Experience and or Extensive Knowledge in providing Occupational Therapy Services in a Rehabilitation or Aged Care setting. Experience across inpatient and outpatient/ community settings would be desirable. 4. A current drivers licence and willingness to travel between HammondCare s sites and services. 5. The organisation provides these services through sub-acute hospitals, residential care and innovative community services in NSW and has a particular commitment to dementia care and research as well as to older people who are financially disadvantaged. Summary of Position The position works within the philosophy and core values of our company to ensure that the service is tailored to the needs of the individual clients and delivered in a style that aims to maximise client satisfaction. The Level 1/ 2 Occupational Therapist delivers a high standard of Occupational Therapy service to patients of the Hospital, along with appropriate support and advice to their carers. The Occupational Therapist will participate as a member of the multi-disciplinary team in patient care, and in the application of the Hospital s philosophy and values to the pursuit of its objectives in a patient centred, continuous quality improvement work environment. Key Responsibilities Responsibilities Capability Outcomes 1.0 Patient/Client/Resident Care Assess and treat patients with a high standard of professional practice, maintaining the ethics and standards of HammondCare and OT Australia Australian Association of Occupational Therapists. Contribute to patient care as part of a multidisciplinary team and maintain good communication with medical, nursing, allied health and other staff, and voluntary workers. Provide comprehensive assessment and treatment programs designed to address deficits in the patient s occupational roles and performance. Design programs to alter or compensate for underlying occupational performance component problems, and/or environmental barriers to performance utilising goal-directed activities and tasks. Re-assess patient s response to therapy on a regular basis and modify therapy accordingly. Referrals are addressed in a timely manner in accordance with protocols for designated caseload. Builds trusting relationships with patients and their families/carers to support effective care provision. Has effective working relationships with the MDT and the MDT value the service provided. Actively participates in multidisciplinary team meetings for designated caseload. Completes documentation of comprehensive

Conduct home visits as required to assess environment and modifications required. Liaise with carers and co-workers concerning Occupational Therapy assessment and intervention, and educate carers and coworkers in the skills they need to assist patients. Respect and maintain patient confidentiality and privacy Involve patients in their own management encouraging decision-making, goal setting and awareness of their condition through education. Report on patient condition and progress through direct communication with team members and through comprehensive documentation in the Medical Record. Liaise with Recreation Activities Officers regarding patient care and ensure comprehensive intervention. Ensure continuity of care by obtaining relevant information and treatment directions from previous carers, and by passing such information to subsequent carers by discharge summary and other means. Facilitate communication and re-integration at discharge by referral to relevant community agencies. assessments, program and interventions in patient medical files in accordance with HammondCare and department policies. Outcomes of therapy will be evaluated and documented with relevant handover given to other services as necessary. Medical records will indicate that patients referred to occupational therapy receive a comprehensive service that addressed the patients occupational needs and contributes to specialist multidisciplinary care. As part of the team assists with cover as required e.g. during periods of leave, to ensure patient across all caseloads have access to services. 2.0 Operational Management Activities Participate in development and promotion of Occupational Therapy services. Actively participate in the implementation of an effective and appropriate quality service. Participate in regular evaluation of practice. Exhibit a commitment to quality service and participate in activities to enhance continuous quality improvement. Attend and contribute to team, case, department and other meetings as needed. Read and understand the Policy and Procedures Manual(s) for the Department and Behaviour and performance are consistent with professional expectations, HammondCare s MIA and Capability Framework. Attends and participates in relevant meetings. Undertakes department support and development activities as directed by supervisor or Manager and completes in agreed time frames.

Maintain and update resource and equipment files. Assist in development of further department resources and therapy aids. Prepares and leads education sessions as agreed with supervisor. Contributes to and participates in minimum of one Quality Improvement activity annually. Monthly statistics will be completed in specified time frame and will accurately reflect workload. 3.0 Professional development/education Maintain and update knowledge by regular reference to journals and texts. Identify skills and knowledge required to fulfil the role and to attend appropriate staff development determined during ADM (and linked with the Occupational Therapy Department and/or service business plan) and/or supervision sessions. Actively participate in continuing professional development in line with AHPRA registration requirements. Clinically supervise Occupational Therapy students as requested. Contribute to the education of Occupational therapy staff, students, other health workers, volunteers and carers. Provide in-services to other departments and organisations as requested. Participate in mandatory training as required. Participates in department professional development program and activities. Participates in clinical supervision with designated Senior Occupational Therapist or Occupational Therapy Manager according to agreed schedule. Attends relevant education to develop skills and knowledge, as agreed in ADM, and uses what you learn.. Prepares and leads education sessions as agreed with supervisor.
