DRAFT UNITED STATES VOLUNTEERS - JOINT SERVICES COMMAND DIRECTIVE SUBJECT: IDENTIFICATION CARD NUMBER 1000 10 September 2005 CG (USV-JSC) 1. Purpose. This Directive implements policy for responsibilities, and procedures for the issuance of Identification cards to members of the United States Volunteers Joint Services Command (USV-JSC). Technical specifications and issuing procedures for Identification cards are outlined in Section 5, the procedures section of this Directive. 2. Application 2.1. This instruction applies to all members of the USV-JSC. 2.2. This publication is directive in nature. These formats and procedures will be followed except when, in the judgment of the commander, exceptional circumstances dictate otherwise. 3. Policy. This is United States Volunteers Joint Services Command policy to provide members of the USV-JSC with a distinct ID card for use in identifying their status as an Active Duty, Emeritus, or Service Support member of the United States Volunteers-Joint Services Command. 4. Responsibilities. 4.1. Commander, USV-JSC. Approving authority for USV-JSC Form 1000 policy. for: 4.2. The J-1, Assistant Chief of Staff Personnel and Administration, is responsible 4.2.1. The physical design of the USV-JSC Form 1000, and acquisition of all production hardware and software. 4.2.2. Establishing overall Command responsibility and procedures for the issuance of USV-JSC Form 1000 to active members of the USV-JSC, and other eligible individuals. 4.2.3. Ensuring training is conducted and only qualified operators utilize card production equipment. 4.3. The J-4, Assistant Chief of Staff for Logistics, will establish overall policy and procedures for the hand receipt of USV-JSC Form 1000 production hardware and software to using units. 4.4. Commanders, USV-JSC Components. Commanders of the Regions and Brigades
will: 4.4.1. Ensure USV-JSC Form 1000 is issued to members IAW this Directive. 4.4.2. Ensure that when new USV-JSC Form 1000 is issued, or when members are released from service, that obsolete cards are recovered and destroyed. 4.5. Identification Card Machine Operators. Operators will; 4.5.1. Ensure USV-JSC Form is issued only to eligible members IAW this Directive. 4.5.2. Ensure that when new USV-JSC Form 1000 is issued that obsolete cards are recovered and destroyed. 5. Procedures 4.5.3. Not issue cards that modify eligible member's rank status or expiration date. 5.1. Card Description. The USV-JSC Form 1000, Membership Identification Card" is a composite 80\20 PVC\Poly card, ISO CR-80 - ISO 7810, 2.125 inches (54 mm) x 3.385 inches (86 mm), of.030 inches thickness. The card face is printed in color with a light purple tamper proof pattern, colored USV-JSC Seal, color photograph, and color USV-JSC Eagle. The face also contains the individual's name, signature, rank, ID number and expiration date. The card reverse is printed in- black and contains personal data and characteristics with a Code 39 Bar Code. Any over printing or modification to the card renders it invalid. 5.2. Card Issue. The following criteria apply to the use and issuance of the USV-JSC Form 1000 Membership Identification Card (Active/ Emeritus/Service Support Command). 5.2.1. Active Members. The USV-JSC Form 1000, Membership Identification Card is the primary ID for active members and shall be used to identity the member's eligibility for access to USV-JSC facilities. The use of other media is not authorized. 5.2.2. Emeritus members. The USV-JSC Form 1000, Identification Card may be issued to Emeritus Members at the discretion of the National Commander. 5.2.3. Service Support Command Members. The USV-JSC Form 1000, Identification Card may be issued to Service Support Command Members at the discretion of the National Commander. 5.3. Re-issuance and Destruction. 5.3.1. Lost or Stolen. USV-JSC Form 1000, Identification Card shall be replaced when lost or stolen. 5.3.2. Change in printed data. A USV-JSC Form 1000, Identification Card, shall be replaced when printed information requires changes, such as a change in rank, as supported by database information. 5.3.3. Defaced. A USV-JSC Form 1000, Identification Card, shall be replaced when any of the media (to include printed data, or bar code) becomes illegible or inoperable with card readers.
