BACKGROUND Designated in 1965 as San Luis Obispo County's Community Action Agency. Annually, the Community Action Partnership of San Luis Obispo County, Inc. (CAPSLO) serves 30,000 persons across San Luis Obispo County and ten other central and southern California counties including Orange, Ventura, Santa Barbara, Kern, Monterey, Fresno, San Diego, San Benito, Santa Cruz and San Joaquin. CAPSLO is a private, nonprofit, Public Benefit Corporation, providing an "umbrella" of direct safety net and family development services. In a commitment to eliminate the causes of poverty, CAPSLO aims to empower individuals and families to achieve economic self-sufficiency and self-determination through a comprehensive array of community-based programs and actions. CAPSLO is a 501c3 nonprofit corporation. CAPSLO is seeking proposals from qualified vendors for the following: Installation, maintenance and service of one 200Mbps internet connection at 1 E-rate eligible site. Portions of the project will be funded through USAC s E-Rate project if approved. Requirements: One 200Mbps (minimum) internet connection at 1030 Southwood Drive, San Luis Obispo, Ca 93401. Service must be upgradable to faster speeds. Project will occur after approval of funding by USAC s E-rate project (post July 1, 2018). All bids must fall entirely within E-Rate Category 1 service. Thus, all hardware necessary (routers) for completion of this project must be provided by the vendor and included in the monthly service fee (leased / vendor maintained equipment). In the case of equipment failure, vendor will replace equipment under lease/maintenance agreement. The vendor must supply all supervision, tools, equipment, hardware, material, transportation, and construction, and all other related services unless specific provisioning by the customer has been denoted. The vendor will be responsible for, and repair all damage to building(s) incurred during install of service. Upon completion of any repair, replacement or installation activity, the vendor must provide evidence of the completion with a successful test with confirmation by CAPSLO. The vendor will be responsible for the prompt correction of all defects in the system. 2
The vendor must leave the premises clean and neat after each work session. All work must be coordinated through CAPSLO s designated contact before the beginning of the installation work. Vendor must assume total responsibility for the actions of subcontractors. Vendors are not allowed to visit sites without CAPSLO s Information Department s consent. Please include the following details on all solutions provided: install price, monthly price, technologies used, link speed, expected latency, any and all equipment needed for connectivity, service level agreements and service reliability statistics. Proposals will include: 1. Description of services to be provided, including informative sheets on services. 2. Price sheet with each site itemized with its network speed, price for network connection and price for equipment (lease / rental). 3. Any shipping and handling fees listed as a line item. 4. CAPSLO is not a tax exempt entity, all appropriate taxes will apply. All applied taxes must be listed as a line item. 5. Full total of project must be listed as a line item. 3
6. Please include any discounts provided to CAPSLO due to its 501(C3) status including any in-kind donations. Additionally, CAPSLO is party to the CALNET2 and CALNET3 contracts please provide appropriate pricing if your services apply. 7. CAPSLO is not eligible for California Teleconnect Fund (CTF). Do not include CTF pricing on bids. 8. Your vendor SPIN. Vendor Requirements: All responsive proposals shall be reviewed and evaluated by the CAPSLO in order to determine which proposal best meets the CAPSLO s needs for this project by demonstrating the competence and professional qualifications necessary for the satisfactory performance of the required services. CAPSLO encourages the submission of proposals from minority, disabled veteran and womenowned businesses. Not providing the appropriate SPIN will disqualify your bid. CAPSLO reserves the right to reject any and all proposals or waive any irregularities in a proposal or in the proposal process. CONTRACT INFORMATION CAPSLO is subject to all federal and state of California rules, regulations, laws and guidelines. Any and all responses to this RFP must follow any and all federal and state of California rules, regulations, laws and guidelines. Multi-year contract will be negotiated between CAPSLO and Vendor after award of project. Contract start date will be July 1, 2018. Due to the inherent uncertainty of the E-Rate funding process and the level of funding applied per site; CAPSLO reserves the right to remove any site from the bid at any time. Removal of site will not be considered a void of contract. This project is fully contingent on E-Rate funding. If no E-Rate funding is approved this year or any following year, CAPSLO may terminate the contract with the vendor.
