ANAHUAC INDEPENDENT SCHOOL DISTRICT REQUEST FOR QUALIFICATIONS Architectural and Engineering Services for Future Potential Bond Projects INTRODUCTION: Anahuac Independent School District ( District ) is seeking statements of qualifications from selected design firms to provide long range facility plan, architectural and necessary related engineering sub-consulting services for contemplated facilities improvements and renovations. The projects are to be funded by future potential bond referendum. Services will include evaluation, refinement, and further development of current facilities master plan, facilities need assessment and programming in preparation for future potential bond referendum, as well as preparation of design documents and contract administration during construction of projects, subject to voter-approval of a bond referendum.

The District will review the completed submittals received by the deadline and may select one or more successful firms. As required by law, the District will make the selection based on the most highly qualified firm for the project types. The selection process will be based on a review of the firm s qualifications, completed projects, project specific needs and client references. Fee negotiations will occur with the most highly qualified firm selected by the District. Responses to the Request for Qualifications shall include the complete submission requirements in the sequence and format prescribed. Provide submissions in a letter-sized binder approximately 10 x 11-1/2 x 1-1/2 deep or less. Page 2

All questions must be submitted in writing by no later than September 21, 2015, and responses to questions will be forwarded to all RFQ responders who provide e-mail addresses or fax numbers to the District for this purpose. Under project references, clearly state the name of the firm who performed the referenced work. If work was performed under the employment of another firm, indicate so by naming that firm. Provide a description of the specific work performed by the firm or individual referencing each project. Front of Binder: Letter of interest. RFQ Submission Checklist showing the type of work best suited to your firm. Tab No. 1: Firm Profile Data (name, address, number of offices, year business was established). Contact person. Personnel (List total number of staff employed in local office: architects, intern architects, construction inspectors, specification writers, administration and clerical staff). Brief history of firm. Financial information and statements for past two years. Volume of work (for last five years). Volume of school district work (for last five years). Professional liability insurance (Limits, Deductible, Claims to Date) include proof of coverage. CAD capabilities (Number of CAD computers, software, support staff). Tab No. 2: Experience General experience of firm. Describe your most successful project incorporating similar project design criteria and why should the District should choose your firm. List of school district and other education entity clients. List of significant school district and education projects, including the following client information: project, client, construction representative, telephone number, date of start and finish, SF, construction contract amount, total construction cost at end of project, cost/sf, contractor, contract type. Note: You may insert photos, floor plans, comments on projects in 8-1/2X11 format or you may submit them separately. Limit ten projects. List related experience. List project delivery methods that you have employed. List awards and recognitions or specialized expertise. List any experience related to bond planning and preparation. List of proposed engineering sub-consultants. Tab No. 3: Personnel, Process List of key personnel and responsibility with specific emphasis on personnel who will be responsible for project including any certifications (i.e. LEED, CHPS, etc.) which they may currently possess. Résumés. Work process, schedule. Documentation of design for presentation. Firm s approach to contract documents, bidding and construction administration. Firm s approach to quality assurance. List of preferred consultants. List proposed personnel from both your firm and proposed engineering sub-consultants. Tab No. 4: References (Name, organization, telephone, fax, & e-mail if available) Provide five client references. Provide five contractor references. Provide five sub-consultant references. Tab No. 5: Other bound information You may submit other relevant information about your firm here. The examples are to be folded as compactly as possible for storage with your submission binder. They will not be returned. The work must have been completed by the firm submitting the project and not under the name of another firm. Provide the name of the school district and a representative who can be consulted about the work. PROJECTS MAY INCLUDE BUT ARE NOT LIMITED TO: School Gym Performing Arts Center Professional Development/ Multi-Media Center Football Field/Track and Field Complex Baseball and Softball Complex Additions and demolition Technology infrastructure SELECTION PROCESS FOR PROFESSIONAL SERVICES The selection of an architectural firm will be made based upon determining the most highly qualified firm, as required by the Chapter 2254 of the Texas Government Code and District policy. The firms determined to be most highly qualified, based on the submission and references, may be placed on a short-list. The District may request interviews, presentations, and tours prior to finalizing the selection process. The District reserves the right to waive any irregularities and informalities and make any decision that they judge is in the best interest of the school district. The District reserves the right to cancel this RFQ, and is in no way obligated to proceed with any project or to enter into a contract with any firm. Any selected firm will be required to (1) complete all required criminal history information certifications required under Texas Education Code Section 22.0834; and (2) submit proof of acceptable insurance limits for both your firm and proposed engineering sub-consultants. CONFLICT OF INTEREST QUESTIONNAIRE For vendor or other person doing business with local governmental entity This questionnaire reflects changes made to the law by H.B. 1491, 80th Leg., Regular Session. This questionnaire is being filed in accordance with Chapter 176, Local Government Code by a person who has a business relationship as defined by Section 176.001(1-a) with a local governmental entity and the person meets requirements under Section 176.006(a). By law this questionnaire must be filed with the records administrator of the local governmental entity not later than the 7th business day after the date the person becomes aware of facts that require the statement to be filed. FELONY CONVICTION NOTIFICATION State of Texas Legislative Senate Bill No. 1, Section 44.034, Notification of Criminal History, Subsection (a), states a person or business entity that enters into a contract with a school district must give advance notice to the district if the person or owner or operator of the business entity has been convicted of a felony. The notice must include a general description of the conduct resulting in the conviction of a felony. Subsection (b) states a school district may terminate a contract with a person or business entity if the district determines that the person or business entity failed to give notice as required by Subsection (a) or misrepresented the conduct resulting in the conviction. The district must compensate the person or business entity for services performed before the termination of the contract. This notice is not required of a publicly held corporation.
