Quatacker) Pages: 17 This instruction implements Air Force Instruction (AFI) 11-401, Aviation Management; Pacific Air Forces Command (PACAF), Supplement. This instruction establishes policies and procedures for conducting 354 Fighter Wing (FW) orientation flights at Eielson Air Force Base (AFB). It provides additional clarification on incentive, familiarization, and public affairs (PA)/civilian distinguished visitor (DV) flights. This publication applies to USAF active duty units and individuals attached to or assigned to the 354 FW at Eielson AFB, including personnel, including Air National Guard (ANG) and Air Force Reserve (AFRES) personnel, under 354 FW control (e.g., ADCON, TACON, OPCON). It does not apply to ANG or AFRES units or aircraft as those units will adhere to their respective supplements to AFI 11-401. Refer recommended changes and questions about this publication to the Office of Primary Responsibility (OPR) using the AF Form 847, Recommendation for Change of Publication. Route AF Forms 847 from the field through the appropriate functional s chain of command. Any change to this instruction must be coordinated with the 354 OSS/OSO. Ensure that all records created as a result of processes prescribed in this publication are maintained in accordance with AFMAN 33-363, Management of Records, and disposed of in accordance with the Air Force Records Information Management System (AFRIMS) Records Disposition Schedule (RDS). The 354 FW/CC is the waiver authority for this instruction except for items that require higher approvals per the parent AFIs. 1.9.4.7.1.1.1. (Added) PA/DV flight procedures:

(Added) PA/DV flights will normally only be performed between the months of April and October or when sustained ground temperatures remain above zero degrees Fahrenheit in order to meet the intent of AFI 16-1301_PACAF Sup 1, Table 2.1. 1.9.4.7.1.1.1.2. (Added) 354 FW/PA will manage the 354 FW PA/DV flight program and will maintain a prioritized 354 FW/CC-approved list of all requests. 1.9.4.7.1.1.1.3. (Added) 354 FW/PA will be authorized PA/DV flights on a case-by-case basis primarily between April and October each year or when sustained ground temperatures remain above zero degrees Fahrenheit in order to meet the intent of AFI 16-1301_PACAF Sup 1, Table 2.1. 1.9.4.7.1.1.1.4. (Added) Provide the 354 OSS/OSO the following information as soon as possible before the requested PA/DV flight: 1.9.4.7.1.1.1.4.1. (Added) An e-mail with pertinent background information stating why the individual should receive the flight that includes the following information and attachments: Personal Physician s Statement for use with 354 MDG Aerospace Medicine Flight to clear individual for flight in USAF aircraft, and a DD Form 2402, Hold Harmless Agreement. 1.9.4.7.1.1.1.5. (Added) Upon approval, the 354 FW/PA will coordinate all PA/DV paperwork with the 18 AGRS to ensure all documents are in the latter s possession prior to flight. The approval process should normally begin 6 weeks prior to the scheduled flight due to PACAF approval requirements (in some cases) that are due one month prior to the scheduled flight. 1.9.4.7.1.2.1. (Added) Familiarization Flight Procedures: 1.9.4.7.1.2.2. (Added) Familiarization flights do not normally restrict a mission profile, require at least a SECRET security clearance, and will be scheduled at the discretion of the flying squadron operations officer. Operations officers will consider the individual s background, expertise, and ability to apply the familiarization flight experience to daily mission accomplishment when determining the appropriate flight profile and mission events. 1.9.4.7.1.2.3. (Added) Familiarization flights will normally only be performed between the months of April and October or when sustained ground temperatures remain above zero degrees Fahrenheit in order to meet the intent of AFI 16-1301_PACAF Sup 1, Table 2.1. 1.9.4.7.1.2.4. (Added) IAW AFMOA/SG3P, AFI 11-403, Aerospace Physiological Training Program, paragraph 2.1.9., physiological training will be accomplished for familiarization flights no earlier than 72 hours prior to the flight. 