3. This project and the quarry shall apply f(jr coverage under ADEC's APDES Multi-Sector General Permit (AKHOSOOOO). This permit requires tl1at a Storm Water Pollution Prevention Plan (SWPPP) describing best management practices to control erosion aud runoff during facility operation be prepared and implemented. Please refer to AD<~C's website for information concerning the APDES storm water permit. 4. Any disturbed ground and exposed soil not covered with fill must be stabilized and revegetated with endemic species, grasses, or other suitable vegetation in an appropriate manner to minimize erosion and sedimentation, so that a durable vegetative cover is established in a timely manner. 5. All work areas, material access routes within the project footprint as described in the Corps Public Notice P<'JA2010496, and surrounding wetlands involved with the road constnwtion shall be dearly delineated and marked in such a way that equipment operators do not operate outside ofthis corridor. 6. Organic overburden soil stockpiles shah be stabilized as soon as practicable after placement to minimize erosion or sediment runoff. 7. No materials used for fill placement shall consist of unsuitable fill material. This includes but is not limited to trash, meta) debris, overburden material, or wood waste. All material used for construction or discharged must be free from toxic pollutants in toxic amounts. BOX 6898 JBER, ALASKA 99506-0898 OCT 2 9 2013 Regulatory Division POA-201 0-496-M2 DEPARTMENT OF THE ARMY PERMT MODFCATON Department of the Army permit number POA-201 0-496-M1, Shakmanof Cove, was issued to Koniag, nc. on September 6, 2012, to change the fill for the barge landing from temporary to permanent, increase fill for the beach work pad by 4.1 acres and to replace culverts at the beach work pad with a stream diversion, and to increase fill for the shoreline roadway by 1.69 acres. This modification totaled an additional 5. 79 acres of fill into waters of the U.S., including wetlands, with total project impacts of 13.7 acres. This is the 2nd modification of the original permit. The permit is hereby modified as follows: - To construct an additional 2 breasting dolphins consisting of two 24-inch steel pipe piles, one vertical and one battered; construct a removable 25-foot steel ramp supported by two 24-inch steel pipe piles; and to place an additional 0.02-acre of fill below the High Tide Line. The work will be performed in accordance with the enclosed plans, sheets 1-5, dated July 2013, which are incorporated in and made a part of this Permit Modification. The project site is located within Sections 13 and 24, T. 26 S., R. 21 W., Seward Meridian; USGS Quad Map KODAK D-2; Latitude 57.9159 N., Longitude -152.6073 W.; located approximately four miles southwest of the community of Ouzinkie, near Kodiak, Alaska. All conditions under which the subject authorization was made remain in full force and effect and shall apply to this modification as well as all previously authorized work.
