NOTICE SARPY COUNTY WELLNESS PROGRAM The Sarpy County Wellness Program is a voluntary wellness program available to all employees; and Spouses who are on the medical plan. The program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act, as applicable, among others. If you choose to participate in the wellness program you will be asked to complete a voluntary health risk assessment or "HRA" that asks a series of questions about your health-related activities and behaviors and whether you have or had certain medical conditions (e.g., cancer, diabetes, or heart disease). You will also be asked to complete a health screening, which will include a blood test for cholesterol and blood glucose. You are not required to complete the HRA or to participate in the blood test or other medical examinations. However, employees/spouses who choose to participate in the wellness program will receive an incentive: Completing a health risk assessment questionnaire Completing the on-site health screening or submitting a preventive visit from your physician (Physician Forms available for those who choose this option) Only employees/spouses who complete the above requirements will receive the incentive. If you are unable to participate in any of the health-related activities or achieve any of the health outcomes required to earn an incentive, you may be entitled to a reasonable accommodation or an alternative standard. You may request a reasonable accommodation or an alternative standard by contacting Lockton Health Risk Solutions by phone at 1.888.251.2260 or by email at membersupport@lockton.com. The information from your HRA and the results from your health screening will be used to provide you with information to help you understand your current health and potential risks, and may also be used to offer you services through the wellness program, such as health coaching. You also are encouraged to share your results or concerns with your own doctor. 3

Protections from Disclosure of Medical Information We are required by law to maintain the privacy and security of your personally identifiable health information. Although the wellness program and Lockton Health Risk Solutions may use aggregate information it collects to design a program based on identified health risks in the workplace, Lockton Health Risk Solutions will never disclose any of your personal information either publicly or to the employer, except as necessary to respond to a request from you for a reasonable accommodation needed to participate in the wellness program, or as expressly permitted by law. Medical information that personally identifies you that is provided in connection with the wellness program will not be provided to your supervisors or managers and may never be used to make decisions regarding your employment. Your health information will not be sold, exchanged, transferred, or otherwise disclosed except to the extent permitted by law to carry out specific activities related to the wellness program, and you will not be asked or required to waive the confidentiality of your health information as a condition of participating in the wellness program or receiving an incentive. Anyone who receives your information for purposes of providing you services as part of the wellness program will abide by the same confidentiality requirements. The only individual(s) who will receive your personally identifiable health information are business associates of Sarpy County and a registered nurse in order to provide you with services under the wellness program. The personally identifiable medical information obtained through the wellness program will not be shared with anyone but you. Your information is stored electronically encrypted, and no information you provide as part of the wellness program will be used in making any employment decision. Precautions will be taken to avoid any data breach, and in the event a data breach occurs involving information you provide in connection with the wellness program, we will notify you immediately. You may not be discriminated against in employment because of the medical information you provide as part of participating in the wellness program, nor may you be subjected to retaliation if you choose not to participate. GINA AUTHORIZATION FORM Required: Authorization to be completed by SPOUSE prior to the collection, in exchange for an incentive, of information regarding the spouse s health history or current health status. Sarpy County offers a wellness program to certain employees and their dependents. As part of the wellness program, spouses are invited to complete a voluntary health risk assessment (HRA) through which the spouse will provide information about his or her health history, health status or both. We may provide financial or other incentives to employees whose spouses participate in the HRA. Your participation in the HRA is voluntary. You are not required to participate in the HRA. If you decide to participate, and if there are questions in the HRA related to your own family medical history or your own genetic information (these questions, if any, are identified on the HRA), you are not required to answer them in order for the employee to receive any incentive we offer for your completion of the HRA. We ll use the health information you provide to help you. Findings gathered from the HRA will be used to provide you with information to help you understand your current health and potential risks, and may also be used to offer health-related services to you. Your health information is confidential. We are required by law to maintain the privacy and security of your personally identifiable health information. Unless we are conducting the HRA, the medical information collected will not be available to us in a way that allows us to identify you or the employee. However, we may use aggregate or summary (e.g., de-identified) information from the HRA to design or provide additional health services. Any individually identifiable medical information we obtain through the wellness program will be maintained separate from personnel records, information stored electronically will be encrypted, and no information you provide will be used in making employment decisions. Appropriate precautions will be taken to avoid a data breach, and in the event a data breach occurs, involving information you provide in connection with the wellness program, we will notify you promptly after learning of the breach. Your health