Give your children the care they need with the experience you trust. CCP West Millcreek is always accepting new patients. Whether you re joining us from another pediatric office, or you are just welcoming a newborn into the world, we re always happy to make your child a part of our network. New Patients Please call to schedule an initial visit so that one of our pediatricians can examine your child and review his or her medical record. Newborns Please tell the hospital staff upon delivery that you have chosen CCP West Millcreek. We will see your baby at the office as soon as necessary after discharge, usually within two to three days. Call our office before you leave the hospital to schedule an appointment.

CCP West Millcreek is dedicated to providing comprehensive medical care for infants, children, adolescents, and young adults in Erie County, PA and surrounding counties. Our staff prides themselves on forming long-lasting relationships with our patients and their families. As a part of Children s Hospital of Pittsburgh of UPMC, CCP is composed of more than 200 pediatric providers. We continue to expand our practices and technologies to meet the ever-evolving demands of health care today, and we take pride in offering exceptional pediatric care, right in our patients communities.

We have a team of pediatric providers you can trust. Our physicians are board-certified in pediatrics by the American Board of Pediatrics and are Fellows of the American Academy of Pediatrics. If your child is experiencing sick symptoms outside of typical office hours, consider utilizing our after-hours services below. Children s Nurse Triage & Advice Line If you have a concern about your child s health, we encourage you to contact our office. The on-duty nurse can provide medical advice and connect you with a pediatric provider should you need further assistance. The helpline is available 24 hours a day, 7 days a week. Children s Express Care By visiting Children s Express Care, our after-hours clinic, your child will receive the same specialized pediatric care you ve come to expect from your CCP pediatrician. Children s Community Pediatrics is one of the first pediatric networks in western Pennsylvania to have accreditation from The Joint Commission. This accreditation demonstrates our consistent efforts to provide your child with exceptional medical care. Each year, we continue to develop our processes and procedures, which allows us to protect your child s health and well-being. CCP strives to provide safe and quality pediatric care at each and every visit.

CCP is a Primary Care Medical Home accredited by The Joint Commission. By functioning as a Primary Care Medical Home (PCMH), CCP puts your child at the center of health care decision-making. Your chosen pediatrician, along with the rest of our office staff, will work in partnership with your family to coordinate and personalize care. Your provider will also work with you and your child to establish a unique care management plan. What else does a PCMH offer your family? Patient-centered care: We will form a close relationship with you and your child. We respect personal needs and values incorporating and understanding your specific language, religious and cultural preferences. Your health care team will also work with you and your child to set attainable goals that fit comfortably into your daily life. Comprehensive care: Our medical team will focus on more than your child s physical health. We will also work to meet all mental and emotional needs, including preventative and well care. Coordinated care: Should your child ever need specialty services, or have a medical emergency, CCP will coordinate care between medical professionals. Access to care: CCP pediatric providers are available in office or by phone 24 hours a day, 7 days a week. Convenient office hours and flexible scheduling, along with Walk-in Service, Children s Express Care and Children s Nurse Triage & Advice Line, give you access to pediatric care when you need it most. Quality, safe care: CCP consistently works to improve our care, asking for patient feedback and complying with national standards. As a PCMH, there is a certain level of responsibility placed on our patients. We are committed to your care, and we need to know that you are committed to us.