1.9.4.7.1.2.5. (Added) 354 OSS/OSO will schedule physiological training with the 354 MDG Aerospace Physiologist or the Flight Surgeon. 1.9.4.7.1.2.6. (Added) Recommending commanders and the WSA will prioritize their lists in case the 354 OSS/OSO is unable to schedule all individuals in the time allowed each year. If no priority is specified, the 354 OSS/OSO will prioritize in the best interest of the program. When considering nominations, commanders should reference paragraph 1.9.4.7.4.8. 1.9.4.7.1.2.7. (Added) Because civilians are not authorized incentive sorties per AFI 11-401, the 354 FW/CC authorizes familiarization flights for 354 FW civilian annual award winners IAW familiarization flight guidelines (i.e., civilian annual award winners at the FW level). 1.9.4.7.1.3.1. (Added) Incentive Flight Procedures:

(Added) F-16 incentive flights reward military personnel assigned to the 354 FW for demonstrated outstanding service and motivate others to achieve similar performance levels. Incentive categories include 354 FW Annual Award winners and 354 FW Group/Staff Agencies allocations. 1.9.4.7.1.3.3. (Added) The 354 FW goal is to execute 28 dedicated incentive sorties annually. The goal reflects incentive flights only (award winners and allocations) and does not include any other category of orientation flights given by a squadron. 1.9.4.7.1.3.4. (Added) Incentive flights will normally be performed only between the months of April and October or when sustained ground temperatures remain above zero degrees Fahrenheit in order to meet the intent of AFI 16-1301_PACAF Sup 1, Table 2.1. 1.9.4.7.1.3.5. (Added) The allocation of incentive rides each year are divided between the WSA and the five groups (OG, MXG, MSG, MDG, and ASOG). The number of incentive flights allocated to each organization listed above is based upon the number of personnel assigned to each unit (refer to Attachment 6, Table A6.1). Each group commander controls how his or her incentive flight allocations are distributed between individuals in the group, and the group commander will appoint an orientation flight point of contact (POC) to work directly with the 354 OSS/OSO. Likewise, the Director of Staff (DS) will coordinate names with the CPTS/CC for the WSA and a designated POC will submit nominees to the 354 FW/OSS once the 354 FW/CV has approved the nominees. When a person receives an incentive flight, that sortie will then be charged against the organization s total allowance and the 354 OSS will highlight the current status of sorties during the weekly 354 FW Staff Meeting normally held each Wednesday. 1.9.4.7.1.3.6. (Added) In each case, requests are subject to mission requirements and sortie availability. Group and WSA orientation flight POCs are required to conduct a quarterly review of their nominees to eliminate personnel who have PCS d, retired, or who do not wish to fly. The group or WSA POCs will submit flight requests to 354 OSS/OSO NLT 15 Jan and 15 May of each year. Out of cycle requests will be considered on a case-by-case basis. 1.9.4.7.1.3.7. (Added) IAW AFMOA/SG3P, AFI 11-403, Aerospace Physiological Training Program, paragraph 2.1.9., physiological training will be accomplished for incentive flights no earlier than 72 hours prior to the flight. 1.9.4.7.1.3.8. (Added) 354 OSS/OSO will schedule physiological training with the 354 MDG Aerospace Physiologist or the Flight Surgeon. 1.9.4.7.1.3.9. (Added) Incentive flights may be scheduled as a formation or single-ship, but formation flights are desired. Special care and attention must be taken before the flight to ensure the mission profile is tailored specifically to the interests, needs, and physiological limitations of the recipient. During the flight, the instructor must communicate with the recipient and change the planned profile if necessary. 1.9.4.7.1.3.10. (Added) Pilots will perform aerobatic maneuvers only when approved by the passenger unless safety of flight necessitates particular maneuvers. 1.9.4.7.1.3.11. (Added) External air-to-ground ordnance will be downloaded and the gun mechanically safed.