5.3.4. Retrieval and Destruction. Invalid, inaccurate, inoperative, or expired USV- JSC Form 1000, Identification Cards, shall be returned to the individual's unit for disposition. Individuals must turn-in their old card for issuance of a replacement card. The individual responsible for issuing the new card will destroy the old card by cutting it up. 5.4. Use. 5.4.1. The card, which is the property of USV-JSC, shall be in the personal custody of the member at all times. It shall be provided to governmental authorities for identification, investigation, or while the member is performing duties in governmental facilities. 5.4.2. Any person willfully altering, damaging, lending, counterfeiting, or using these cards in any unauthorized manner is subject to dismissal. Photographing or otherwise reproducing or possessing a USV-JSC ID card in an unauthorized manner is subject to dismissal. 5.4.3. Unauthorized or fraudulent use of the USV-JSC Form 1000, would exist, if a person uses the card to obtain benefits and privileges to which he or she is not otherwise entitled. 5.4.4. The USV-JSC Form 1000 shall not be posted or shown on web sites, or shown actual size. Any photocopy will always be shown with "SAMPLE" on it. The likeness shown within this document will be used to the maximum extent possible. 5.4.5. The USV-JSC Form 1000 shall not be amended, modified, or overprinted by any means. No stickers or other adhesive materials are to be placed on either side of the USV- JSC Form 1000. Holes shall not be punched into the USV-JSC Form 1000. 5.5. Access. 5.5.1. The USV-JSC Form 1000, Identification Card" is the primary ID for active duty USV-JSC members and shall be used to identify the member's eligibility for access to USV- JSC facilities. 5.5.2. The USV-JSC Form 1000, Identification Card, does not guarantee access to Federal Installations. Access is based on local policies of the federal installation and additional authorization, such as orders, may be required. 5.6. Benefits. 5.6.1. This card does NOT authorize the member any eligibility for medical benefits or commissary privileges. This card does not, by itself, authorize exchange, commissary, or morale, welfare, and recreation privileges. Additional authorization, such as federal orders, are needed for access to Federal Post Exchanges (PXs) where there are local agreements with federal military installations. 5.6.2. The USV-JSC Form 1000 will be required for meal headcounts during annual training and other subsistence supported events. 5.7. USV-JSC ID Card Production. 5.7.1. Database. The automated system will enter the appropriate information in the appropriate blocks based on the information contained in the USV-JSC database. The database provides Name, Rank, Expiration Date, Last Four Numbers of SSN, ID Card Number, Date of Birth, Weight, Height, Hair Color, Eye Color, Blood Type, Category and last 4 numbers of SSN. 5.7.2. Name. The individual s name will be entered as shown in the database. If
there has been a change, such as due to marriage, verifiable documentation will be provided to update the database prior to issuance of a card. 5.7.3. Rank. The individual's rank will be entered as shown in the database. If there has been a change due to unit promotion, verifiable documentation will be provided to update the database prior to issuance of a card. The operator IS NO AUTHORIZED to issue a card at other than the listed rank. 5.7.4. Photograph. The photograph will be a fall face photo, in color, comprising at least 80 % of the image area. The background may be white, off white, light gray, or light blue gray in color. 5.7.4.1. On site issue. That block shall contain a digitized, fall-face photograph generated from the digital camera peripheral. 5.7.4.2. Off site request. A color digital photograph 640 pixels x 480 pixels in JPG format will be forwarded with a written request for issuance of ID Card. The background must include the area behind the head to include shoulders. The individual's face must comprise at least 1/6 th of the image. 5.7.4.3. Re-Issue. The previous photograph on file will be used. 5.7.5. Signature. 5.7.5.1. On site issue. That block shall contain the signature of the cardholder generated from the Signature Pad peripheral. 5.7.5.2. Off site request. To be announced later; J-1 to develop system. 5.7.5.3. Re-Issue- The previous Signature on file will be used. 5.7.5.4. Expiration Date. The "expiration date" shall be two years from date of appointment or enlistment. Enter all dates in the four-digit year, two-digit character month, and two-digit day format (YYYYMMDD). 5.7.5.5. USV-JSC ID #. The USV-JSC ID number is entered from the database. This field will not be altered. 5.7.5.6. ID Card Number Bar Code. The USV-JSC ID Card number is generated from the database. This number uniquely identifies the card and is used to manage ID Card processing. This field will not be altered. 5.7.5.7. Date of Birth. The date of birth is entered from the database. This field will not be altered. The date will be entered in the four-digit year, two-digit character month, and two-digit day format (YYYYMMDD). 5.7.5.8. Weight. The individual's weight is entered from the database if that information has been provided. If missing this information will be entered, and a batch program will provide updated information to the USV-JSC database. The weight is in pounds and will be entered in whole numbers. 5.7.5.9. Height. The individual's height is entered from the database if that information has been provided. If missing this information will be entered, and a batch program will provide updated information to the USV-JSC database. The height is listed in feet and inches.
5.7.5.10. Hair Color, The individual's hair color is entered from the database if that information has been provided. If missing this information will be entered, and a batch program will provide updated information to the USV-JSC database. Enter basic colors only, do not abbreviate; (1) AUBURN (2) BLACK (3) BLOND (4) BROWN (5) GRAY (6) RED (7) WHITE (8)BALD (don't use unless medical condition has rendered individual hairless) 5.7.5.11. Eye Color, The individual's eye color is entered from the database if that information has been provided. If missing this information will be entered, and a batch program will provide updated information to the USV-JSC database. Enter basic colors only, do not abbreviate; (1) BLUE (2) BLACK (3) BROWN (4) GRAY (5) GREEN (6) HAZEL (7) VIOLET 5.7.5.12. Blood Type. The individual's blood type is entered from the database if that information has been provided. If missing this information will be entered, and a batch program will provide updated information to the USV-JSC database. (1) APOS (2) ANEG (3) B POS (4) BNEG (5) ABPOS (6) AB NEG (7) OPOS (8) ONEG 5.7.5.13. Category. The category field will be populated from the database as one of the following; (I) ACT Indicates an active member. (2) EMER Indicate an emeritus member. (3) SSC Indicates a member of the Support Services Command.
5.7.5.14. Date of Issue. The date of issue will automatically be entered by the computer. The date will be entered in the four-digit year, two-digit character month, and two-digit day format, separated by slashes (YYYY/MM/DD). The date time group of issue, and operator, will be included in the batch program update to the USV-JSC database. 6. EFFECTIVE DATE This Directive is effective immediately. ALLEN A.
