einteract Overview of einteract INTERACT (Interventions to Reduce Acute Care Transfers) is a quality improvement program focusing on management of acute change in resident condition. It includes clinical and educational tools for use in every day practice in long-term care facilities. The einteract components are designed to work together to assist you in preventing unnecessary hospitalizations and to promote positive resident outcomes. einteract Goals: Preventing unnecessary hospitalization of residents by promoting early identification and assessment of changes in resident condition. Managing changes in condition of the residents in the facility when it is feasible and safe. Expediting pertinent documentation to communicate resident condition to acute care hospital staff in the event of a necessary transfer. einteract Tools: Stop & Watch Alerts Change in Condition Alerts and Change in Condition Evaluation (must be enabled by a PointClickCare representative). Transfer Form QI Tools Hospital Transfers Portal Security for einteract The following are security permissions related to einteract of the Clinical application. There are two Clinical Setup Role (system) roles available or you can create a custom role with the following permissions. 1. Do one of the following: Single Facility: Admin > Setup > Security Roles. Multi-Facility Organizations: Management Console > Standards > Financial Management > Security Roles. Page 3
2. Edit an existing or create New security role(s). Click Save. 3. Ensure appropriate users are assigned to the role(s). Determines the access a user has within Setup - A/R Configuration. Determines a user's access level to A/R Configuration. Major Module = EMC Multi-facility Organization Only Setup - Care Content Directory EMC - Clinical Reports (parent) Rehospitalization Report (N/R/A/E/D) - Determines a user's access to Care Content Directory. R and higher allows only read access to the Care Content Directory. Determines the access a user has within the Management console to Clinical Reports. Y - Access to this report. Reviewing Admin and Clinical Setup for einteract There are sections of Admin and Clinical Setup that must be reviewed as part of einteract to confirm the accuracy of the settings and maximize the functionality of the module. Admin Setup Pick Lists: To/From Type - Confirm To/From Types are mapped to correct External Facility Type. Pick Lists: Contact Types - Confirm Contact Types are mapped to correct Standard Contact Type. Pick Lists: External Facility Types - Confirm all appropriate External Facility Types are present. External Facilities - Confirm names of all External Facilities are entered. A/R Configuration - Confirm Force To/From Location in Census for Hospital To/From Type is selected. General Configuration - Confirm CCD Preview is selected. Clinical Setup Standard Dashboard Layouts - Confirm the applicable dashboard items are selected for the appropriate positions and modules. Enabling the Hospital Transfers Portal, Transfer Form and Quality Improvement (QI) Tools Enabling the einteract program provides you with the tools to collect essential resident information regarding hospital transfers. WARNING The Hospital Transfers portal and the Transfer Form must be enabled first to enable the QI tools. After the Hospital Transfers portal is enabled, it cannot be disabled. 1. Clinical > Setup > General Configuration > Enabling the einteract Program. 2. Select Yes for option(s). 3. Click Save. Hints and Tips There are two pre-defined schedules for the Transfer Form for the trigger types of Discharge and Transfer Out. These schedules cannot be changed. After QI Tools are enabled, QI Icons appear under the Transfer Log and there are QI Reviews and QI Analysis tabs in the Hospital Transfers portal. You can disable the Transfer Form by selecting No and clicking Save. The Transfer Form is no longer available for use and all schedules associated with the form are cleared from the resident Assmnts tab. You can disable the QI Tools by selecting No and clicking Save. Enabling the Stop and Watch Alerts The einteract Stop and Watch Alerts allow you to identify resident changes. Early identification of resident changes helps to decrease unnecessary hospitalizations and improve resident outcomes. After enabling, the Stop and Watch Alerts are available in the Standard Alert Library and any area where you can trigger alerts. WARNING After being enabled, the Stop and Watch Alerts cannot be disabled. Page 6
1. Clinical > Setup > General Configuration > Enabling the einteract Program. 2. Select Yes. 3. Click Save. Hints and Tips The einteract Stop and Watch Alerts cannot be modified or deleted. When the Stop & Watch Alerts are enabled, they appear in the Clinical Alerts pane. If the Change in Condition Alerts are enabled, then all of the einteract alerts appear in the Stop & Watch and Change in Condition Alerts pane. Managing the Transfer Form and Change in Condition Evaluation After the einteract Transfer Form and Change in Condition Evaluation are enabled, you cannot edit the content or order of the questions. The assessments are located under the Ready for scoring section so you can access the triggers, lookback and narrative links as required. WARNING Create all of the narratives before creating the template for the Structured Progress Note (SPN). After the template is started and saved, the narrative text boxes are disabled and cannot be edited. After the Transfer Form and Change in Condition Evaluation Guide are placed online, it is not possible to move them back to create the SPN at a later date without losing the einteract designation. 1. Do one of the following: Single Facility: Clinical > Setup > Custom Assessments > Ready for Scoring. Multi-facility: Management Console > Standards > Care Management > Custom Assessments > Ready for Scoring. 2. Click view. 3. Complete the fields as required. 4. Click Save. Page 7
