ROLE OF DEPARTMENT Caring is fundamental t nursing services and t all nursing functins. 5. SCOPE AND RANGE The district nursing service perates an pen access plicy that is available 365-days a year. The service des nt perate a waiting list; hwever, care needs are priritised t be able t respnd t bth planned and unplanned care. This includes the ability t respnd timeusly t unscheduled care requests fr thse patients wh require urgent district nursing interventin. The District Nurse Sister/Charge Nurse will be respnsible fr the management f a caselad f cmmunity patients leading and managing a team f qualified nursing staff and health supprt wrkers. The caselad ppulatin is derived frm either the GP practice t which the team are aligned, r within a defined gegraphical ppulatin and/r a defined client grup. The caselad cmprises f patients with a range f cmplex needs, requiring high-level nursing interventins, such as cancer and nn-cancer palliative care patients, thse requiring nursing care in relatin t chrnic disease management and the prvisin f skilled technical interventins, e.g. subcutaneus infusins, central venus lines and trachetmy management. The District Nurse Sister/Charge Nurse will prvide reciprcal cver and supprt fr the wider district nursing teams in the absence f the designated Case lad hlder in rder t prvide clinical leadership and ensure the cntinuatin f service delivery. Whilst there is n direct financial respnsibility with this pst, the pst hlder has respnsibility fr the rdering f nursing equipment within a defined budget (up t 750 per rder). The pst hlder will be expected t wrk cllabratively with the Senir Nurse and Team leader t ensure effective utilisatin f resurces - this will include the nursing wrkfrce resurce, as well as the effective management f specialist nursing equipment and supplies. The pst hlder will be respnsible fr apprpriate and cst-effective nurse prescribing including dressings, devices and medicatin. The pst hlder will wrk independently and autnmusly withut supervisin and be respnsible fr delegating and reviewing the wrk f his/her team. MAIN TASKS, DUTIES AND RESPONSIBILITIES The pst hlder is respnsible and accuntable fr the nging management f the caselad t ensure the assessment, planning, implementatin and evaluatin f prgrammes f care. These will be develped in partnership with Patients, Family, Carers and Significant thers, and will be evidence based taking int cnsideratin the lifestyle, race, gender and cultural backgrund, and factrs which impact n physical, mental r emtinal health. Maintain accurate up-t-date clinical recrds and care plans in accrdance with NMC standards fr recrds and recrd keeping, ensuring cmpliance with the Data Prtectin Act and the Freedm f Infrmatin Act. The pst hlder will be respnsible fr the utilisatin f varius clinical and envirnmental risk assessment tls in rder t identify actual and ptential risks and ensure implementatin f apprpriate interventins. The pst hlder will als be respnsible fr reprting incidents and critically appraising them in rder t reduce any recurrences and participate in further investigatin as required. The pst hlder will have the skills t respnd t emergency situatins e.g. anaphylaxis and cardiac arrest, by prviding basic life supprt. The pst hlder will use a systematic, flexible and innvative apprach t prblem-slving where there are either barriers t understanding r financial cnstraints within the patient s hme, in rder t prvide a safe and effective care package. Initiate and lead in the develpment and implementatin f clinical audits and instigate and/r participate in research that will ensure cntinuing imprvements in practice and enhance individual and team perfrmance fr the imprvement f patient care.
