Modification of the required years of experience for individuals to render Therapeutic Behavioral Services (TBS)

Modification of the required years of experience for individuals to render TBS* QMHSs without a Bachelor s or Master s degree in a relevant field will be required to have a minimum of 3 years of experience in order to provide TBS. The 3 years of experience in a relevant field can be achieved at any time, not solely on or before July 1, 2017. Additional Details TBS years of experience are not restricted to a single agency, do not have to be contiguous, and must be documented in the agency employment record. In addition, the employing agency s policy can be more restrictive than state guidelines. *Please keep in mind that all proposed changes are still subject to approval by CMS 9

Coding guidance for ASAM Outpatient Level of Care 1, SUD Group Counseling Two billing codes are available for SUD group counseling provided by a licensed practitioner at the ASAM Level 1 outpatient level of care Group psychotherapy (other than of a multiple-family group) 90853 Service may be rendered by a licensed practitioner providing psychotherapy in a group setting. 90853 may be billed when the service provided complies with AMA/CMS billing guidance and the session is 52 minutes or less. National Correct Coding Initiative (NCCI) edit of 1 encounter per person per day. $21.63 per encounter licensed practitioner and $25.45 per encounter SUD physician per person in the group. SUD Group counseling 15-minuite unit for SUD licensed practitioners who are not physicians H0005 may only be billed when a group session is 53 minutes or more and the practitioner bills for the correct number of 15-minute increments following AMA/CMS billing guidance. $7.21 per 15-minute unit, per person in the group SUD Group counseling 15-minute unit for SUD physicians H0005 HK H0005 AF H0005 may only be billed when a group session led by a physician is 53 minutes or more and the practitioner bills for the correct number of 15-minute increments following AMA/CMS billing guidance. $8.48 per 15-minute unit, per person in the group 10

15 Crisis Psychotherapy Psychotherapy for Crisis Services Definition An urgent assessment and history of a crisis state, a mental status exam, and a disposition. The treatment includes psychotherapy, mobilization of resources to defuse the crisis and restore safety, and implementation of psychotherapeutic interventions to minimize the potential for psychological trauma. Coding for Crisis Services People who are in a life-threating situation MUST receive immediate attention and response from a qualified practitioner to either address and alleviate the crisis situation OR make arrangements for alternative interventions (law enforcement/ambulance/hospital care). The crisis psychotherapy, psychotherapy and any other related behavioral health services (eg. CPST/PSR/CM) that are provided and billed must be supported by the documentation in the medical record. The only clear rule is that crisis psychotherapy and psychotherapy CPT codes can not be billed on the same day for the same person by the same licensed practitioner. 16

Whitepapers were published on the website explaining Ohio s behavioral health redesign background and future changes. Stakeholders are able to submit questions and concerns via the website. Newsletters are frequently sent to stakeholders. Newsletters cover general policy and detailed technical information. Detailed billing-related resources and manuals are posted on the website. All trainings are posted on the website as reference for providers and stakeholders who are unable to participate. Videos were developed and posted on the website to actively and visually engage stakeholders.