4 AFI11-401_EIELSONAFBSUP 26 MAY 2015 1.9.4.7.1.3.12. (Added) Personnel will be restricted to one incentive sortie while assigned to the 354 FW. 1.9.4.7.4. (Added) 354 OSS/OSO (Current Operations) Responsibilities: 1.9.4.7.4.1. (Added) Determine the level of approval required IAW AFI 11-401 and AFI 11-401_PACAFSUP_1. 1.9.4.7.4.2. (Added) Prepare and staff a consolidated 354 FW Orientation Ride Incentive/Familiarization Flight MFR as required. Approved nominations are valid until the FW/CC signs an updated Incentive/Familiarization Flight MFR. 1.9.4.7.4.3. (Added) Monitor, track, and prioritize incentive flight requests to ensure group quotas are not exceeded (Attachment 6). Summarize the orientation program statistics and progress weekly at the 354 FW Staff Meeting normally held each Wednesday. 1.9.4.7.4.4. (Added) Determine incentive flight priorities from the 354 OSS/OSO master listing for the upcoming week based on availability of back seats allocated by the 18 AGRS. Individuals will be selected in sequential order if possible. 1.9.4.7.4.5. (Added) If an individual declines three opportunities to fly (excluding leave/tdy/mission essential duties), he or she will be removed from the master listing unless waived by the 354 FW/CC. 1.9.4.7.4.6. (Added) Provide the individual receiving an orientation ride the approved Incentive/Familiarization Flight MFR and notify him or her of the flight prerequisites. 1.9.4.7.4.7. (Added) Schedule appointments for orientation flight personnel to accomplish flight prerequisites (e.g., medical clearance, egress training, physiological training). 1.9.4.7.4.8. (Added) Ensure orientation flight individuals meet weight requirements IAW AFI 48-123, Flying and Special Operations Duty, Table 6.1. The F-16 has an ejection seat design limit (SDL) of 140-211 lbs. The F-16 weight for flight (WFF) is 103-245 lbs. Persons outside the SDL (weighing less than 140 lbs or more than 211 lbs) must receive a waiver in order to receive an orientation flight. Persons outside the WFF (weighing less than 103 lbs or more than 245 lbs) will not receive orientation flights. Waiver approval authority for military personnel outside the SDL but within the WFF, including USAFA and AFROTC cadets, is the 354 FW/CC. For civilians whose weight falls outside the SDL but within the WFF, PACAF/A2/3/10 is the approval authority. 1.9.4.7.4.9. (Added) Maintain a master roster of all approved orientation flight personnel. Incentive lines will be filled in the following order: award winners, approved group allocations, and PA flights. 1.9.4.7.4.10. (Added) Advertise dates and times of upcoming PA/DV flights to 18 AGRS via the Wednesday OG/MXG scheduling meeting. All other flights will be coordinated directly between the 354 OSS/OSO and the 18 AGRS Scheduling Office. 1.9.4.7.5. (Added) 18 AGRS: 1.9.4.7.5.1. (Added) Provide 354 OSS/OSO the following week s anticipated available back seats and dedicated incentive flights via email NLT Thursday at 1200L if not accessible via PEX. Strive to schedule available D-models for incentive flights at least two weeks in advance to allow nominees time to rearrange their work schedules.

(Added) Squadron schedulers will provide their AFE shops at least one day advanced notice of the orientation flight equipment fit requirement. 1.9.4.7.5.3. (Added) Contact PA at least one day prior to the sortie to verify dates and times of DV/PA flights. 1.9.4.7.5.4. (Added) Verify the individual receiving an orientation sortie has a photo pass. If not, attempt day-of coordination if possible and if desired. Otherwise, ensure video recording devices are not taken onto the airfield. The ultimate responsibility for gaining photo pass approval rests with the individual receiving the orientation flight (see section 1.9.4.7.6.2.9). 1.9.4.7.5.5. (Added) Prepare a certificate of flight if time permits. 1.9.4.7.5.6. (Added) Utilize the DD Form 2131, Passenger Manifest, for all orientation flyers and also update the Orientation Flight Log daily with all back seat sorties accomplished for the day. The Orientation Flight Log is located on the 354 OSS shared drive in the OSO folder and organized by fiscal year. 1.9.4.7.6. (Added) Responsibilities of Personnel Selected to Receive an Orientation Flight: 1.9.4.7.6.1. (Added) When notified by the 354 OSS/OSO of an upcoming orientation flight, contact Current Operations (building 4110) to coordinate required appointments before the orientation flight. 1.9.4.7.6.2. (Added) Hand-carry all forms and checklists provided by the 354 OSS/OSO to the Aerospace Medical Clinic (building 3349). Upon successful completion of the medical appointment, maintain a copy of the medical clearance (AF Form 1042 or DD 2292). 1.9.4.7.6.2.1. (Added) Remain within the weight limits set forth in AFI 48-123. The minimum weight of persons flying in the F-16 is 140 pounds and the maximum is 211 pounds. For military members (including USAFA and ROTC cadets), the 354 FW/CC is the waiver approval authority for individuals between 103 and 245 lbs. PACAF/A2/3/10 is the approval authority for civilian weight waivers. 1.9.4.7.6.2.2. (Added) On the day of flight, be within the prescribed weight limits per paragraph 1.9.4.7.6.2.1. 1.9.4.7.6.2.3. (Added) Complete F-16 Egress/Hanging Harness training at 354 OSS/OSL and obtain an AF Form 1522 upon successful completion of the course. 1.9.4.7.6.2.4. (Added) After completing all prerequisites, return to 354 OSS/OSO to complete a Go-No Go review of all paperwork. 1.9.4.7.6.2.5. (Added) Contact 18 AGRS Operations Desk, 377-6164, to verify show times for flight at least one day prior. Hand-carry all paperwork (AF Form 1042 or DD 2292, physiological training card or AF Form 1522 indicating receipt of physiological training, and AF Form 1522 indicating receipt of egress training) to the 18 AGRS to perform the flight. 1.9.4.7.6.2.6. (Added) Rated individuals in Fighter Aircraft (T-38, F-16, F-15C, F-15E, F-22, F-35) currently on flying status do not require local flight surgeon clearance, but must present a copy of their approved Incentive/Familiarization memorandum for record (MFR), AF Form 1042 or DD2292, and AF Form 1522 indicating egress training to the 18 AGRS. For all other rated aircrew, local flight surgeon clearance is required.

