Managing the health of our members. NEED TO LICENSE Transition of Care coverage is provided to new members in active treatment for medical conditions with non-presbyterian network providers. These treatments can include surgeries, dialysis, second or third trimester pregnancy, or other complex medical conditions. Direct no-cost access to medical advice is available through PresRN, a nurse advice line available to you 24 hours a day, 7 days a week, including holidays. Case management for acute conditions is provided for conditions such as transplants, high-risk pregnancy, and transition of care. Disease management for chronic conditions helps our members with certain diseases manage their health. These diseases can include diabetes, coronary artery disease, asthma, and dialysis.
