In additin, they will help ensure achievement f service bjectives that cntribute t the delivery f Smkefree services at Nrth Shre and Waitakere Hspitals including all activities relating t the Ministry f Health Better help fr smkers t quit health target. Service verview Referrals will be sught and accepted frm within the DHB, and fllwed up with an assessment f the patient s barriers t quitting. Pririty will be given t r thse n the elective surgery waiting list Patients will be ffered persnalized advice n a range f pharmaclgical supprt ptins including nictine replacement therapy (NRT), Nrtriptyline, Buprpin, and Varenicline. Patients will be ffered telephne r ne-t-ne supprt fr up t 2 mnths after they have successfully quit. Telephne supprt will be ffered fr an additinal 12 mnths after they have quit. Data will be managed accrding t standard prcedures with all participant dcumentatin cmpletely and securely filed. Data will be cllected, stred and reprted in a cnfidential manner, in accrdance with the principles f the Privacy Act 1993. Systems will be put in place fr recrding, updating and string infrmatin including: Clients referred; Clients fllwed up and assessed fr readiness t quit Type f interventin (brief, intensive, relapse etc.) Client prfile Number f clients enrlled n the prgramme; Number f clients quit at ne, three, six and twelve mnths Prvide data n request t the Hspital Smkefree Manager and participate in evaluatins and audits f the smking cessatin service. Netwrks Develp wrking relatinships with tbacc cntrl and smking cessatin services thrughut the Auckland regin, and linkages with natinal tbacc cntrl services, and wrk cllabratively n reginal tbacc cntrl activities impacting n DHBs and primary care agencies, specifically PHOs Wrk cllabratively with ADHB, CMDHB, Waikat and Nrthland smkefree crdinatrs and smking cessatin specialists.
