Who s Your Neighbor? Grant Application Funded by the Orton Family Foundation and Heart and Soul Cortez Who lives next door to you? Do you know? This is your opportunity to find out, and to get to know your neighbors and your neighborhood on a more personal level! The goal of this grant is, simply, to help people in our Cortez neighborhoods get to know each other. One of the values the Heart & Soul Cortez project identified was our small town feel. Everyone here likes the idea that we know one another, and that we have strong ties to both family and friends in our community. The events funded by this grant will help strengthen this value, building a framework for communication and trust in our neighborhoods. The Who s Your Neighbor? grant will supplement or cover the cost of holding small neighborhood gatherings by providing a way to socialize and an opportunity to identify neighborhood needs or concerns. Activities May be used for a variety of activities such as block parties, potlucks or ice cream socials; or seasonal events to help fund neighborhood trunk or treat activities, holiday cookie exchanges, holiday caroling, etc. Anything that brings people in the neighborhood together. Grant Summary Grants may be awarded up to a maximum amount of $300 each based on the grant application and activity receipts. Eligible grants will be awarded on a first come first served basis until the allocated funds are depleted. The event must be held on or before August 14, 2015. Applications must be submitted at least two weeks prior to the scheduled event. The last deadline for applications is July 31, 2015. Applications submitted after the activity will not be accepted. Grant Eligibility and Application Requirements Any resident or property owner within the City of Cortez is eligible to apply for a Who s Your Neighbor? grant. To facilitate neighborhood involvement, all

applications must be submitted by residents of the neighborhood rather than management entities. Applicants must submit a completed Who s Your Neighbor? grant application form to the City of Cortez Planning Department, Tracie Hughes. All applications must represent a neighborhood group (formal or informal). You will be asked to identify your neighborhood on a City map (provided) All funding must be utilized for activities that benefit the block or neighborhood and are held within the Cortez city limits. The City of Cortez cannot close streets for these events, so please consider holding the event at a common space within the neighborhood, or at a neighbor s home Reimbursement Eligible expenses may include food and supplies needed for a neighborhood gathering. Alcohol, tobacco, cannabis, and illegal substances are not eligible for reimbursement. Upon submittal of a summary evaluation form for the event and associated receipts, costs may be reimbursed up to the amount of $300. All receipts must be submitted to the City no later than August 17, 2015.

Event Budget The City of Cortez has allocated Orton Family Foundation Heart & Soul Implementation Funds to strengthen neighborhood support and improve neighborhood communication via these neighborhood events. To receive this funding, please provide your budget by completing the form below. Once your activity is over, please submit an updated budget form that reflects your actual costs along with receipts for the requested expense reimbursements.
