Job description Post: Support & Well Being Worker Grade: Indicative Grade Scale 5 Responsible to: Support & Wellbeing Hub Coordinator Basic objectives of the post As part of the Support and Well-Being Service, the post holder will deliver a need led service both in sheltered schemes and within community. Support and Well Being Worker will provide coordinated and structured support and intervention to help residents sustain their tenancies. The role will be important in supporting residents to achieve greater independence, positive mental and physical wellbeing and social and financial inclusion; promoting independence and quality of life. It is not part of the role to provide personal care to tenants. This post is exempt from the Rehabilitation of Offenders Act 1974 (as amended in 1986) Duties and responsibilities 1. To be empathic to the needs of older people and develop warm and trusting relationship with residents to encourage them to express their needs, views and concerns. 2. To be able to clearly explain the remit of the service to a range of clients and other professionals. 3. Conduct morning welfare checks to residents living in the schemes and for residents in the community. 4. To be a named support worker for clients living within the supported housing accommodation scheme. 5. and procedures and the requirements of stakeholders and commissioners. 6. To maintain good links with local statutory and voluntary agencies to enable clients to access appropriate services and support. 7. To accurately input all client data onto our electronic monitoring system and ensure records are maintained to evidence key performance targets, and comply with commissioner requirements. 8. To support clients to make informed choices about decisions that affect them and their family, and to take control of their support. 9. To provide time limited, structured and flexible support to clients, in line with the service specification, policies and procedures, to enable individuals to maintain their independence and do as much as they can for themselves. 10. To offer a range of personalised and outcome focused support to older people in their own homes. 11. To support with managing finances, support with their daily home management and information about managing their health needs and keep safe. 12. To help with providing advice and information on how to access a range of subjects, including housing advice, help around the home, education, volunteering, and community transport initiatives 13. To help in reducing social isolation by getting involved in local community, accessing support for carers and contact with family/friends. 14. To support residents with filing in forms and deal with other correspondence. 15. Undertake any other duties reasonably required that are consistent with the grade and basic objectives of the post. 3. To carry out any other duties, within an appropriate level of responsibilities as required. 4. To take responsibility for the health and safety of themselves and all other persons affected by their actions or omissions during their work. 5. To work for long periods without direct supervision within a community setting, adhering to risk assessments and following policy guidelines on lone working and Homes for Haringey procedures in accordance with legislation and national minimum standards. 6. To conduct testing of the pull cord system, smoke detectors and carry out health and safety checks in the schemes 7. To be part of a team responsible for proactive monitoring of the safety and security of service users and direct provision of information for Safeguarding of Vulnerable Adult investigations, including attending SOVA strategy meetings as required. 8. To respond to emergency situations promptly and where necessary provide practical assistance but not including personal care. To undertake other non permanent duties consistent with the basic objectives and/or duties of the post and at all times to carry out duties in accordance with the Homes for Haringey s policies and procedures. Good organisational skills, including time management and prioritising workload. Ability to collaborate and work as part of a team.
