Safety Authority for maintenance staff has been scheduled for this fall at our South Bay site, and we have reached out and invited other Long Term Care maintenance staff in our community to attend. The Volunteer Manager worked with Colleges and Universities in the community to accommodate academic placements for the coming fall term, including College Boreal, Cambrian College, Laurentian University, NOSM and St. Albert s College. Placements include Nursing programs for RNs, RPNs and PSWs, Social Work Programs, Paramedic Program, Human Kinetics, Sport & Physical Education and Nutritional Dietetic Programs.

VSGV received a certificate from the Reseau De Mieu Etre Francophone du Nord and the NELHIN in October for its designation as a French Language Service facility under the French Language Service Act. VSGV met and in some instances surpassed the indicators set in its Quality Improvement Plan with integrity this past year. Some of the highlights include: Under the indicator to continue to promote/educate the spiritual and religious care component in serving residents. 75% of Chaplain s time was spent on direct resident care. (target was set at 74%) Under its safety quality indicator of hand hygiene compliance before resident contact, 80% compliance was achieved. (target was set at 98%) The Quality Council has developed an Action Plan to target increased compliance in the upcoming year. Under its safety quality indicator of staff compliance with immunization policy. 72% compliance was achieved. (target was set at 95%). Under the indicator for annual education for Ethical Discernment, staff compliance on an annual basis was reported at 100%. (target was set at 95%) Several operational activities of note occurred this past year including the following: VSGV was the recipient of large donations from the Belanger family and the family of Monique Landry- Sabourin. The donations, along with funds raised by this year s Gala will be put towards the completion of the 2 Healing Gardens at VSGV in 2016. The Gala raised $71,000 for the VSGV Healing Garden. VSGV began the Healing Garden landscaping project. The project will see the 2 courtyards and the entire grounds receiving new structures, raised garden beds and more trees, plants and shrubs. The project is expected to be completed sometime in July. VSGV has been the recipient of many donations from community partners such as Les Filles D Isabelle, The Onaping Quilters, The Chelmsford Lions Club, and the Chelmsford Community Action Network. We have been getting media coverage in relation to these donations. Christmas was celebrated in full fanfare with Christmas caroling from multiple community and school groups and staff and Board members. Christmas parties were hosted with residents and their families. A Christmas meal was provided to all staff the week prior to Christmas, in addition, all staff working on Christmas day were also provided with meals. Seniors Month was celebrated with a BBQ for residents and families along with some festive music in June. Gift cards were distributed to those staff that had perfect attendance during the summer schedule in 2015. Staff were very appreciative of the cards and the recognition of a job well done in perfect attendance.

On May 25 th, a Staff Appreciation Event was held to celebrate the tremendous accomplishments of staff in their 5 th and 10 th year of service with the organization. In the face of increasing demands for our services and the challenges of fiscal restraint, we take pride in the exemplary care we offer to our residents and visitors and acknowledge the honour it is to be trusted to provide this care. Our residents continue to play a fundamental role in the Villa by offering feedback through our Resident Satisfaction survey, quarterly resident rounding sessions, at Resident Council and participating in the day-to-day activities that make it their home. Overall, this has been a dynamic year and one that has resulted in enhanced satisfaction from our residents. My warmest thanks go out to all of you for your hard work and outstanding contributions to making Villa St. Gabriel Villa a home of choice in Chelmsford. In particular, my gratitude is extended to our Site Administrator, Ray Ingriselli, who oversees and leads our team at Villa St. Gabriel. All of our organizational achievements this year have been made possible not only because of your individual efforts but also the team effort put forward by all employees each and every day.
