I. PGY1 Program Purpose PGY1 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy resident training. II. Introduction The Bayhealth Postgraduate Year (PGY1) One Residency Program is a one-year training program primarily in an inpatient healthcare practice setting. The PGY1 resident will receive extensive education and training in treating hospitalized patients through guided practice from experienced preceptors. The program is tailored to meet the needs and interests of individual residents. During the program, residents will participate in all aspects of our comprehensive pharmacy service. Rotations in acute care practice areas, ambulatory care, pharmacy management, research, and pharmacy systems are offered. As a vital member of the healthcare team, residents will have the opportunity to enhance practice skills that are necessary to evaluate a patient s medication regimen. Throughout the one year program, residents will have numerous opportunities to provide education to patients and other health care professionals. In addition, residents will gain experience in providing written responses to drug information questions. The knowledge gained over the course of the program will enable residents to competently conduct a research project. In addition, residents will be expected to conduct a medication utilization evaluation (MUE) and/or complete a project that will enhance medication safety. Opportunities for teaching pharmacy students and earning a teaching certificate are available. Residents will acquire foundation skills needed to become competent and confident clinical practitioners and future leaders in pharmacy practice. Also, residents will gain experience in collaborative relationships with health care team members through proactive recommendations during patient care rounds and while working as a decentralized pharmacy team member. Bayhealth pharmacy started its PGY1 residency program in 2003. The program received ASHP accreditation in March 2005 and was re-accredited most recently in 2013. III. General Program Outcomes At the completion of the Bayhealth PGY1 Residency program the resident will be able a. To provide quality patient care through collaborative working relationships with other healthcare providers b. To design, recommend, and monitor patient-specific treatment regimens. c. To provide concise, comprehensive, applicable, and timely response to drug information requests. d. To provide effective education to healthcare professional and patients. e. To demonstrate professionalism, leadership skills, responsibility, and ethical conduct. f. To identify medication safety issues and to improve medication systems to maximize patient safety. 3

IV. PGY1 Residency Program Purpose Statement The purpose of the Bayhealth postgraduate year one residency program is to prepare pharmacists for clinical acute care positions, adjunct faculty positions, or for postgraduate year two training in a focused practice area. V. Elective rotations are only to be scheduled the last several months of the residency year. 6

Clinical research project for presentation at the Eastern States Residency Conference. The project should be of such quality that the written results may be submitted for publication in a peer-reviewed journal. 2. Research poster presentation at the ASHP Mid-Year meeting 3. Required Meetings 1. P&T Committee 2. Clinical Pharmacy Meeting 3. Interdisciplinary Rounds IX. Staffing Responsibilities and Benefits 1. Staffing Responsibilities a. The Pharmacy Resident shall perform and demonstrate proficiency as a Staff Pharmacist during the course of the Program. a. The normal assignment will be eight hours of evening shift per two-week pay period or four hours of evening shift weekly. The evening shift assignment may be scheduled at either Bayhealth campus depending on staffing needs. b. The normal weekend assignment will be every third weekend. This rotation will include one Saturday and one Sunday for the shift assigned. The weekend assignment may be scheduled at either Bayhealth campus depending on staffing needs. 7

c. The resident will be required to work one (1) Summer Holiday and one (1) Winter Holiday. The holiday assignment may be scheduled at either Bayhealth campus depending on staffing needs. d. This schedule is subject to change by the Residency Program Director. 2. Benefits a. Stipend. b. 15 days of paid time off (PTO) which includes sick leave, vacation leave, paid holidays, and bereavement leave. Please note that Bayhealth has six paid holidays and any holiday not worked will be subtracted from the 15 days of paid time off. o Paid Time Off (PTO) PTO accrues based on the number of hours you work each pay period. You use your PTO for vacations, holidays or sick days. PTO must be earned before it can be used. Borrowing or taking advances against future PTO accruals is not permitted. Scheduled time off must be approved by your manager or supervisor at least two business days prior to the requested date(s). o Extended Sick Time (EXS) EXS allows you to access sick time for your own illness and/or injury which extends beyond two consecutive days. You must use PTO for the first two consecutive days of any illness/injury, regardless of the reason, including hospitalization. You may begin using your EXS on the third consecutive day. If you do not have PTO available, the first two days (up to 16 hour) will be unpaid. o Family Medical Leave Act Allows employees to take up to 12 weeks of family leave during a rolling 12- month period to eligible employees under the provisions of the FMLA Act. This leave shall be first paid leave, accessing extended sick leave when eligible and /or utilizing PTO with the balance being unpaid if a paid leave does not cover the entire period of absence. To qualify for leave under FMLA, employees must have worked for Bayhealth for at least 12 months (52 weeks) and must have worked at least 1250 hours during the 12 month period immediately before the date of the leave request. c. Pre-employment Requirements i. State and national background check, physical assessment, urine drug screen d. Financial support to selected professional meetings e. Payment of professional liability insurance. 4. Duty Hours a. Duty hours must be limited to 80 hours per week, averaged over a four-week period, inclusive of all in-house call activities and all moonlighting. (Appendix A) IX. Resident Responsibilities a. Development of personal goals for the residency following an initial evaluation of career interests, prior experience, and areas of strength and weakness. b. To meet the residency requirements, it is expected that the PGY1 residency will have to work in excess of 40 hours per week. Thus, moonlighting outside of Bayhealth Medical 8

