FUNDING SOURCE o ODOT is the pass-through entity for funds o Medicaid Money Follows the Person balancing funds o Funds Ohio gets due to cost savings for transitioning people from institutions to community settings o Needs to be used to increase community integration 9 Local Coordinated Planning Grants Overview

FUNDING TIMELINE o May 15, 2017 Application Released o https://odot.formstack.com/forms/local_coordinated_planning_grant o May 24, 2017 Webinar o July 1, 2017 Application Due o July 15, 2017 Notice of funding award o Informal notice we will need to get approval from ODOT, issue a contract, etc. All expenses on or after July 1, 2017 will be eligible. 13 Local Coordinated Planning Grants Overview
FUNDING AMOUNTS Good News: o No Limit to funds to be requested o Funds will be provided at 100% - No local match Not as Good News o We only have $250,000 total o Not all projects may be selected, not all expenses may be approved 14 Local Coordinated Planning Grants Overview

ALLOWABLE EXPENSES o Expenses Directly Related to Coordinated Planning o Staff time o Arranging and hosting meetings, performing research, drafting the plan, meeting with decision makers, performing outreach, working with ODOT o Community Meetings and Forums o Advertising, printing, renting meeting space, providing accommodations for people with disabilities o Performing surveys o Printing, advertising, social media 20 Local Coordinated Planning Grants Overview

GETTING REIMBURSEMENT o Funding based on completed deliverable o Agency submits coordinated plan for review/ approval o ODOT will review and approve the plan o Once the plan is approved, lead agency will provide invoice of all expenses o ODOT will review and approve invoice, process for final payment 26 Local Coordinated Planning Grants Overview
REPORTING AND MONITORING o Lead Agencies must follow all FTA and ODOT guidance o Including record keeping requirements o Records must be provided to ODOT upon request 27 Local Coordinated Planning Grants Overview
WHEN ARE THESE DUE? o If you want to apply for Section 5310 funding for CY 2019 o The need to be approved by ODOT by December 31, 2017 o Drafts due to ODOT by November 17 if this is not possible you need to communicate with ODOT o If you do not want to apply for Section 5310 until CY 2020 o The plan does not need to be approved by ODOT until December 31, 2018 28 Local Coordinated Planning Grants Overview
