Medical Staff Bylaws New Category Proposal ARTICLE 4. CATEGORIES OF THE MEDICAL STAFF 4.1 CATEGORIES The Staff shall be divided into Active, Ambulatory Proceduralists, Affiliate and Honorary Categories. 4.2 ACTIVE STAFF 4.2-1 Qualifications The Active membership Category is designated for Members who meet the basic qualifications set forth in Section 3.2-1 who: a. hold a current valid license to practice unrestricted in the State of Nebraska; b. demonstrate an interest and commitment to the Medical Staff and pediatric care; c. admit or consults on pediatric patients at Children s Hospital & Medical Center or d. actively participate on a Medical Staff committee; or e. serve in a peer review or proctoring capacity; or f. regularly attend Children s Continuing Medical Education activities; or g. provide a service to the Hospital which is not available from other sources; or h. demonstrate an ongoing clinical competence in area of specialty. 4.2-2 Prerogatives The Prerogatives of an Active Member shall be: a. To exercise such Clinical Privileges as granted by the Credentials Committee and pursuant to Article 7; b. To vote on all matters presented at general and special meetings of the Medical Staff and committees of which the Member belongs; c. To hold office in the Staff organization or committees to which the Member belongs. d. To perform approved privileges at the hospital or surgery center. 4.2-3 Responsibilities
Each Active Member shall: a. Satisfy the basic responsibilities set forth in Section 3.2-1. b. Actively participate in the performance improvement activities required of the Staff and discharge such other Staff functions as may from time to time be required. c. Satisfy the requirements set forth in Medical Staff Documents for attendance at meetings of the Staff and committees of which the Member belongs. d. Agree to serve on the Emergency Department Call Schedule on a rotational basis either for general or subspecialty call as indicated by their level of privileging. This call service includes covering for inpatient consults. 4.3 AMBULATORY PROCEDURALISTS 4.3-1 Definition Ambulatory Proceduralists shall consist of members who desire to practice in the Surgery Center, but who do not wish to, or do not qualify for the exercise of clinical privileges on an inpatient basis at the hospital. Ambulatory Proceduralists members may provide procedures, care and treatment on an outpatient basis for patients. Members of the Ambulatory Proceduralists may not admit inpatients but may initiate an admission by referring a patient to a physician with admitting privileges. Ambulatory Proceduralists members may visit their hospitalized patients and review their patient s medical records, but may not write orders for inpatients or exercise any inpatient clinical privileges. 4.3-2 Qualifications The Ambulatory Proceduralists membership Category is designated for Members who meet the basic qualifications set forth in Section 3.2-1 who: a. hold a current valid license to practice unrestricted in the State of Nebraska; b. demonstrate an interest and commitment to the Medical Staff and pediatric care; c. provides care and consults on pediatric patients at Children s Hospital & Medical Center or d. serve in a peer review or proctoring capacity; or e. regularly attend Children s Continuing Medical Education activities; or f. provide a service to the Hospital which is not available from other sources; or g. demonstrate an ongoing clinical competence in area of specialty.
4.2-2 Prerogatives The Prerogatives of an Active Member shall be: a. To exercise such Clinical Privileges as granted by the Credentials Committee and pursuant to Article 7; b. To vote on all matters presented at general and special meetings of the Medical Staff and committees of which the Member belongs; c. To hold office in the Staff organization or committees to which the Member belongs. 4.2-3 Responsibilities Each Ambulatory Proceduralists Member shall: a. Satisfy the basic responsibilities set forth in Section 3.2-1. b. Actively participate in the performance improvement activities required of the Staff and discharge such other Staff functions as may from time to time be required. c. Satisfy the requirements set forth in Medical Staff Documents for attendance at meetings of the Staff and committees of which the Member belongs. d. Initiates an admission by referring a patient to a physician with admitting privileges. e. Pay dues equal to those of the active staff members. 4.34.4 AFFILIATE STAFF 4.34.4-1 Definition The Affiliate membership Category of the Medical Staff is designated for those Members who indicate that they would like to be apprised of Children s Hospital & Medical Center activities and attend Continuing Medical Education (CME) events, but do not have an ongoing interest in Active membership status. 4.34.4-2 Qualifications An Affiliate Member shall meet the basic qualifications set forth below. By vote of the Medical Executive Committee, sub-sections d and f may be waived.
a. Document their background, training, experience, physical and mental health status, and current competence in the care of pediatric patients. b. Are determined, on the basis of documented references, strict adherence to the ethics of their respective professions, and to be willing to participate in the discharge of Staff responsibilities. c. Provide documentation of their ability to work with others. d. Maintain professional liability insurance in accordance with Medical Staff Bylaws. e. Have successfully completed a residency program approved by the Accreditation Council for Graduate Medical Education (for Applicants applying after July 1, 1992). f. g. Have signed an Acknowledgment of the Member s obligation to comply with the Children s Hospital & Medical Center Corporate Compliance Plan. 4.34.4-3 Prerogatives An Affiliate Member: a. shall have the Prerogative to attend Medical Staff or Hospital education programs; b. shall not be eligible for Clinical Privileges; c. shall not be eligible for committee membership except for Medical Education Committee; d. shall not be eligible to hold office or to vote. 4.34.4-4 Responsibilities Each Affiliate Member shall be required to: a. Abide by the Medical Staff Documents, and all other lawful standards, policies, and rules of the Hospital including the Hospital s Performance Improvement Plan and Corporate Compliance Plan. b. Abide by the ethical principles of the profession; c. Pay, in a timely fashion, annual Medical Staff dues as determined by Medical Staff Documents. 4.3-5 4.4-5 Reappointment
4.44.5 HONORARY STAFF a. Reappointment to the Affiliate Staff will be conducted by Children s Hospital & Medical Center Medical Staff Services as outlined in the Credentialing Policy & Procedure manual and will not be forwarded to the Nebraska Credentials Verification Organization for processing. b. Reappointment shall consist of querying the National Practitioner Data Bank (NPDB), an evaluation by a peer reference, verification of licensure, board certification and professional liability insurance in accordance with the Medical Staff Documents. c. The above documents will be reviewed by the appropriate Department Chair, Credentials Committee, Medical Executive Committee and Quality & Patient Safety Committee like any other application for reappointment as defined in the Medical Staff Documents. 4.44.5-1 Definition A Member of the Medical Staff who is no longer actively practicing medicine and who has been voted to Honorary status by the Medical Executive Committee in recognition of their contributions. 4.45-2 Qualifications The Honorary Member shall be a Practitioner recognized for outstanding reputation, noteworthy contributions to the health and medical sciences, or previous long-standing service to the Hospital. They do not need to be residents of the metropolitan Omaha area. 4.45-3 Prerogatives Honorary Staff Members are not eligible to admit patients to the Hospital or to exercise Clinical Privileges in the Hospital. The Honorary Staff Members are not required to go through the reappointment process or maintain professional liability insurance as outlined in Article 14.2. The Prerogatives of an Honorary Staff Member shall be to attend Staff meetings, and Medical Staff or Hospital education programs. Honorary Members shall not be eligible to vote, hold office, or required to pay dues. 4.56 LIMITATION OF PREROGATIVES The Prerogatives set forth under each Staff Category are general in nature and may be subject to limitation by special conditions attached to a Member's Staff membership, by provisions of the Medical Staff Documents, or by other policies of the Hospital.
