Appendix B: Welcme Baby: Summary f Jb Respnsibilities fr Key Persnnel Prgram Management Staff Prject Directr (suggested qualificatins include: B.A. r Masters level in Public Health, Public Administratin r Early Childhd Develpment with experience in prgram implementatin, hme visitatin prgrams, and knwledge f maternal child health). The Directr has the verall respnsibility fr the implementatin f the Welcme Baby prgram. The Prgram Directr must wrk effectively as part f a multidisciplinary team that includes the hspital, ther Welcme Baby sites in their cmmunity, Best Start Cmmunity Partnership, First 5 LA, and Hme Visitatin Oversight Entity. Oversees and mnitrs prgram t ensure standardized implementatin, including adherence t prtcls, plicies, prcedures and dcumentatin. Assists in maintaining and imprving the quality f the prgram by ensuring that the fllwing is cnducted by the management team: Bi-mnthly grup reflective supervisin with hme visitatin staff; Weekly individual reflective supervisin f hme visitatin staff; Peridic shadwing f hme visitatin staff n hme visits; and Regular chart reviews t ensure the accurate cllectin f data and cmpletin f all client dcumentatin in a timely and rganized manner. Mnitrs prgress in achieving prgrammatic gals and bjectives within the Scpe f Wrk. Prepares quarterly and year-end reprts. Cllabrates with the Oversight Entity and First 5 LA t assist with data and evaluatin needs. Mnitrs prgram implementatin fr nging prblems and barriers and identifies pssible slutins. Wrks clsely with ther lcal prviders t develp a cmprehensive referral netwrk and track any gaps r barriers t clients being able t access needed services. Assists with fiscal mnitring f prgram and helps frmulate annual budget and needed budget mdificatins thrughut the year. Supervises prgram staff, including Clinical Supervisr, Outreach Specialists, and cnducts administrative and individual reflective supervisin. Cnducts mnthly staff meetings t address and btain feedback n prgram updates, needs, challenges, and utcmes. Wrks clsely with Clinical Supervisr t assess nging needs fr staff training and t identify internal r external prfessinal develpment activities. Participates in lcal cmmunity meetings and develp relatinships with prviders and cmmunity rganizatins t enhance partnerships and cllabratin as apprpriate. Shares key prgram data with partners and ther stakehlders n a quarterly basis, including utcmes, demgraphics, and enrllment infrmatin. 1

Establishes a relatinship and visibility fr the prgram with key persnnel at the hspital, including Labr and Delivery, Cuplet Care, NICU and Scial Wrk staff. Cllabrates with First 5 LA marketing effrts. Wrks clsely with the Outreach Specialist and Parent Caches t mnitr and imprve prenatal utreach, acceptance and enrllment. Clinical Supervisr (suggested qualificatins include: Registered Nurse, Licensed Clinical Scial Wrker, r Licensed Develpmental Psychlgist). The Clinical Supervisr will be respnsible fr day-t-day versight f Welcme Baby hme visitatin staff. They will versee Parent Cach Team Supervisrs and Registered Nurses (RNs) and ensure staff are prperly trained, receive reflective supervisin, and prvide empathetic and client-centered services fr clients. The Clinical Supervisr will wrk clsely with the Prject Directr and Data and Evaluatin Manager t mnitr prgress in achieving prgram gals and bjectives. It is expected that the Clinical Supervisr wuld be hired by the cmmunity-based partner r entity. Supprts and trains staff n identifying client strengths, needs, barriers and risks and identifies cmmunity resurces fr families in need. Prvides reflective supervisin fr individual staff n a weekly basis and a biweekly grup basis fr all hme visitatin staff. Wrks clsely with the Prject Directr t implement dcumentatin, training, prcedures, plicies and prtcls. Ensures the quality f the prgram by verseeing services prvided thrugh peridic shadwing f hme visits and cnducting regular chart reviews. Wrks clsely with the Outreach Specialist(s) cnducting utreach and Hspital Liaisns t track enrllment f new clients and assign cases t Parent Caches and Welcme Baby Nurses in a timely manner. Tracks client enrllment and frecasts upcming visits in rder t assign cases and mnitr visits fr each Parent Cach and Welcme Baby Nurse in a fair and equitable manner. Prvides cnsultatin t all prgram staff n high risk clinical issues such as mental illness, develpmental delays, child abuse and family vilence t ensure that the needed supprt and fllw-up is prvided. Cnducts nging team building activities t enhance emplyee relatins and teamwrk. Mentrs and assists with nging training f emplyees t ensure that they are mdeling empathy and prviding hlistic and strength-based services. Explres and identifies new referrals and resurces fr clients. Assists with and prvides nging staff develpment related t the issues and needs f clients such as maternal and infant health, breastfeeding, parenting, attachment, hme safety, family planning and child develpment. Data and Evaluatin Manager (suggested qualificatins include B.A. r Masters level in Public Health, Public Administratin r related field, with experience in prgram evaluatin and data management). The Data and Evaluatin Manager assists the Prgram Directr with prgram implementatin and evaluatin activities related t ensuring delivery f quality services and achievement f prgram gals. 2