(Added) As scheduled, but NLT one hour prior to the briefing time, visit the Aircrew Flight Equipment section of the 18 AGRS to ensure proper fit of all required flight equipment. 1.9.4.7.6.2.8. (Added) In lieu of a medical record review, civilians will provide the Aerospace Medical Clinic a statement of health from their physician, including a summary of medical problems and medications. Civilians will then hand-carry the approved Incentive/Familiarization Flight MFR, statement of health, a current physiological training card or AF Form 1522 indicating receipt of physiological training, and an AF Form 1522 (indicating receipt of egress training) to the 18 AGRS to perform the flight. 1.9.4.7.6.2.9. (Added) To the maximum extent possible, include the family of the individual receiving a flight in all briefings and flight activities. This includes coordinating for a flight line photo pass with the 354th Security Forces Squadron via 354 FW/PA. This format has been adapted to allow for an easy-to-use program reference. The table contains a list of the programs, as well as where in the supplement to find information about a specific program, purpose, qualifications, restrictions, and approval authority. The approval authority for all orientation flights is the 354 FW/CC or higher as required by parent AFIs. Incentive and familiarization flight MFRs must state both familiarization and incentive flights in order to provide maximum flexibility. A5.2.1.3. (Added) 354 FW DCCs are eligible for incentive or familiarization flights on a noninterference basis at Eielson AFB or deployed/tdy locations. A5.2.1.4. (Added) Each AMU s orientation flight POC will be responsible for submitting a prioritized list of no more than 10 personnel to 354 OSS/OSO semi-annually (DCCs may not fly under the OG-MXG Exchange Program) and can update the list at any time, but at least semiannually. A5.2.2.2. Maintenance personnel recommended for familiarization flights must have no active or pending administrative actions against them, have a passing PT Test, and have a specialty that directly impacts flying operations at Eielson AFB. A5.2.2.3. Maintenance personnel will be restricted to one sortie while assigned to the 354 FW. A5.3. (Added) Visiting Rated Pilots and Weapons Systems Officers (WSOs): A5.3.1. (Added) If visiting rated aircrew are seeking to log any time on the AFTO Form 781, then refer them to Interfly procedures located in AFI 11-401, PACAF Sup paragraph 1.3.3. Otherwise, they will need a 354 FW/CC approved Incentive/Familiarization Flight MFR. A5.3.2. (Added) Refer to Interfly procedures in AFI 11-401, PACAF Sup paragraph 1.3.3. regarding rated F-16 pilots from other MAJCOMs. Non-current F-16 pilots in PACAF must meet requirements listed in para 1.9.4.7.1.2.1. A5.3.3. (Added) Visiting rated pilots and WSOs are not limited to sortie types. A5.4. (Added) Cadets: A5.4.1. (Added) USAFA and AFROTC cadets are allowed an unlimited number of familiarization flights on a non-interference basis. A5.4.2. (Added) Cadets must have a letter from their Air Officer Commanding (AOC) or commanding officer giving them permission for the flights. The letter must include the following information: cadet s name, height, weight, dates of visit, physiological training card status if applicable, security clearance status, and a phone number where the cadet can be contacted once they are in the local area. A5.4.3. (Added) Cadets must have an official memorandum from their respective AOC or commanding officer stating they are eligible for each repeat visit. If multiple requests are received at the same time, the priority will be: cadets on official orders and cadets on leave. Prioritization will be based on the date of request. All cadets require an approved Incentive/Familiarization Flight MFR or aeronautical orders, AF Form 1522 (Egress/Hanging Harness), flight surgeon medical clearance, and physiological training. A5.4.4. (Added) 1.9.4.7.1.2.1. Cadets are not restricted on the type of sortie flown IAW paragraph A5.5. (Added) Federal Aviation Association (FAA) personnel: A5.5.1. (Added) FAA personnel are allowed one familiarization sortie to enhance their understanding of Eielson AFB flying operations. A5.5.2. (Added) FAA personnel will be flown on continuation training sorties only, and handled on a case-by-case basis. Follow familiarization guidelines in para 1.9.4.7.1.2.1. A5.6. (Added) Tactical Air Control Party (TACP) and Joint Terminal Air Controller (JTAC):

