JOB TITLE: Universal Worker DEPARTMENT: Ridgewood DATE ISSUED: April 1995 DATE REVISED: July 2012 JOB SUMMARY: PERSONAL CARE ATTENDANT sists residents with activities of daily living, providing nursing care, housekeeping services and other related services as is necessary to meet the personal needs and comfort of the residents and assist in maintenance of a safe, clean environment. ESSENTIAL DUTIES AND RESPONSIBILITIES: Provide for personal hygiene care which may include; complete or partial bath, back care, oral hygiene, combing hair, shaving, toileting, dressing resident, changing bed linen and other personal care as is necessary. Demonstrates competencies in computer programs: Matrix, Achieve and EMR. Provide for assistance with medications including glucometer monitoring and sub q injections. Participates in and directs both divisional and planned activities. Records activities in activity book. Responds appropriately to emergency situations i.e.; ambulance response, 911, first aide, CPR. Organize and order supplies weekly and as. Supervise and encourage participation in exercise classes, social and recreational activities. sists residents with handling and care of personal laundry. Performs routine treatment procedures including; application of appliances for heat/cold, routine skin care, hand held nebulizers, oxygen, pulse ox, and other treatments. 1
Takes and/or records temperature, pulse, respiration, weight, and intake/output. Maintains records as. sess and monitor physical, cognitive and social dimensions of the resident. Observes, monitors and reports any adverse symptoms, findings or changes in resident s physical or cognitive behaviors to LPN, Administrator or to the resident's physician. Review and respond if necessary, to results of resident lab and other diagnostic tests. Record accurately, clearly and concisely all pertinent information on resident records. Interprets, documents, transcribes onto resident records, the physician's plan of care. Instructs and counsels residents on the same. Promptly responds to all call bells, and provides for resident care needs accordingly. Responsible for reporting any changes observed in condition or behavior and any unusual incidents. Provides for resident privacy at all times, knocking before entering room, closing room doors, etc. before providing care. Serves resident meals in a family style environment. Establishes and maintains interpersonal relationships with residents assuring confidentiality of resident information. Participates in and contributes to resident care needs. Attends in-service education programs as assigned/required. Responsible for receiving and replacing call-offs on off shifts. sists in maintaining a safe and clean environment for residents and staff. QUALIFICATIONS NECESSARY TO PERFORM ESSENTIAL DUTIES AND RESPONSIBILITIES: The Department of Public Welfare Personal Care Home regulations require that personal care attendants be 18 years of age or older, have a high school diploma, GED or active registry status on the PA nurse aide registry. Staff must successfully complete an approved course on medication administration. Staff must be free from all medical conditions, including drug or alcohol addiction, which would limit staff from providing necessary personal care services with reasonable skill and safety. Previous experience in hospital or nursing home preferred. Must possess a valid drivers license. Must successfully complete first aid and CPR certification or possess current certification. Must be able to lift, turn and reposition residents both alone and with assistance with and without the use of mechanical devices. Must also be able to assist other with lifting, turning and repositioning residents both with and without the use of mechanical devices. Must be able to perform the physical requirements of the position including lifting residents with help. JOB SUMMARY: DIETARY AIDE/PRODUCTION WORKER/COOK Provides a variety of Food and Nutrition Department functions as including production, cook, dietary aide and cleaning to insure safe and 3
sanitary meals are prepared for residents and staff while promoting a cooperative relationship with other all facility personnel. ESSENTIAL DUTIES AND RESPONSIBILITIES: Prepares menu items as assigned to assure the menu is ready for service at appropriate meal times. Plate resident s meals for serving, keeping presentation of plate looking nice. Operates dishwasher, washes pot and pans and puts all clean items away in appropriate areas. Resets dining room for meals. Maintains department sanitation, safety and productivity standards. Maintains daily taste and temperature log. Prepares production sheets. Puts away stock. May be responsible to transport supplies between facilities as necessary. Cleans kitchen equipment, vacuums and mops floors. May bag bread, fill milk and water pitchers, brew coffee and fill coffee pots, pour milk, prepare toast, fill condiments, make juice, peel potatoes. Will be required to participate in the planning and implementation of special functions within the personal care facility. JOB SUMMARY: HOUSEKEEPER According to established procedures, cleans and services building areas. Moves furniture, equipment and supplies in and around facility departments; and performs a variety of environmental service duties to maintain the facility in a neat, orderly and sanitary condition. ESSENTIAL DUTIES AND RESPONSIBILITIES: Selects cleaning materials and supplies and prepares solutions according to procedure. Loads service cart and transports to work area. Requests cleaning supplies and equipment as. Operates various types of cleaning equipment both mechanical and electrical, i.e. buffers, scrubbers, vacuums. Cleans equipment and notifies Manager of Environmental Services of equipment needing repair. Returns unused supplies to janitor closet, cleans carts and equipment and ensures all items are replaced in an orderly manner. Maintains vinyl floors in resident rooms and common areas per maintenance requirements from manufacturer. Responsible for periodic washing of windows in resident rooms and common areas. Clean windows and blinds once a month, and outside 1-2 times annually. Keep waterfall and fountain full spring to fall. Clean up dead flowers every fall. Personal Care Facility Performs procedures 1-8 in resident rooms per cleaning schedule, storage, staff base, central bath area, public restrooms, lounges, entrances, offices, beauty shop, laundry room, pantry and all other common areas. Performs procedures 1-3 - 4-6 in all corridors and common areas. NON-ESSENTIAL DUTIES AND RESPONSIBILITIES: May assist in resident moves within facility or to health care facility and may be required to transport residents within building. Attends meeting and training seminars as required. May climb ladder to reach high areas for cleaning purposes. May collect waste from all areas of facility and may sanitize all garbage cans. May collect or distribute soiled or clean linen. May assist Personal Care Attendants with activities, meals, personal care. Performs other duties as assigned.
