The Francophone Population There are more than 50,000 Francophones in the Mississauga and Brampton areas. Francophones who live in the Region of Halton and Peel are from other parts of Canada, Europe, Africa and the Caribbean.

French Language Services Act The French Language Services Act entitles Francophones the right to access primary health care services in French. Until now, access to primary health care services in French has not been readily available in Halton and Peel.

Service Gap Until now, the nearest primary health care services in French for Francophone residents in Halton and Peel have been in Toronto and Hamilton.

Best Model of Care Primary health care in French is about so much more than a French speaking physician. The most comprehensive approach to care and wellness is achieved through a family health team model of care.

Family Health Team The Family Health Team model of care offers an inter-professional approach that collaboratively manages disease for optimal outcomes.

Family Health Team Members The interdisciplinary members of a physician Family Health Team are: practical nurse or nurse practitioner registered nurse dietitian social worker diabetes team comprised of a diabetes nurse educator and dietitian specialized in diabetes

Wellness is here In September 2011, The Ministry of Health and Long Term Care approved an expansion of the already established Credit Valley Family Health Team, Academic site. The site, recognized for excellence as a best practice model in the province, was selected to offer primary health care services in French.

Santé!! The Équipe de Santé Familiale/Credit Valley Family Health Team - Bilingual site opened its doors to patients in February 2012. The Family Health Team model is comprised of a multi-disciplinary team of professionals who offer comprehensive health care for Francophones in French.

Care in French There are 155 family health teams in Ontario Until now, none of them offered primary health care in French in the Halton Peel region

Family Health Team Family Health teams are noted for their expertise in the promotion of wellness and disease management through an inter-disciplinary approach to care. The Family Health Team model offers the most comprehensive approach to best manage all aspects of disease. The Family Health Team offers programs to specifically meet the needs of the community it serves.

Wellness Programs The Équipe de Santé Familiale Credit Valley / Credit Valley Family Health Team offers expertise in the promotion of wellness through programs such as: Obesity management Smoking cessation Newborn wellness programs Lung Wellness Nordic Pole Walking

Chronic Diseases The Équipe de Santé Familiale Credit Valley / Credit Valley Family Health Team also has a mandate to proactively manage chronic diseases such as : Depression Chronic Obstructive Pulmonary Disorder (COPD) Asthma Diabetes ** **Based on The Ontario Diabetes Strategy, the family health team model offers the most comprehensive approach to manage all aspects of disease.

Diabetes Team Francophones of certain ethnic groups are identified as at-risk for developing diabetes. The Équipe de santé Diabetes Team offers patients a customized approach to prevention and management of diabetes.

New Patients The Équipe de Santé Familiale Credit Valley/Credit Valley Family Health Team is accepting new patients Patients must have valid OHIP card Patients must be Francophone (French speaking) Patients must reside in the areas of Oakville, Milton, Halton Hills, Brampton, Mississauga Patients who do not already have a family physician or do have a family physician, but would prefer to transfer for services in French
