Medical Simulation Orientation Familiarization with IMSE s METI ECS Simulation Manikin Getting to know the Simulator
Aims and Goals of Orientation To allow participants to familiarize themselves with the mannequin and equipment prior to actual simulation training session To understand some of the realism and limitations of the simulation to optimize the simulation experience
It is REAL until proven otherwise. Reminder
Reminders All responses and the lack of responses during the simulation training should be taken as the real clinical response and verbal feedback will be provided only when appropriate via the mannequin Participants will not be prompted unless deem necessary by the controller
Reminders Interventions: Perform any and all airway intervention (e.g. intubation, LMA insertion) as you would in the real world. Administration of drugs participants are required to state the drug, the route and the dose prior to administration the drugs via the intravenous cannula.
Entering into the simulator The first step into simulation
Meet your patient: IMSE s METI ECS
Getting to know the mannequin Close up of your patient, a normal Joe..
Face to face meeting
Here s looking at you..
Blink blink
Having an eye for you..
Having eyes for you Pin-point pupil Dilated pupil
Doctor, how are you The mannequin will talk whenever appropriate (via speakers)
Doc, there is something I need to get off my chest..
Breathing in and out Chest wall rises equally and bilaterally as it would in a normal patient
I can hear you breathe Red dots = breath sounds
and I can hear your heart beat Green dots = heart sounds
and I can hear your stomach growl Yellow dots = bowel sounds
Shocking the patient Place the paddles at these 2 locations Apical placement Sternal placement
There is a pulse! Brachial pulse Dorsalis pedis pulse The carotid arteries are also palpable and so are the femoral pulses
Getting to know the monitoring device Get me the vital signs stat!
Monitor display The monitor can display the following parameters: ECG (lead II) Heart rate Non-invasive BP Invasive arterial BP SpO2 Not all parameters will be displayed or available, depending on the clinical scenario but made available on request.
Getting to know the resuscitation trolley Crash in the Crash cart
The Resuscitation Trolley
On the top CODEMASTER DEFIBRILLATOR Fully functioning defibrillator Capabilities: Mono-phasic Power: 0-360 joules Defibrillation Synchronised cardioversion Pacing
On the top: Resuscitation drug kit Drugs available What s inside Drugs are labeled
The Resuscitation Trolley Equipment available: Defibrillator Resuscitation drugs Bag mask Airway equipment Oxygen delivery devices Syringes, needles, blood tubes Fluids
Inside the Resuscitation Trolley Drawer 1 : Airway Apparatus Drawer 2 : Syringes, Needles, CVP, Blood Tubes
Inside the Resuscitation Trolley Drawer 3 : Mask, LMA, Suction Catheter Drawer 4 : NG Tube, Foley s catheter, Urine Bag, Chest Tube
Inside the Resuscitation Trolley Drawer 5: Fluids, IV Tubing Others The following investigation may be provided upon request and may require time to be available as in the real world: ECG, ABG, radiology Blood products
Getting to know the telephone I need to call someone!
If the need arise to call someone, a telephone is available. Just mention who you would like to call prior to making the call. The Telephone
The Telephone To make a call, do the following: Declare clearly who you would want to call. Pick up the phone. Press the INT button on the multi-directional center button. Then press the number as stated on the phone (e.g. O ) Talk through the handset.
Hands-on orientation session to be done in the simulation centre Time for the real thing
Breath sounds: Things to look out for: Normal Rhonchi/Wheeze Crepitations Heart sounds Pulses Brachial Femoral Dorsalis Pedis
Before beginning simulation training Final Words
Things do to before starting: Please sign confidentiality form (compulsory) Do remember to treat all responses or lack of responses as real Please treat the mannequin with respect as if it s a real patient.
It is REAL until proven otherwise. Reminder