Configuration Options Below is a list of the configuration options available to you and an explanation of the impact of each configuration setting. 1. Triggers: You can select more than one trigger for each category. Review Adding Triggers to a Question [9] for more information. 2. Lookback: (Available in POC enabled facilities only). The lookbacks allow you to map specific Care Plan interventions or tasks to a specific question within the assessment. Review Adding Lookbacks to a Question [11] for more information. 3. Narratives: Use the narrative link to create alternate narrative text in setting up an SPN. When creating an SPN, you can select the narrative response as opposed to the regular response. Review Creating Narrative Responses [12] for more information. Hints and Tips If the Transfer Form or Change in Condition Evaluation is retired, any due or overdue scheduled, but not opened, assessments are removed from the resident's Assmnts tab. After retiring the Transfer Form or Change in Condition Evaluation, all created assessments with a status of In Progress are available for completion in the resident's chart. To reactivate the Transfer Form or Change in Condition Evaluation, select the Make available online link to the left of the retired assessment. Page 8
Appendix A - Adding Triggers to a Question After questions are added to an assessment, you can add triggers for certain question types. Triggers are suggested items that push out to other areas of the application based on a question response. There are several trigger types; below is a listing and definition/explanation of each. Care Plan Triggers - You can set a trigger for a Focus, Goal and/or Intervention. Triggered items flow to the View Triggered Items Now section of the resident's Care Plan. Task Triggers - You can set a trigger for a Task using Focus Categories or the Task Library. Triggered items flow to the View Triggered Items Now section of the resident's Tasks tab and/or Care Plan. Assessment Schedule Triggers - You can set a trigger for an assessment. Triggered items flow to the Next Assessment Due section of the Resident's Assmnts tab. High Risk Alert Triggers - You can set a trigger for a High Risk Alert using the Standard Alert Library, einteract alerts (if applicable) or by creating a new alert. Triggered items flow to the Clinical Dashboard. einteract Triggers (available in Change in Condition Evaluation Guide only): Allows you to set up and view einteract Trigger notifications in the Alert pane on the Clinical Dashboard and resident Dashboard. Care Program: Creates a Care Program in the Care Program tab of the resident's chart. Care Programs must be enabled in your facility to see this option. 1. Do one of the following: Single facility: Clinical> Setup > Custom Assessments Multi-facility: Management Console > Standards > Care Management > Custom Assessments 2. Click edit for the assessment. If in a multi-section assessment, click edit for the section you want to add a question trigger to. 3. Click triggers for the question. 4. Triggers for Response: select the response. 5. Click New for the Trigger > select the item(s). 6. Click Save. Hints and Tips For Assessment Schedule Triggers, use the Triggering Type of Trigger. Page 9

Appendix B - Adding Lookbacks to a Question After questions are added to an assessment, you can add lookbacks. A lookback allows you to view documentation from Point of Care. Interventions or Tasks that are documented on can be viewed from within the assessment (using the question mark icon) and may assist you in answering the question. 1. Do one of the following: Single facility: Clinical > Setup > Custom Assessments. Multi-facility: Management Console > Standards > Care Management > Custom Assessments. 2. Click edit for the assessment. If in a multi-section assessment, click edit for the section you want to add a question lookback to. 3. Click lookback for the question. 4. Click New for Interventions or Tasks. 5. Select the Intervention or Task > Save. The Intervention or Task appears as a facility scoped item. 6. Click edit for the item to update the Look Back Period and/or Scoping. Page 11
Appendix C - Creating Narrative Responses After questions are added to an assessment, you can add narratives for certain question types. Narratives are used in a Structured Progress Note (SPN), and they are optional. When creating an SPN, you can select the narrative response as opposed to the regular response. If you are using narratives, they must be added while the assessment is in the Under Construction section. They cannot be added after the assessment is moved to Ready for Scoring/SPN Setup. 1. Do one of the following: Single facility: Clinical > Setup > Custom Assessments. Multi-facility: Management Console > Standards > Care Management > Custom Assessments. 2. Click edit for the assessment. If in a multi-section assessment, click edit for the section you want to add a narrative response to. 3. Click narrative for the question. 4. Enter narrative text for each question response. This text can be inserted into an SPN and appears exactly as typed (including punctuation) in place of the regular response(s). 5. Click Save & Close.
