Review process Health Net Federal Services helps support programs and organizations that promote military and veteran health and wellness or enhance their quality of life. Requests are reviewed by the Health Net Federal Services Review Committee on a quarterly basis.* Where applicable, legal review for compliance with Health Net policies and other relevant legal and industry guidelines is completed.* Requester is informed of the funding decision via letter and email. A Sponsorship Agreement is sent to the requester for signature, then returned to Health Net Federal Services for approval. Health Net Federal Services Corporate Giving Sponsorship and donation guidelines Health Net Federal Services helps support programs and organizations that promote military and veteran health and wellness, or enhance their quality of life. The communities we serve include Veterans, active duty and National Guard and Reserve service members, family members and survivors. We accept grant applications year-round only from non-profit groups. Our priority is to support programs that will have a direct impact on our communities and constituencies. To help us choose from the hundreds of requests we receive each year, we have developed guidelines to govern our corporate giving program. Several principles form the basis of these guidelines: Health Net Federal Services corporate giving is focused on Veterans, active duty and National Guard and Reserve service members, families and survivor health and wellness, and enhancement to their quality of life. We focus on projects and programs that provide a direct service to military and Veteran individuals and communities in need. We give priority to programs that benefit the communities where our employees and constituencies live and work. We only support organizations that are fiscally sound and have strong management and accountability practices. How to Apply: Health Net Federal Services accepts applications from IRS-Qualified 501(c)(3), and 501(c)(4) non-profit organizations whose mission and programs are consistent with our areas of philanthropic interests, and which serve the communities where our employees and constituencies live and work. Proposal Submission Process We accept funding requests year-round; however, we strongly urge you to submit your application by the end of the third quarter of the year, as we use an annual budgeting process to prioritize our donations. At a minimum, funding requests should be submitted at least 12 weeks in advance of the date of required funding. Funding decisions are announced by letter and email to recipient organizations. We will contact you if we need additional information. Focus: Military and Veteran Quality of Life, and Health and Wellness Like most companies, we receive many more requests than we can fund. Because our resources are limited, we must make choices about how to allocate them. A fundamental choice we have made is to focus our efforts on areas where we believe we can make a meaningful difference. 2
Providing information as complete as possible increases your chances of receiving your requested donation. We concentrate our giving in these key focus areas: Rehabilitation or wellness initiatives for service members, Veterans and their caregivers/families Programs that support military families and Veterans in transition Initiatives to improve the quality of life and behavioral health of service members, Veterans, survivors, retirees and their families Events/programs that recognize the contributions of service members, Veterans and their families As a result, we may decline support for other worthwhile organizations and programs whose activities do not fall within our focus areas. Such a response does not reflect the value of the organization or its services. Only those proposals that fall clearly within our corporate giving mandate and follow the funding proposal process outlined will be considered for funding. Funding Proposal Requirements Each funding proposal must contain the following application elements (download an application here): The name, address, phone number, fax number and Web address of your organization. The name and email address of a contact person familiar with the proposal details An overview of your organization, including your mission statement. The title of the project or program for which you are seeking support. A summary (one page max.) of your project or program needs, including: what it is; why it is needed; whom it serves; and how it fits into one, several or all of the focus areas of Health Net Federal Services corporate giving program A proposed timeline An estimated program budget or donation/sponsorship being requested including sponsorship package details W-9 form for your organization Your latest IRS Form 990 A list of your organization s board of directors and executive officers Other evidence of meeting Health Net Federal Services eligibility criteria Restrictions To further clarify our review process, we have instituted the following limitations within our corporate giving program. Health Net Federal Services will not consider funding for the following: Individuals General operating costs For-profit organizations Arts and cultural organizations Sports teams, both professional and amateur Travel costs, including student trips and tours Educational programs not related to military or Veteran health Projects or services outside of regions or areas where we have operations 3
Organizations not registered as non-profit or charitable organizations Political organizations (Such requests are referred to our Government Relations Department for consideration.)
