ROTARY INTERNATIONAL DISTRICT 6490 2014-2015 DISTRICT GRANT APPLICATION (MATCHING DISTRICT GRANT or CLUB GRANT) BEFORE SUBMITTING YOUR GRANT APPLICATION, CHECK ALL THE FOLLOWING REQUIREMENTS GRANT APPLICATION o Completely describes the project, location and objectives. o Explains the community need for the project. o Estimates the number of people who will benefit. o Explains how the club members will be involved (beyond distributing or spending funds). o Explains how the project will proceed if only partial funding is available. o Lists at least two club contacts. o A complete itemized budget for the project is included or attached. o Project timetable is included. Club understands deadlines and will submit reports on time. o Project will be completed within the Rotary year granted. (Exceptions may be allowed) o Exclude any liability to District 6490, the Rotary Foundation or Rotary International (except for the amount of the grant). o For international projects Communication and work responsibilities are described. o For international projects Letter(s) of support (if any) from project partner(s) are included or attached. TERMS AND CONDITIONS FOR DISTRICT GRANTS o Contributions to the Rotary Foundation or lack thereof will have an impact on grants awarded. o Project supports service activities and humanitarian endeavors. o Club will be responsible for overseeing funding and implementation of the project. o Promotes active and personal participation of Rotarians in the implementation of the project. o Club will promote club s involvement with appropriate signage, labels, public relations, etc. o Club will not establish a permanent foundation, trust or interest bearing account. o Project does not directly benefit a Rotarian, spouse, lineal descendant or ancestor. o Grant is not a reimbursement for a project that is already in progress or already completed. o Club is the primary sponsor of the project, not some other non-rotary organization. o None of the funds will be used for salaries, stipends or honorariums for project beneficiaries or individuals working for the project partners. o None of the funds will be used for travel expenses. o None of the funds will be used for scholarships or partial scholarships of any kind. o Any uses of the name Rotary or Rotary s symbols will comply with Rotary International policies. o The funded project cannot involve a new structure nor add to an existing structure where individuals live, work or engage in some gainful activity. Funds will not be used to construct a structure. Funds will not be used to renovate a structure. Funds will not be used to provide new services to a structure. Funds will not be used to upgrade the electrical or plumbing of a structure. REPORTING AND STEWARDSHIP o Reports must be submitted every six months as the project moves forward. o A Final Report must be submitted within 30 days of the full expenditure of the grant funds. o Club will submit receipts for all expenditures. o Club will provide competent and proper stewardship of all Grant funds. o Club will submit independent financial audit results if requested by the District 6490 Audit Committee. 1

In consideration of receipt of these grant monies, we agree to perform the work necessary to complete this project in the time frame stated and to submit a complete final report within 30 days of the project being completed. Failure to follow the requirements as outlined in the above identified documents including failure to complete the project or to timely file the Final Report can disqualify the Club from future District Grants and/or necessitate the repayment of the grant award received.