The Data and Evaluatin Manager als wrks clsely with the Prgram Directr and Clinical Supervisr t develp and implement systems fr prgram management, mnitring, quality assurance and reprting f utcme measures. The Data and Evaluatin Manager will truble-sht data system prblems and barriers t accurate prgram evaluatin n an n-ging basis, as well as implement data quality assessment prcedures t review data validity and implement quality assurance tls. It is expected that the Data and Evaluatin Manager wuld be hired by the cmmunity-based partner r entity. Wrks clsely with the Prgram Directr and Clinical Supervisr t develp and assess prgrammatic gals and structure. Assists in the develpment and imprvement f prgrammatic prtcls t ensure the quality f service delivery, data cllectin and evaluatin. Develps and implements systems fr nging prgram management and assessment f utcmes. Prvides feedback n successful prgram mnitring strategies t help achieve prgram gals and bjectives. Wrks with the Family Strengthening Oversight Entity and First 5 LA t ensure the accuracy f utcme measure reprts. Implements quality assessment tls t ensure that all client data is entered in a timely manner by prgram staff and the data entry clerk. Assists with regular chart reviews t ensure the accurate cllectin and entry f data and adherence t prgram prtcls. Develps and implements a quality imprvement plan. Assists with the preparatin f quarterly and year end prgram reprts fr First 5 LA. Attends partner meetings, Oversight Entity led meetings, and ther cmmunity meetings, as apprpriate. Outreach Specialist (suggested qualificatins include: Bachelr s degree in child develpment, scial wrk, psychlgy, human develpment, r a related field r Child Develpment Assciate (CDA) certificatin preferred). The Outreach Specialist will cnduct cmmunity utreach at health centers, prvider ffices and ther scial service sites aimed at recruiting and enrlling eligible clients int the Welcme Baby prgram. The Outreach Specialist will als be respnsible fr develping relatinships with key cmmunity cntacts, maintaining prenatal utreach lgs, fllwing up with ptential clients, and cnducting client intakes. The Outreach Specialist will wrk clsely with the Prgram Directr, Clinical Supervisr, and ther team members t prvide input int the prgram dcumentatin, training, prcedures and verall structure f the prgram. Wrks clsely with all prgram staff t prvide a cntinuum f services. Develps relatinships with prviders and cmmunity rganizatins. Participates in apprpriate cmmunity meetings and marketing and cmmunicatins effrts as needed. Prvides input n cmmunicatin and utreach strategies. Cnducts utreach (grup and ne-n-ne) at prvider sites and cmmunitybased rganizatins t recruit ptential clients. 3

Cntacts ptential clients frm utreach t encurage participatin in the prgram and enrll them, if apprpriate. Cnducts client intakes, including cllecting key infrmatin in a sensitive and nnjudgmental manner. Welcme Baby Hme Visitatin Staff Parent Caches (suggested qualificatins include: Bachelr s degree in child develpment, scial wrk, psychlgy, human develpment r related field, Child Develpment Assciate (CDA) and experience in prviding hme visitatin services, r cmmunity health wrker/prmtra). These Parent Caches will cnduct the prenatal and pstpartum visits in the hme and ver the phne. Parent Caches are respnsible fr supprting families, cnducting assessments and prviding parent educatin in the areas f prenatal and pstnatal care, breastfeeding, parent-infant attachment, child develpment, hme safety and ther maternal and child health related tpics. They will als help identify client strengths, needs, barriers and risks and prvide cmmunity resurces fr thse needs and prvide and mdel empathetic supprt and feedback when wrking with new mms, babies and family members. It is expected that Parent Caches wuld be hired by the cmmunity-based partner r entity. Wrks clsely with the Welcme Baby Nurses, ther Parent Caches, supervisrs, and administrative staff t prvide a cntinuum f services. Develps trust and rapprt with clients and prvides supprt using an empathic and client centered apprach. Adheres t all prgram guidelines, plicies and prtcls and actively wrks tward achieving the gals f the prgram. Effectively cnducts required client assessments using standardized tls fr maternal depressin, parent-infant attachment, scial supprt, develpmental milestnes, and hme safety, as well as ther nnstandardized client assessments. Assesses infant feeding and prvides assistance and supprt as needed. Prvides infrmatin t clients abut maternal and infant health, breastfeeding, parenting, attachment, hme safety, family planning, child develpment and ther related tpics using a client-centered apprach. Identifies apprpriate resurces fr clients and prvide referrals as needed. 4

