Welcome to Neighborcare Health Meridian Center for Health Neighborcare Health at Meridian is located within the Meridian Center for Health, a health center offering comprehensive health care and social services in one location. It is a partnership between Neighborcare Health, Public Health Seattle & King County and Valley Cities Behavioral Health. We work together to provide medical, dental, behavioral health, WIC nutrition, maternity support services and other social services to shared patients. The center also offers community resources, classes, and space for gathering and meeting, fostering better health and wellness for the North Seattle community. Thank you for choosing Neighborcare Health at Meridian Neighborcare Health is committed to creating a health care home, encouraging longterm relationships between patients and providers. This means offering medical care as well as dental care, behavioral health (mental health and substance use recovery) services, health education and other services to care for the whole person. We meet our patients where they are geographically, culturally, physically and emotionally. We also encourage our patients to be actively involved in their own health, ask questions and take an interest in getting well and staying well.

Appointments Medical & Dental: 206-296-4990 Same-day appointments are often available. Please call for exact days and hours of operation, or visit our website neighborcare.org. We are closed most federal holidays. If you are going to be late to or miss your appointment, please call as soon as possible. We require one business day notice if you cancel or change an appointment. This allows us to make your appointment available to others. We will be happy to reschedule your appointment. Same-Day and After-Hours Care For urgent medical or dental care, walk-in services are available on a first-come, first-served basis. Please call 206-296-4990 for a same-day appointment or for after-hours care when we are closed. A free 24-hour nurse advice line is also available at 1-866-418-1002.

We care for you when you are sick and help you stay healthy with regular check-ups and tips for living a healthy lifestyle. We will also help coordinate your care within Neighborcare Health and in the community. Services we offer include: n Preventive primary medical care n Family medicine n Help with chronic conditions, like diabetes n Free 24-hour nurse advice line 1-866-418-1002 n Connection to WIC nutrition and Maternity Support Services in the same building provided by Public Health Seattle & King County n Connection to additional mental health and drug and alcohol recovery services in the same building provided by Valley Cities Behavioral Health n Care coordination with hospitals, rehabilitation centers, and other care providers n Access for you and your family to evidencebased care and self-management support n Diabetes education and management n Gynecology and reproductive health care (family planning) n Pregnancy and newborn care including midwifery n Pediatrics and well-child care n Vaccinations n Screening and counseling of sexually transmitted infections n Nutrition counseling and health education n Integrative behavioral health (mental health, substance use, healthy lifestyle changes) n Social work services n Pharmacy and laboratory services n Referrals to specialists

Dental Services / 206-296-4990 Neighborcare Health at Meridian provides comprehensive general dentistry and emergency dental care to patients of all ages. We are committed to partnering with our patients to improve their dental health and wellness through education, tips for a healthy diet, and regular access to dental care. A healthy mouth is essential to keeping the rest of your body healthy, therefore we want to see you regularly for your dental check-ups and care. Services we offer include: n Dental exams, X-rays, cleanings, sealants, fillings and extractions n Oral health education Pharmacy Services / 206-548-5900 For your convenience, Neighborcare Health at Meridian has a pharmacy on site. n Pharmacy staff can work directly with your provider on site. n Pharmacy staff members can answer questions in many languages. n Reduced prices on medications are available to patients who qualify for our sliding-fee discount program. If you use a non-neighborcare Health pharmacy, please coordinate with your pharmacy for your prescription requests. n Specialty care, including limited oral surgery, crown, denture and root canal services n Emergency care services for all patients on a first-come, first-served basis Interpreter Services Patients speaking more than 45 different languages and dialects visit our clinics. We have interpreters and other staff members at our clinics who speak Somali, Spanish, Vietnamese and many other languages. We also provide on-site interpretation services in other languages.

What to Bring If you are a new patient: Please bring any written records of your medical care outside of Neighborcare Health. Also, please bring the following: n Any medications you are currently taking. n Your insurance or ProviderOne card. If you are uninsured: We ask all uninsured patients to make an appointment with an eligibility specialist to learn if they qualify for health insurance through the Washington Healthplanfinder, the Neighborcare Health Sliding Fee Discount Program, or other programs. Please bring the following documents to your eligibility appointment: Income Verification: n Taxable earned gross income for the past 30 days for every working member in household. (Ex., pay stubs, tips, pension, unemployment, alimony, unearned income, etc.) n 1040 Federal Tax Form (most recent year) n Other documentation (if applicable) Employer letter containing employer name and address, and income information. Award letter of benefits (Ex., Social Security, pension, etc.) Unemployment award letter or last two unemployment check stubs. A letter of support or reference from an individual or organization. Identity and Residence Verification: n If you are a US citizen, please bring Social Security numbers, Certificates of Naturalization (if available,) passports (if available) and names of family members to be enrolled. n If you are a legal immigrant, please bring a Permanent Resident Card (green card), passport (if available) and Social Security numbers of family members to be enrolled. n A current photo ID such as driver s license, state-issued identification card, or military ID. If you do not have access to these documents please speak to an eligibility specialist. We provide services regardless of immigration status. Other Important Information: n An email address, if available. n Additional documents may be required on a case-by-case basis. Family Size Verification: n Birth and marriage certificates for all family members if 1040 Federal Tax Form is not available.

Fees and Insurance We believe that quality health care should be available to everyone. No one is denied services due to ability to pay. We accept Washington Apple Health (Medicaid), Medicare and most types of insurance. Proof of insurance or payment is expected at the time of service, including co-pays. We ask uninsured patients to meet with an eligibility specialist to determine if they are eligible for public insurance or a sliding fee discount. If you are uninsured and choose not to apply for public insurance or the sliding fee discount, we ask for a deposit to cover the initial cost of your care. Deposits: n $80 for medical n $90 for dental $5 discount if fee paid at time of your appointment If you qualify for the sliding fee discount program, the fees start at: n $20 for a medical appointment n $45 for a dental appointment $5 discount if fee paid at time of your appointment Neighborcare Health patients are billed directly by LabCorp for any lab tests and services. Patients are responsible for paying LabCorp directly. Neighborcare Health informs LabCorp if a patient is eligible for the sliding fee discount, and LabCorp applies the discount. Patients with questions or concerns about a bill are welcome to contact our billing department at 206-548-3100. Neighborcare Health is a non-profit organization. Public grants and insurance reimbursement cover only a portion of our expenses. Even a small tax-deductible gift can help cover the unmet need to care for a patient. thank you!