information will not be sold, exchanged, transferred, or otherwise disclosed except to the extent permitted by law to carry out specific activities related to the wellness program (including the health plan which it is a part of), and you will not be asked or required to waive the confidentiality of your health information as a condition of participating in the wellness program or our provision of an incentive. Anyone who receives your information for purposes of providing you services as part of the wellness program is required to abide by the same confidentiality requirements. In addition to you, the only individuals who will receive your personally identifiable health information will be licensed health care professions and board certified genetic counselors in order to provide you with health or genetic services under the wellness program. We may disclose your information as necessary to respond to a request from you for a reasonable accommodation to allow you to participate in the wellness program, or as expressly permitted by law. Steps 1 and 2 of the program include a self-report of health habits as well a health screening. We are excited to continue with you on your wellness journey with Sarpy County in 2018. Enclosed you will find: Instructions for completing the Online Health Risk Assessment. NOTE: you must complete the Online Health Risk Assessment in order to participate in the health screening. Dates for the on-site health screenings and instructions. Details about the health report review with a Lockton nurse and how to engage in health coaching with a Lockton Nurse Advocate. Participation in the Health Risk Assessment and health screening is REQUIRED for the Wellness Incentive. See www.sarpy.com/wellness/incentive.html for more information about the Wellness Incentive. The Lockton Health Risk Solutions Team and Sarpy County STEP 1: Online Health Risk Assessment (HRA) The online HRA will open February 5, 2018 and close February 19, 2018. You will log on to the Health Risk Solutions Webpage http://www.locktonforhealth.com and click the "Take the Health Risk Assessment" Box: THEN Click the English button: Next, you will have to enter your Employer ID # 121018. At the time you begin the HRA, an e-mail will be sent to your e-mail address containing a 14 digit personal login. You will only need to use this in the event that you would need to stop the HRA and restart where you left off at a later time. If you do not have computer access and need to complete a paper HRA, contact Sarpy County Human Resources at 402-593-4465. If you do not have an email address, please use NurseLine@lockton.com when completing your HRA. If you completed the HRA in previous years, you will need to create a new username for 2018. Re-entering your 2017 HRA and making the changes for this year will not qualify as completion. STEP 2: On-Site Health Screenings 1. The on-site health screenings are tentatively scheduled for the end of February through the beginning of March. Human Resources will contact you with information regarding how to self-schedule appointment times. The health screening (blood draw) will test for blood chemistry, complete blood count, complete lipid panel, hemoglobina1c, and thyroid stimulating hormone (TSH). If you prefer to complete the health screening as part of an age related preventative visit with a personal physician, please contact Human Resources at 402-593-4465 for the appropriate form. The laboratory testing included with On-site screening may be different than what is allowed as preventive at a physician visit. 2. You must fast for at least 10 hours prior to the screening; however, you should drink plenty of water as it will help with the blood draw. 3. You can and SHOULD take your medications (EXCEPT diabetes medications). Bring your diabetes medications with you and take them with food AFTER your blood draw. If you have any questions about whether or not you should take a certain medication, please contact your healthcare provider for advice. 4. Please avoid drinking alcohol the night prior to the test, as it may affect test results. 5. If you are pregnant, you are not required to participate in the health screening process. Please have your physician fax Lockton HRS (Attn: Brittany Whittington) a note that includes your full name, date of birth, and that you are currently pregnant and under the physician's care. We will document that you are complete in our files. We will respect your privacy in this matter. The Lockton HRS fax number is secure: 1-888-251-2264. 6. Please bring a photo ID and your insurance card with you to your screening appointment. 7. Once you obtain your results, you are welcome to visit with a Lockton nurse who will explain your results and answer your questions. We also encourage you to take advantage of the Nurse Advocate Program, which is described in the attached flier. 7

2018 Wellness Nurse Advocate Program We are excited to announce that have we have partnered with Lockton Health Risk Solutions as part of our wellness program. This program is one more resource that supports Sarpy County s goal to improve the health of our employees. The Nurse Advocate will not try to sell you any products, services, insurance, etc. It is simply a great FREE resource to help you maintain a healthy lifestyle! The Nurse Advocate can be contacted by phone or email for private appointments to: Meet you and introduce the program Learn about your health and lifestyle Review your wellness results if you have participated in the health screening program Help you learn about how to manage your personal risk factors Teach you about important medical care that is recommended for your best health The Nurse Advocate program is a voluntary program and an added benefit resource for Sarpy County Employees. In order to achieve our wellness goals and objectives, the Lockton nurses will have access to your health information including: biometric screening results, annual physical results, and claims information from our insurance company. Health information provided to the Lockton clinical nurses is confidential and only used for purposes of care coordination for your overall wellbeing. The integration of health information establishes a foundation to identify health risks and coordinate personalized health care and is not shared with any employee of Sarpy County or our insurance company. Health information is maintained in strict privacy in accordance with federal regulations. 8