Attends and participates in all staff meetings, bi-mnthly grup reflective supervisin, and weekly individual reflective supervisin with Team Supervisr. Cmpletes all needed paperwrk fr each client in a timely and rganized manner, including clear and well written prgress ntes. Cmpletes required data entry fr each client in a timely manner. Hspital Liaisns (suggested qualificatins include: Bachelr s degree in child develpment, scial wrk, psychlgy human develpment r related field, r Medical Assistant certificatin and previus wrk experience in maternal and newbrn health services preferred). It is expected that mst hspitals will elect t hire Hspital Liaisns as hspital staff. Participating hspitals may chse t hire this staff directly as hspital emplyees r partner with ne r mre cmmunity-based rganizatins r entities and prvide access rights fr the rganizatin t cnduct the hspital visit. If the hspital has partnered with ne r mre cmmunity-based rganizatins, it is expected that the hspital will crdinate with the rganizatins t identify prenatally enrlled clients and cmmunicate the results f the risk screening assessment fr apprpriate referrals. Hspital Liaisns will be based in the hspital, engage families after delivery, and enrll new families int the prgram. They will then have t infrm any rganizatins r entities the hspital may have partnered with f the new families infrmatin s hme visits can be scheduled. Recmmended Hspital Liaisn qualificatins include: Appraches prspective clients at bedside t intrduce the prgram, verifies willingness t participate, btains cnsent, and gathers referral infrmatin. Develps a file fr each client recruited int the prgram, dcumenting all cntacts with the family and agencies and cmpleting data frms and prgram dcumentatin. Develps trust and rapprt with clients and prvides supprt using an empathic and client centered apprach. Adheres t all prgram guidelines, plicies and prtcls and actively wrks tward achieving the gals f the prgram. Cnducts an interview with each client recruited int the prgram, bserving parent-infant interactin; assessing invlvement f father r ther secndary caretaker, level f scial supprt, infant feeding and maternal depressin; and prvides breastfeeding supprt and newbrn health educatin. Effectively cnducts required client assessments using standardized tls fr maternal depressin and a risk screening interview. Wrks clsely with the Clinical Supervisr, Supervisr and administrative staff t prvide a cntinuum f services. Cnfers with assigned nurse regarding need fr any additinal hspital services r referrals and assists with referral prcess, if needed. Cllabrates and acts as liaisn with ther members f the healthcare team, including nurses, lactatin specialists, scial wrkers, and physician t crdinate prvisin f high-quality patient care. 5

Attends and cmpletes all trainings, including successful cmpletin f CLE training prgram. Attends and participates in all hme visitatin staff meetings, including bimnthly grup reflective supervisin. Cmpletes all needed paperwrk fr each client in a timely and rganized manner, including clear and well written prgress ntes. Cmpletes required data entry fr each client in a timely manner. Registered Nurses (RNs). Welcme Baby Nurses will be respnsible fr visiting families at that critical first hme visit after discharge frm the hspital. The Welcme Baby Nurse is respnsible fr supprting families, cnducting maternal and newbrn assessments and prviding parent educatin in the areas f pstnatal care, breastfeeding, parent-infant attachment, hme safety and ther maternal and child health related tpics. They will als help identify client strengths, needs, barriers and risks and prvide cmmunity resurces fr thse needs and prvide and mdel empathetic supprt and feedback when wrking with new mms, babies and family members. Wrks clsely with ther Welcme Baby Nurses, Parent Caches, supervisrs, and administrative staff t prvide a cntinuum f services. Cnducts hme visits with pstpartum clients within apprximately ne week f hspital discharge. Develps trust and rapprt with clients and prvides supprt using an empathic and client centered apprach. Adheres t all prgram guidelines, plicies and prtcls and actively wrks tward achieving the gals f the prgram. Cnducts a maternal assessment f the client fr pstpartum recvery and signs and symptms f pstpartum cmplicatins. Cnducts a head t te examinatin f the newbrn t ensure healthy develpment, apprpriate weight gain and assess fr signs and symptms f pssible cmplicatins, such as jaundice r dehydratin. Assesses infant feeding and prvide assistance and supprt as needed. Effectively cnducts required client assessments using standardized tls fr maternal depressin, parent-infant attachment, scial supprt, hme safety, as well as ther nn-standardized client assessments. Refers clients t medical prvider if any cmplicatins with the mther r infant are identified and prvides referrals fr ther services as needed. Prvides infrmatin t clients abut maternal and infant health, breastfeeding, parenting, attachment, hme safety, family planning, infant behavir and ther related tpics using a client-centered apprach. Attends and cmpletes all trainings, including successful cmpletin f CLE training prgram. Attends and participates in all staff meetings, bi-mnthly grup reflective supervisin, and weekly individual reflective supervisin with Clinical Supervisr.
