And because we will crdinate all f yur care needs, TRU PACE cannt guarantee r prmise that yu will cntinue t see yur specialty prvider(s). Why des this happen? When participants jin PACE, their care is fully crdinated frm visits t the Center t the Clinic t hme and having t many prviders can cause cnfusin fr all f us. Crdinating all yur care is an imprtant cncept f the PACE mdel because it has been shwn that PACE case management will imprve a participant s lngevity, their health, and quality f life.