Center is strongly discouraged. Any moonlighting must be approved by the Director of the Residency Program. c. Compliance with rotation expectations: a) Meeting with the rotation preceptor to discuss individual goals and objectives for the rotation. b) Completing assignments by the end of the rotation. c) Informing the Residency Director of difficulties encountered in meeting goals and objectives or problems with preceptor. d) Assuming the responsibilities of the rotation preceptor in his/her absence. e) Completing an evaluation of each rotation experience, preceptor, and a selfevaluation at the conclusion of the rotation that includes accomplishment of objectives and experiences gained with recommendations for improvement. d. Completion of quarterly evaluations to be reviewed by the Residency Director. The purpose is to assure that the established residency goals are being achieved. e. Provision of pharmacy staffing coverage. f. Maintenance of active membership in the American Society of Health-System Pharmacists. g. Completion of a major residency project and medication use evaluation. h. Completion of required presentations throughout the residency. i. Maintain and complete a resident-portfolio of accomplishments. j. Submission of articles to pharmacy newsletters. k. Attendance at the ASHP Midyear Clinical Meeting and Eastern States Conference for Pharmacy Residents and Preceptors. Attendance at applicable local pharmaceutical education programs is expected. Residents may attend other professional meetings if the staffing schedule permits. l. Reading and following the Attendance and Dress Code Policies. m. Following patient privacy laws and regulations. n. Document hours worked on the assigned excel spreadsheet. X. Assessment of Learning Experiences a. For each week of the rotation, progress will be verbally discussed with the PGY1 resident. b. The PGY1 resident is recommended to maintain a feedback diary throughout the residency year. Two entries per week are recommended. Feedback diary entries should be forwarded, via e-mail, to the preceptor and the Residency Director on a Bi-weekly basis if maintained. c. There will be a mid-point discussion between the preceptor and resident will be reviewed with the discussed verbally and in writing. If the mid-point discussion indicates that the PGY1 resident is not meeting rotation goals ( needs improvement for goals), a written action plan will be discussed with the PGY1 resident for those goals. Failure by the PGY1 resident to address the items on the action plan may result in failure of those goals ( needs improvement ) for that rotation. d. There will be a summary evaluation by the preceptor which will be reviewed with the PGY1 resident by the last day of the rotation. e. If the PGY1 resident fails the majority of goals ( needs improvement ) for a rotation, the Academic and Organizational Provisions Resident Corrective Action and Discipline Policy (Exhibit A of the Residency AGREEMENT) will be followed. f. The PGY1 resident will complete an evaluation of the preceptor and rotation and submit it to the Director of the Residency Program by the last day of the rotation, includes longitudinal rotations. 9

o Preceptors are required to complete timely, formal assessments of the resident s performance during the learning experience. Discussions of the evaluations are to be scheduled by the preceptor. o For 3-4 week learning experiences, PharmAcademic preceptor evaluations must be completed no later than 3 business days after the learning experience conclusion; completing these evaluations on the last day of the learning experience is preferred. Evaluations are formal and are to be reviewed with the preceptor verbally, as well as, in writing. No written comments on evaluations should be a surprise to the preceptor. o When completing preceptor evaluations residents should provide adequate written comments that are specific, actionable, and criteria based to help preceptor s improve their performance. Strategies for preceptors to improve should be included and emphasized to address identified areas for improvement. g. The Director of the Residency Program will conduct quarterly customized evaluations with the PGY1 resident. h. Preceptors for longitudinal rotations will conduct quarterly evaluations with the PGY1 resident, as well as, a summary evaluation. These evaluations will be reviewed with the PGY1 resident. XI. Requirements for Program Completion Completion of the PGY1 residency program is attained by: a. Achieving 85% or greater of the Pharmacy Systems I, II and III goals. b. Achieving 85% or greater of all other rotation goals. c. Submission of the final paper for the residency project to the Director of the Residency Program. d. Completion of 85% of the required projects listed in section VI of this manual. The official Bayhealth Medical Center Residency Certificate will be withheld until all requirements are fulfilled. XII. Appendix A Duty-Hour Requirements for Pharmacy Residencies Definitions: Duty Hours: Duty hours are defined as all scheduled clinical and academic activities related to the pharmacy residency program. This includes inpatient and outpatient care; in-house call; administrative duties; and scheduled and assigned activities, such as conferences, committee meetings, and health fairs that are required to meet the goals and objectives of the residency program. Duty hours must be addressed by a well-documented, structured process. Duty hours do not include: reading, studying, and academic preparation time for presentations and journal clubs; travel time to and from conferences; and hours that are not scheduled by the residency program director or a preceptor. Scheduled duty periods: Assigned duties, regardless of setting, that are required to meet the educational goals and objectives of the residency program. These duty periods are usually assigned by the residency program director or preceptor and may encompass hours which may be within the normal work day, beyond the normal work day, or a combination of both. Moonlighting: Voluntary, compensated, pharmacy-related work performed outside the organization (external), or within the organization where the resident is in training (internal), or at any of its related participating sites. These are compensated hours beyond the resident s salary and are not part of the scheduled duty periods of the residency program. Continuous Duty: Assigned duty periods without breaks for strategic napping or resting to reduce fatigue or sleep deprivation. Strategic napping: Short sleep periods, taken as a component of fatigue management, which can mitigate the adverse effects of sleep loss. DUTY-HOUR REQUIREMENTS Residents, program directors, and preceptors have the professional responsibility to ensure they are fit to provide services that promote patient safety. The residency program director (RPD) must ensure that there is not excessive reliance on residents to fulfill service obligations that do not contribute to the educational value of the residency program or that may compromise their fitness for duty and endanger patient safety. Providing residents with a sound training program must be planned, scheduled and balanced with concerns for patients safety and residents well-being. Therefore, programs must comply with the following duty-hour requirements: ASHP Duty-Hour Requirements 1 11

