Let us know If you know anyone who would be interested. Decision taken not to mention HEV on patient information leaflets (treated the same as CMV) First draft of PBM Survey. Looking for somewhere to host them National Standards for Clinical Transfusion Process final draft ratified, waiting for platform to host. SHOT & TLM Group want to highlight errors caused by staff shortages. Staff shortage and staff not having time to attend training is a national concern. When reporting errors, please identify whether the cause was staff shortage. London RTC Update Jen Heyes is now back and covering London Clare Denison will be returning in July and also cover London. Rachel Moss will be covering South Central and London until October Emily Carpenter will be covering South East Coast and London until October Haematology & Trauma Group: Holding an all day free education event on 18 th May at St. George s Hospital. Working on an aide-memoire re. major incidents following terrorist attacks LoPAG Group: Where do Platelets Go RTC and hospitals reports finished. Copies of both available from PBM team. Platelet Champion Newsletter No5 sent out in January. Copies available from PBM team Platelet Champion Day planned for October/November 2016.

National Platelet Group now established Massive Haemorrhage DVD: North West region s DVD is finished. Filming of ours will start in the summer if deemed appropriate. Nurse Authorisation: Still waiting for response from RCN. Has anyone got an idea how to engage with either RCN or NMC? Nurse Lanyard Cards: Have now been printed. Will discuss with TP group how to take forward. BMS Empowerment: Two empowerment workshops held in March and April. Consent Group: Looking for new Chair Will discuss with TP group how to implement in Trusts. January Renal Day Feedback: RTT not aware of another region that has done an educational day just for renal medicine. Evaluated well with 100% top box score. Permitted presentations are on our You Tube channel Search for London RTC. https://www.youtube.com/channel/ucadmgic5h5d9gpu3ekadjlq 04/16 Shared Care Update Results for patients not irradiated blood when required in 2014 SHOT report were poor No audit has been done since the recommendations were introduced. Less than 10% of hospitals are using the shared care form. Please use the form it is available on JPAC website and can be amended to suit your own hospital. If you are not following the shared care process, you cannot be sure you are transfusing the right blood. How can we put HEV under special requirements on the shared care form, we need to transfer the information to another Trust. Such patients need a I need special blood card, but not sure if it is going to be given to patients. 05/16 NHSBT Update HEV ve Products: Stock is now available. NHSBT able to meet demand of RBC, FFP and Cryo, but HEV platelets remains a daily challenge. Therefore, would like hospitals to minimise the stock they hold, but appreciate that big transplant centres need more. CMV Testing: Has been reduced for blood donations following downgrading of risk by SaBTO. This has been trialled in the North East and was successful. FFP Post-Thaw Expiry: Expiry of adult FFP only to be extended for major haemorrhage cases from 24 to 120 hours. Guidelines to follow. OBOS HLA Platelet Ordering: Now available, but only for second and subsequent ordering. Transport Boxes for Hospitals: 3500 boxes ordered and will be validated by NHSBT. Will be trialled by one hospital first before being rolled out to others. Recalls: Recall form and accepted reasons for recall were shown. One delegate mentioned that to be able to recall you need a fax machine, but many hospitals are removing them. RW said that they are looking for alternatives, but cannot find anything suitable. Labelling of Samples: RCI and H&I samples are stored at different temperatures so it is very important to label samples properly. RW showed some examples of incorrect or poorly labelled samples. Please ensure samples are labelled correctly and clearly. Also do not

use tubes that are out-of-date as they will not be processed. Phoning RCI: RCI need to fill out a form (Record of Verbal Contacts form) for every telephone call they receive an example of the form was shown. Make sure you have the answers ready before you call them. 06/16 Where do you Find all this Transfusion Information? There is lots of transfusion information available, but people are not aware of what it is or how to find it. Hospital & Sciences Website: A very useful website with a lot of information. Contains items that have been produced and things you can order or download. JPAC (Transfusion Guidelines): Lots of resources, articles from journals plus RTC pages SHOT: Slides are updated every year. You can use their slides, cases and diagrams in your own presentation as long as you credit them in the presentation. SHOT Bites is very interesting. International Websites (via ISBT): To be able to access their sites, you mostly need to be a member of their organisation. Australian and Canadian blood services have lots of resources to use for laboratory and clinical transfusion practice 07/16 Support for Implementing NICE Guidelines JM explained about NICE s objectives, what it does, its core principles and why to use NICE guidance. She said that NICE guidelines are not statutory, but you are expected to implement them. On their website is a section Put Guidance into Practice. Also, in every Trust, there is someone who can help you if you are struggling to implement NICE guidance. There are usually four reasons why you cannot implement the guidelines find out which is your reason. Once you know the reason, use the right solution to solve it. The best way is to help it happen. Acknowledge that you will not be able to get everyone on board at the same time it will happen gradually. Do not get disheartended slow gains are better than none. Target the innovators and early adapters (Rogers (1962) Diffusion of Innovation) first and make them champions, then the others will follow. NG24 is the blood transfusion guideline. The quality standard on blood transfusion is being drafted at the moment consultation due on 20/06/16. 08/16 Just Save One Sharing Success This follows on from the presentation about wastage and the implementation of the Just Save One campaign at last April s RTC meeting. Great Ormond Street Saving of 20,000 so far Got Quality Improvement on board to assist Reduce reservation time Use up older blood first before fresh Use AB Octaplas for Group O Reduce flying squad provision Next Steps: Will audit the appropriateness of requests Allocate a daily lab wastage champion Guy s & St. Thomas Concentrated on platelet wastage as this is where the problem was Communication with staff and getting them to help is key you cannot do it by yourself Review of standing orders are they necessary? No reservations Identifying and using short-dated stock first Changing wastage codes

Electronic ordering Results: WAPI since January 2015 as shown in graph King s College Concentrated on platelet wastage in the lab and in patients Urge BMS s to query requests from clinicians Have wastage champions Manage stock and standing orders better Results: Wastage has from August 2015 to Feb 2016. Wastage is still occuring because they are a big user and some products have a short shelf life Usage has also, but their activity has increased this proves that components were being wasted into patients. Imperial Imperial operates across four labs Biggest problem was out of temperature control Got IT on board to help with collating data this really helped Have saved 100,000 in 7 months Reduction in RBC, platelet wastage and also usage They waste a lot of plasma so will look into this. Next project will be looking into O Neg. 09/16 Anti-D Provision the Current Problem with Supplies Changes to the supply of Anti-D in the UK. BPL are withdrawing the 250 unit vials of D-GAM. They allocate each hospital a certain number of units and no more. At Imperial they always have a shortfall which CSL Behring has kindly been making up. Imperial are considering changing to Rhophylac. This issue will be taken to the National Lab Managers Group. It was questioned whether hospitals should enrol for the NHSBT scheme. RW said that NHSBT can test D- mothers for a cost of 11 which is extremely accurate. It is better to do all D- mothers rather than individual patients. Erika Rutherford is the person who can explain more about it. 10/16 Ask the Audience 1. Reduction of Wastage One delegate stated that, if you reduce wastage, your usage will also go down. He mentioned it is impossible to measure a 1% reduction. 2. Electronic Tracking Royal Marsden are having problems with this. This is a future topic for discussion. 11/16 Any Other Business a) Demand for Platelets NHSBT are struggling to keep up with the demand for platelets every week. Please try to reduce platelet usage. There are lots of different strategies for saving platelets. One delegate suggested that requests for blood can only go trhough a Consultant this saved a large amount in her hospital.
