Dear CBMS Partner, Thank you for expressing interest in the CBMS-AKFI Development Grant Program, our modest contribution to the clarion call for good governance and transparency in resource allocation through evidence-based needs identification, program design and impact monitoring. An application kit for submitting a request for grant assistance is attached. Additionally, we are providing you with the required forms and guidance documents to help you ensure that your proposal meets the minimum requirements as outlined in the attached brochure. This kit is available on the internet at the CBMS section of the PEP website: www.pep-net.org. Directions for Preparing the Cover Sheet The cover sheet is the first page of the application and is to be filled out by all applicants. See next page for a blank copy of the Cover Sheet. o The Project Name is the name of the proposed project submitted for funding. o Project Location is the site in which the project will be conducted. o The Organization Name is the group or any other organization applying for funding. Please include the organization s fax and telephone number. o The Contact Person is the person in that organization that can be contacted by the PEP-CBMS Network Office for any needed additional information. On the line provided, include the name and title of the contact person. o Fill in the Contact Person s Email address, if available. o Under Organization Address, be sure to include the street name and number on the first line and the zip code on the second line. o Duration of Project. Indicate the approximate amount of time needed to implement the project, in months. Note that the maximum duration of projects for the CBMS-AKFI Development Grant Program is 1 year. o Preferred Start Date. Indicate the month you would like to start your project. o The Person with Grant Acceptance Authority and Their Title. The person with grant acceptance authority is the person who has the authority to accept responsibility for the terms and conditions of the contract. This may be the contact person or somebody else in the organization. o The contact person also needs to sign the Cover Sheet as indicated, or submit the proposal via their email address. Include their title.

Directions for Preparing your Project Proposal To help you structure your project proposal, we are providing you with a project proposal format (see next page). Please provide the information in the order listed on the format. Consider each heading and the corresponding instructions as they apply to your project. Use 11- or 12-point font on 8.5 x 11 paper. Whenever possible, always refer to the results (Summary Table of CBMS Indicators and Poverty Maps) of the CBMS Survey conducted in the project site. Any meaningful information that helps support your project proposal but does not need to be in the body of the proposal should be included as appendices. Important Reminder: An application will not be considered and/or regarded as cancelled if it fails to meet the requirements as stated in the grant application kit and grant brochure. Delivery Instructions Applications must be emailed as an attachment, hand-delivered by 5 p.m., or sent by postmark dated no later than March 2, 2012. CBMS-AKFI Development Grant Program Project Proposal Format A. Project Description Maximum five pages. Please provide the following information in the order listed, using the topic headings. Consider each bulleted statement as it applies to your project. Use 11- or 12-point font on 8 ½ x 11 paper. Number the pages and attach to Page 1 (the Grant Application Cover Sheet). 1. Brief Summary of the Project o Indicate the main development concern that the project wishes to address and what is known about it. Provide empirical evidence about the concern raised by citing CBMS Survey results whenever applicable. o Elaborate on the potential impact of the project on the community. o Describe the indicators that will demonstrate that poverty conditions are being addressed. 2. Project Goals and Objectives o Describe project goals (what you hope to achieve) and objectives (measurable, if possible). o Describe how goals and objectives will be accomplished. 3. Targeted Beneficiaries o Identify the direct and indirect beneficiaries of the project. o Demonstrate how CBMS data was used to identify the beneficiaries of the project. 4. Partners o Describe any other partners in the project, their roles, and commitments. o Describe any other sources of funding for the project including other grants you have received that relate to this proposal. 5. Project Sustainability o Demonstrate how the benefits/returns of the project can be greater than the costs. o Describe how the project will be sustained after the CBMS- AKFI assistance. 6. Evaluation o Describe how the project will be evaluated.
B. Workplan & Timetable Maximum three pages. Identify and describe the major components and activities to be carried out to address the poverty conditions. The plan should be presented by task (with subtasks, as necessary). For each of the tasks, identify who will be responsible for carrying out the task. Include a timetable of activities showing when each task will be completed. This should be presented in a table format and cover all months or quarters of the project. C. Budget Information Maximum 2 pages. Fill out the form below. Include detailed financial schedules of the proposed budget (including notes and assumptions). E. Project Management Name the key officers and staff that will be directly accountable for the project. Describe the management scheme, internal monitoring and evaluation procedures. F. Checklist of Requirements Aside from the project proposal itself, the following should be submitted: Summary Table of CBMS Core Indicators and Poverty Maps (Barangay and Municipal level) Complete CBMS database of the LGU Location map showing project sites/areas List of targeted beneficiary households Development plan or socioeconomic profile of project site based on CBMS Other supporting/relevant documents showing absence (or shortage) of school facility Plan and Timeline of the LGU for Maintenance and Use of the Output/s of the Proposed Project