(Added) Members will coordinate with 354 OSS/OSO at least two weeks prior to the flight. An approved Incentive/Familiarization Flight MFR is required for flight. A5.7. (Added) Aircrew Flight Equipment (AFE): A5.7.1. (Added) AFE personnel are authorized one familiarization sortie on a non-interference basis while assigned to the 354 FW. A5.7.2. (Added) The AFE Superintendent will be responsible for submitting a prioritized list of personnel to 354 OSS/OSO semi-annually. A5.8. (Added) Weather Personnel: A5.8.1. (Added) Airmen assigned to 354 OSS/OSW are authorized one familiarization sortie on a non-interference basis. Weather personnel will be restricted to one sortie while assigned to the 354 FW. A5.8.2. (Added) The Weather Flight Chief will be responsible for submitting a prioritized list of personnel to 354 OSS/OSO semi-annually. A5.9. (Added) Airfield Operations (AOF) Personnel: A5.9.1. (Added) Air traffic controllers, ATCALS, and airfield management personnel assigned to the 354 FW are authorized one familiarization sortie on a non-interference basis. A5.9.2. (Added) The OSA Flight Commander will be responsible for submitting a prioritized list of personnel to 354 OSS/OSO semi-annually. A5.10. (Added) Intelligence Personnel: A5.10.1. (Added) Airmen assigned to 353 CTS/IN are authorized one familiarization sorties on a non-interference basis while assigned to the 354 FW. A5.10.2. (Added) The Senior Intel Officer will be responsible for submitting a prioritized list of personnel to 354 OSS/OSO semi-annually. A5.11. (Added) Aerospace Medicine Personnel: A5.11.1. (Added) Airmen assigned to the Aerospace Medicine Flight are authorized one familiarization sortie on a non-interference basis while assigned to the 354 FW. A5.11.2. (Added) The Aerospace Medicine Flight Chief will be responsible for submitting a prioritized list of personnel to 354 OSS/OSO semi-annually. A5.12. (Added) Aviation Resource Management Personnel: A5.12.1. (Added) Aviation Resource Management personnel are authorized one familiarization sortie on a non-interference basis while assigned to the 354 FW. A5.12.2. (Added) The 1C0X2 Functional Manager will be responsible for submitting a prioritized list of personnel to 354 OSS/OSO semi-annually. A5.13. (Added) Non-Flying Group/Deputy Group Commanders and Squadron Commanders/Director of Operations:

(Added) Non-flying group/deputy commanders, and non-flying squadron commanders are authorized one familiarization flight and will be submitted on a blanket Incentive/Familiarization Flight MFR. A5.13.2. (Added) 354 OSS/OSO will make every attempt to fly the non-flying group/squadron commanders and directors of operations within the first six months of their assignment. An approved Incentive/Familiarization Flight MFR is required for flight. Approved familiarization sorties will be flown on a non-interference basis. Non-flying group commander/deputy group commanders and non-flying squadron commanders/director of operations personnel will be restricted to one sortie while assigned to the 354 FW. A5.14. (Added) Survival, Evasion, Resistance, and Escape (SERE) Personnel: A5.14.1. (Added) 354 FW assigned SERE personnel are authorized one familiarization sortie on a non-interference basis while assigned to the 354 FW. A5.14.2. (Added) The 354 FW SERE Superintendent will be responsible for submitting a prioritized list of personnel to 354 OSS/OSO semi-annually.