I. Personal and Professional Responsibility for Patient Safety A. Residency program directors must educate residents and preceptors about their professional responsibilities to be appropriately rested and fit for duty to provide services required by patients. B. Residency program directors must educate residents and preceptors to recognize signs of fatigue and sleep deprivation, and adopt processes to manage negative effects of fatigue and sleep deprivation to ensure safe patient care and successful learning. C. Residents and preceptors must accept personal and professional responsibility for patient care that supersedes self-interest. At times, it may be in the best interest of patients to transition care to another qualified, rested provider. D. If the program implements any type of on-call program, there must be a written description that includes: the level of supervision a resident will be provided based on the level of training and competency of the resident and the learning experiences expected during the on-call period; and, identification of a backup system if the resident needs assistance to complete the responsibilities required of the on-call program. E. The residency program director must ensure that residents participate in structured handoff processes when they complete their duty hours to facilitate information exchange to maintain continuity-of-care and patient safety. II. Maximum Hours of Work per Week and Duty-Free Times A. Duty hours must be limited to 80 hours per week, averaged over a four-week period, inclusive of all inhouse call activities and all moonlighting. B. Moonlighting (internal or external) must not interfere with the ability of the resident to achieve the educational goals and objectives of the residency program. 1. All moonlighting hours must be counted towards the 80-hour maximum weekly hour limit. 2. Programs that allow moonlighting must have a documented structured process to monitor moonlighting that includes at a minimum: a. The type and number of moonlighting hours allowed by the program. b. A reporting mechanism for residents to inform the residency program directors of their moonlighting hours. c. A mechanism for evaluating residents overall performance or residents judgment while on scheduled duty periods and affect their ability to achieve the educational goals and objectives of their residency program and provide safe patient care. d. A plan for what to do if residents participation in moonlighting affects their judgment while on scheduled duty hours. C. Mandatory time free of duty: residents must have a minimum of one day in seven days free of duty (when averaged over four weeks). At-home call cannot be assigned on these free days. D. Residents should have 10 hours free of duty between scheduled duty, and must have at a minimum 8 hours between scheduled duty periods. ASHP Duty-Hour Requirements 2 12

E. If a program has a 24-hour in-house call program, residents must have at least 14 hours free of duty after the 24 hours of in-house duty. III. Maximum Duty-Period Length A. Continuous duty periods of residents should not exceed 16 hours. The maximum allowable duty assignment must not exceed 24 hours even with built in strategic napping or other strategies to reduce fatigue and sleep deprivation, with an additional period of up to two hours permitted for transitions of care or educational activities. B. In-House Call Programs 1. Residents must not be scheduled for in-house call more frequently than every third night (when averaged over a four-week period). 2. Programs that have in-house call programs with continuous duty hours beyond 16 hours and up to 24 hours must have a well-documented structured process to oversee these programs to ensure patients safety and residents well-being, and to provide a supportive, educational environment. The well-documented, structured process must include at a minimum: a. How the program will support strategic napping or other strategies for fatigue and sleep deprivation management for continuous duty beyond 16 hours. b. A plan for monitoring and resolving issues that may arise with residents performance due to sleep deprivation or fatigue to ensure patient care and learning are not affected negatively. C. At -Home or other Call Programs 1. At-home call must not be so frequent or taxing as to preclude rest or reasonable personal time for each resident. 2. Program directors must have a method for evaluating the impact on residents of the athome or other call program to ensure there is not a negative effect on patient care or residents learning due to sleep deprivation or serious fatigue. 3. Program directors must define the level of supervision provided to residents during athome or other call. 4. At-home or other call hours are not included in the 80 hours a week duty-hour calculation, unless the resident is called into the hospital/organization. 5. If a resident is called into the hospital/organization from at-home or other call program, the time spent in the hospital/organization by the resident must count towards the 80-hour maximum weekly hour limit. 6. The frequency of at-home call must satisfy the requirement for one-day-in-seven free of duty, when averaged over four weeks. No at-home call can occur on the day free of duty.
