HEALTHY STEWARDS 2014 & WELLNESS REWARDS Program Length: Twelve months long from 1/1/2014 to 12/31/2014. Activities for Rewards: $100 Wellness Profile Complete a brief questionnaire to learn more about your health and receive a personal plan. $100 Health Screening - The screening includes measures for cholesterol levels (total, good & bad, triglycerides) & blood glucose (sugar). $100 Setting a Coaching Goal Set a goal with a Blues on Call Health & Wellness Coach, or set a goal & make progress online with MyHealthAssistant coaching. 3

COMPLETING THE HEALTH SCREENING A quick, easy and FREE process to determine your current health status, based on blood testing. Results and recommendations for improving screening values. Screening results are completely confidential; your individual information is never shared with the PBUCC. Screening results will be automatically incorporated into your online Health Record for your reference. Measurements include: Total Cholesterol HDL ( good ) cholesterol LDL ( bad ) cholesterol Triglycerides Glucose (blood sugar) 13

HEALTH SCREENING OPTIONS* 1. Lab Voucher Voucher or requisition provided to you to obtain testing through a venipuncture (blood draw) at any LabCorp facility in the U.S. Lab Voucher sent to you by email or by postal mail You will receive your results by mail within approximately 3 weeks from your testing date 2. Home Test Kit Test kit with all supplies for you to provide a blood sample through a fingerstick (finger prick), and send the blood sample by mail to a laboratory for analysis Home Test is sent directly to your home address; no cost for you to send your blood sample to the laboratory You will receive your results by mail within approximately 3-4 weeks from your testing date. 3. Physician Results Download form for your health care provider to complete with your lab test results obtained within the past 6 months. *Onsite screening services to be provided at certain UCC office locations. 14

Important Health Screening Dates In order to ensure receiving your Health Screening incentive in 2014, you will need to send in your home test kit or complete your lab visit (using a LabCorp voucher) no later than October 15, 2014. Participants whose screenings are completed after this date will receive the incentive in 2015. The last day to order a home test kit or voucher for the 2014 Healthy Stewards Wellness Rewards program is November 15, 2014. 16
