Additional Ordering Requirements Computerized Prescriber Orders Customized order form template must be reviewed by Research Pharmacy Services prior to first use via ecris Scheduled (II-V) medications Contact us for requirements Order completion All required elements Approved abbreviations only (www.ohsu.edu/pharmacy) Signatures Who: MD, PA (special requirements may apply), NP (only if FDA approved medication) NCI studies: ONLY CTEP Investigators who are registered with CTEP and in good standing (not expired status). NO nurse practitioners or fellows. No signature stamps Verbal orders Advance orders Ongoing Requirements Monitoring visits at least 48 hours advanced notice required, however 4 to 6 weeks notice recommended Notify RPS on all: Protocol modifications Change of drug dose or handling of drug Changes related to number or frequency of treatments or changes in supply of investigational drug (e.g., change to open-label, or new dosage form/strength) If NCI study, please notify of all protocol/ib updates IDB Updates Pharmacy termination IRB termination Top Things to Remember When Using RPS 1) Contact RPS early and often 2) Remember to budget for RPS 3) Notify RPS in writing about status of study (termination, extension, changes in protocols, etc) 4) Provide advanced notice for upcoming monitor visits 5) Provide accurate and up-to-date account numbers 5

Top Things to Remember when Using RPS 6) Send COMPLETE & SIGNED orders in a timely fashion 7) Outsourcing of pharmacy services must be done with the knowledge & approval of RPS 8) No samples all drugs dispensed in the outpatient setting at OHSU must be labeled per ORS and dispensed from a licensed pharmacy 9) Notify RPS regarding patient schedules and visits 6
