Lights Out

On The Wings Of Compassion, Excellence And Innovation Our History Founded in 1907, the Institute was established by a group of benefactors as a shelter for homeless older people in 1914 and incorporated as the Harlem House of the Daughters of Israel. Due to the changing needs, the Home gradually expanded its services to include health care. Eventually, the high prevalence of medically complex residents led to the development of a modern facility with the resources to provide more sophisticated care. This led to the development of a geriatric facility with a revolutionary vision to create an institution that would provide total care for the geriatric patient. Its orientation would be unique: rehabilitation, restoration, and return 3 to the community.

About Us For over 100 years we have been a innovator in the design of specialized programs to meet the needs of older adults. We also serve as a powerful advocate on behalf of older New Yorkers, working with a myriad of community organization members to provide needed services to older people. We expanded continuously throughout our history to meet the needs of elderly New Yorkers. Its mission is to is to provide, with compassion and dedication, superior quality health care and rehabilitation for adults. Changes in Health Care in the United States 8

Affordable Care Act Overview 9 Source: APHA, 2012 9

Changing Infrastructure Health care delivery system is dramatically changing. Health care facilities are shifting focus towards: Reducing hospitalization rates Increasing community-based care. Providers are beginning to move care delivery to the most appropriate and lowest-cost setting. More than half of hospitals and health systems are either planning to or currently repurposing space as they transition to value-based payment models and community-based population health management. 10

New Payment/Delivery Models Primary Care Medical Home - A hybrid care delivery and payment reform model that receive upfront payments and works to integrate and coordinate care by providing a monthly management fee. 11

Emerging Technology Personalized Medicine Genetic profiling for targeting drugs. Genetic diagnosis, testing, and therapy. Regenerative medicine Culturing and grafting human cells Stem-cell research. Remote Patient Monitoring Biometric sensors Organ printing Anti-aging drugs Enhanced organs Machine-augmented cognition Biologically extended senses 12

The Role of Healthcare IT Use of EMR in all clinical divisions. Enables safe, effective and efficient care delivery. Improves quality of care. Promotes care coordination and integration of services. Ensures medication cost containment and risk management. Promotes communication. 13

What is Parker Doing Now? 14

Importance of Strategic Planning Healthcare reform will significantly alter every aspect of the current health care delivery system. Therefore Strategic Planning plays a crucial role in: Accommodating the shifting environment. Establishing a firm foundation for success in a post reform environment. Preparing for financial impact of the reform, especially in areas of reimbursement and funding. Establishing clear direction and priorities in an environment of uncertainty. 15

Essential Components of Strategic Planning Risk Mitigation strategies Quality focused initiatives/programs Data driven analytics Analysis of reform requirements and regulations 16

Workforce Excellence An integral part of quality improvements at Parker is enhancing the relationship between our residents, direct care workers and the professional staff, a critical requirement of meeting the changing needs of older adults. Changing the organizational culture of nursing homes cannot be successfully achieved without the participation of the day-to-day direct care staff in the care planning process. 17

Change in Culture In 2000, the Parker Jewish Institute embarked on a culture change journey that envisions long term care environments in which older adults not only survive, but thrive, and in which staff not only perform daily duties but proactively contribute to improved methods of care. 18

Bundled Payments Bundled payment is a single payment to providers or health care facilities (or jointly to both) for all services to treat a given condition or provide a given treatment. Bundled payment asks providers to assume financial risk for the cost of services for a particular treatment or condition, as well as costs associated with preventable complications. Payments are made to the provider on the basis of expected costs for clinically defined episodes that may involve several practitioner types, settings of care, and services or procedures over time. 25

Did You Know? The U.S. Department of Health and Human Services (HHS) has established a target of tying 30% of Medicare payments to quality or value through alternative payment models such as bundled payment arrangements and Accountable Care Organizations--by the end of 2016, and 50% of payments by the end of 2018. 26

Did You Know? Post-acute care efficiency is related to three things: re-hospitalizations, skilled nursing facility length of stay, and use of high-cost PAC settings (for example, SNFs instead of home health). 27

Strategic Benefits Protect important service lines from compettion Increase in-network care through better coordination Seamless care is more applealing to providers Bundling is more appealing to ACOs and networks Market Share Positive Patient Reaction Seamless care Increased shared decision-making Improved Alignment with Performance Readiness for New Payment Models Shared accountability Rewards for improving care delivery Manage cost Improve quality 28

Care Choices A new option for Medicare beneficiaries to receive palliative care services from selected hospice providers while concurrently receiving services provided by their curative care providers. The model empowers clinicians, beneficiaries and their families with greater flexibility in deciding between hospice care and curative treatment when faced with life limiting illness. CMS will evaluate whether providing hospice services can improve the quality of life and care received, increase patient satisfaction, and reduce Medicare expenditures. 29
