Page2 INTRODUCTION: With an objective to reduce the burden of disease and out of pocket spending for specialized care for BPL families of Surat, State Nodal Cell, Health & Family Welfare Dept. Gandhinagar, District Panchayat, surat in active coordination with Surat Municipal Corporation-Surat, organized First Mega health Camp under Mukhyamantri Amrutum Yojana on 28 th February, 2014 at New Civil Hospital campus, Old Casualty, Majuragate, Ring Road, Surat. From 10:00 A.M. To 17.00 P.M. The Mega health camp under MA Yojana aims to act as a safety net for the BPL families only by holding their hands in each step of treatment starting from village level screening by MO's to transport them to camp side by concerned FHWs/ASHA, by providing free multi-specialty consultation, all necessary diagnosis, drugs required at mega health camp, finally guide them further by referring patients to avail the needful treatment at nearest empanelled hospital and lastly follow up under MA Yojana. OBJECTIVES: The main objective behind this mega health camp was to screen out BPL patients for tertiary care which fitted in MA Yojana Cluster. For that the patients from village level were screened by concerned medical officers, and provide special consultation, instant diagnose & necessary treatment along with drugs. The Patients, who need higher tertiary care treatment, are as soon as referred to various network hospitals under MA Yojana for future treatment. If any patient are not covered under MA Yojana or not required, are referred to hospitals which are empanelled under other different health scheme like RSBY. All patients had got the consultation with free diagnosis and medicine for the treatment required by them at present based on the diagnoses and also advises them to be in contact with their respective PHC/CHC/UHC and Medical officers for future follow up for the same.

Page3 Beside above all primary objective it also focuses on to develop public awareness about MA Yojna and its benefits to common BPL people. It also focused on provide new enrolment of MA beneficiaries at camp side. ORGANISING COMMITTEE: The main organizing committee was formed who had to monitor, supervise & coordinate the activities during camp with various other sub-committee members for successful execution this camp activities. Page6 CAMP PREPARATION AND ACTIVITIES: PRIMARY SCREENING: Primary screening was started on all CHC after completion of meeting with CDHO on 24.02.2014. All THO has urgently inform to all PHC medical officers. Through the help of ASHA, FHW, and MPHW, House to house survey was done for enlisting eligible BPL beneficiaries. Prescribed sheet was given which includes name, head of family, age, sex, address, contact number, URN no of either MAA or RSBY or BPL score or BPL No. Line list was prepared accordingly by each CHC. And list submitted to Jilla Panchayat-Surat. VENUE: Venue was decided at New Civil Hospital-Surat in Old Casualty having enough rooms for the camp. New Civil Hospital, Surat, It is located on the Majuragate, Ring Road, and having enough area for parking as well as for necessary camp arrangements. Here, to every hospital has given separate rooms, chairs, tables and other necessary equipments. IEC ACTIVITIES: All the THO S were instructed to keep Orientation cum Planning meeting for the MA Mega Health Camp at each taluka place. All the grass root level health functionary s i.e. ASHA, FHW, and MPHW were trained in the meeting for the 1st screening at village level. The aim was to mobilize and disseminate information about the Mukhyamantri Amrutum Yojana to be undertaken during the Mega Health Camp & different health schemes. The mobilization was done with help of following activities. (All the activities completed within 3 working days.)

Page7 1. After meeting with CDHO, all the THO has immediately inform to asha workers and inform them for door to door calling and inform to patients for the mega health camp. 2. The PHC level MOs screening for MA health camp is the key initiative by Surat district, which reduced the extra burden on camp but increase the number of specialty consultation and referral to really needy patients. 3. Advertisement of Mega Health camp in local Radio Channel-My FM and also given news in Gujarat Samachhar on 26.02.2014. 4. All UHCs of Surat Corporation has also calling meeting of all MOs and inform for the same. 5. Banners also hang on civil hospital registration counter, so maximum beneficiaries can aware about camp.

Surgeon Cardiologist REFRESHMENT AND FOOD AREA: Separate food stalls were setup, food coupons were provided to all beneficiaries and their relatives. Early in the morning arrangement of Tea&poha was made for the beneficiaries. Gujarati Thali contained Daal, Bhat, Shakpuriwas served to all the beneficiaries.also separate room was given to VIP persons and all doctors. FACILITIES: Medical Facilities. Major specialist Doctors were available. All primary investigation likes laboratory, ECG, 2D- Echo etc. Drugs were provided to patient at free of cost. Emergency Room 2 with necessary emergency kit. Patient Counseling with counselors. Pick and Drop Facility for Patient. Waiting area. Drinking Water facility.

Page10 CAMP MANAGEMENT: On camp site, with help of New Civil Hospital-Staff, MD India, and Jilla panchayat has arranged / follow below mention lines; 1. First of all, patient registered in registration counters. (HIMS software is install here and all patients register in HIMS.) 2. After registration of patients, a patient goes for primary screening. Where Civil Hospital residents and SMIMER hospital residents diagnosed them. If any patient found those disease which are not cover under MA Yojana, then patient refer to respective RSBY empanel Hospitals. Or given medicine and count as outpatient. 3. If any patient found, who suffer from those disease, which are cover under MA Yojana, then patient refer to their respective MA Yojana Empanel Hospital.

Page11 4. MA Yojana Empanel hospital specialist can diagnosed them with help of necessary investigations and call them in their hospital for further treatment. MEDICINE FACILITIES: As per the valuable guidance to patients, by Civil Hospital / Surat Municipal Hospital doctors, we have arranged below given medicines for all patients.

Page12 Medicine Photo: TRANSPORTATION: The key reason behind selecting New Civil Hospital area as the venue of camp was because of familiarity of place situated in the center of city where the smooth transportation for the patients. From different talukas & corporation areas, movement staff members, doctors from different hospitals, for the movement of logistics, instruments for mega camp. With coordination with Jilla panchayat, THO of different taluka has arranged local vehicle like Tempos, Trucks etc.
